Comparison of ICD-10 diagnostic guidelines and research criteria for enduring personality change after catastrophic experience.

Enduring personality change after catastrophic experience (EPCACE) is a diagnostic category included in the International Statistical Classification of Diseases and Related Health Problems, 10th revision (ICD-10), as one of the adult personality disorders. Preliminary investigation suggests there is considerable endorsement in principle for this new category amongst experts in the field of intentional human trauma, yet many aspects of the diagnosis remain contentious. EPCACE is described in 2 sets of ICD-10 criteria: the Diagnostic Criteria for Research (DCR) and the Clinical Description and D

Affective outcomes of virtual reality exposure therapy for anxiety and specific phobias: a meta-analysis.

Virtual reality exposure therapy (VRET) is an increasingly common treatment for anxiety and specific phobias. Lacking is a quantitative meta-analysis that enhances understanding of the variability and clinical significance of anxiety reduction outcomes after VRET. Searches of electronic databases yielded 52 studies, and of these, 21 studies (300 subjects) met inclusion criteria. Although meta-analysis revealed large declines in anxiety symptoms following VRET, moderator analyses were limited due to inconsistent reporting in the VRET literature. This highlights the need for future research stud

Social anxiety and posttraumatic stress in combat veterans: relations to well-being and character strengths.

There are few studies examining the relationship between psychopathology and positive experiences and traits. Although initial studies suggest persons with posttraumatic stress disorder (PTSD) are at increased risk for excessive social anxiety, there have been no studies to date evaluating how these conditions might interact to affect positive experiences and traits. Using self-report scales, informant ratings, and experience-sampling methodologies, we examined the association of social anxiety with well-being and character strengths in veterans with and without PTSD. Controlling for PTSD and

[Biological basis of anxiety disorders and serotonergic anxiolytics].

Selective serotonin reuptake inhibitors (SSRIs) have wide indications for the treatment of anxiety disorders, including panic disorder, generalized anxiety disorder, posttraumatic stress disorder, obsessive-compulsive disorder and social anxiety disorder in addition to depression. Until recently, no animal model has been available for screening the anxiolytic effect of SSRIs and studying its mechanism of action. We have investigated the relationship between serotonin neurotransmission and anxiety using conditioned fear stress (CFS), an animal model of anxiety. CFS increased serotonin neurotran

Commonality and specificity of personality disorder profiles in subjects with trauma histories.

Recently, attention has been drawn to a range of disturbances in personality functioning that commonly characterize individuals with a history of severe or prolonged trauma. Many of these features overlap with criteria for some of the Axis II personality disorders. The current study investigated the similarity of personality disorder features in different samples of patients with trauma histories, and specificity of such features compared to other psychiatric samples. Profiles of Axis II features, based on relative frequencies of individual disorder "diagnoses" derived from a common measure (P

Physical and psychological sequelae to torture. A controlled clinical study of exiled asylum applicants.

The study comprised 24 male Lebanese refugees living in Denmark. Twelve of them alleged having been tortured in Lebanon during the period 1981-85. The remaining twelve had neither been imprisoned nor tortured and thus acted as control persons. All the testimonies were found to be valid according to a method previously used by the author. The most common forms of torture were blows against the head, body and foot soles, suspension and asphyxiation. Threats and solitary confinement were frequent, and sexual violations were also reported. At the time of examination (March-November 1986), the main

Selective effects of acute and chronic stress on slow and alpha-theta cortical functional connectivity and reversal with subanesthetic ketamine.

Anxious, depressive, traumatic, and other stress-related disorders are associated with large scale brain network functional connectivity changes, yet the relationship between acute stress effects and the emergence of persistent large scale network reorganization is unclear. Using male Thy 1-jRGECO1a transgenic mice, we repeatedly sampled mesoscale cortical calcium activity across dorsal neocortex. First, mice were imaged in a homecage control condition, followed by an acute foot-shock stress, a chronic variable stress protocol, an acute on chronic foot-shock stress, and finally treatment with

Phenomenology and course of psychiatric disorders associated with combat-related posttraumatic stress disorder.

OBJECTIVE: Studies indicate that chronic combat-related posttraumatic stress disorder (PTSD) is frequently associated with other psychiatric disorders. Questions regarding the nature and interrelationships of these conditions require clarification. The purpose of this study was to address primary and secondary illness relationships by focusing on the specific phenomenology and course of illness onset of PTSD comorbidity. METHOD: In order to minimize confounding factors, only outpatients without recent substance use disorders were included. Sixty subjects who had been exposed to severe combat

Examining the prognostic utility of symptom etiology groupings on the VA's comprehensive traumatic brain injury evaluation.

Traumatic brain injury (TBI) is a prevalent condition among military service members and Veterans. As such, the Veterans Health Administration established the TBI Screening and Evaluation Program to identify and assess suspected deployment-related TBI's among post-9/11 Veterans. Veterans who screen positive on an initial four-item TBI Screen are referred for a Comprehensive Traumatic Brain Injury Evaluation (CTBIE), a structured clinical interview used to confirm TBI diagnosis and evaluate the likely etiology of current symptoms. This study examined whether clinician-rated symptom etiology cla

Effect of brief online mindfulness-based cognitive therapy in patients with digital replantation: a study protocol for a randomized controlled trial.

BACKGROUND: Traumatic amputations are common traumatic injuries that require digital replantation to restore the form, function, and sensation of the hand. The post-surgical survival of replanted digits is influenced by multiple factors, including psychological factors such as posttraumatic stress disorder (PTSD), anxiety, and depression. Mindfulness-based cognitive therapy (MBCT) is considered a potential intervention to address such emotional distress. Traditionally, MBCT involves an 8-week face-to-face group format. Due to feasibility concerns regarding implementation and patient adherence,

Associations between imaging features and PTSD risk in traumatic brain injury: An analysis of bleed location and MRI pathology.

INTRODUCTION: Traumatic brain injury (TBI) can lead to post-traumatic stress disorder (PTSD), with risk factors potentially linked to injury characteristics visible on imaging. This study examines associations between PTSD at 6 months and two imaging features: bleed location (epidural, subdural, subarachnoid, and contusions) and MRI pathology. METHODS: Using data from the TRACK-TBI Pilot Dataset, we analyzed imaging characteristics and PTSD status in 586 patients. Bleed types and MRI results were assessed in relation to PTSD rates at 6 months. We applied logistic regression for bleed location

Single-Nucleus RNA Sequencing Reveals That Gabra6+ Neurons in Prefrontal Cortex Promote the Progression of PTSD After Shockwave-Induced TBI.

Shockwave-induced traumatic brain injury (TBI) results in the onset of post-traumatic stress disorder (PTSD), triggered either by the TBI itself or other stressors. However, the interplay and underlying mechanisms of how these factors synergistically induce PTSD remain inadequately elucidated. Here, mice in the TBI (induced by biological shock tube blast injury) and PTSD (induced by single prolonged stress method) groups both displayed symptoms of PTSD behaviors, with the TBI+PTSD (composite model) group exhibiting more severe manifestations. The result of snRNA-seq demonstrated a noticeable i

Military Traumatic Brain Injury: The History, Impact, and Future.

This review examines how lessons learned from United States military conflicts, beginning with the United States Civil War through the engagements in Iraq and Afghanistan, have shaped current traumatic brain injury (TBI) care in the United States military, influenced congressional mandates and directives, and led to best practices in caring for the warfighter. Prior to the most recent war, emphasis was placed on improving the surgical and medical care of service members (SM) with severe and especially penetrating brain injuries. However, during the Iraq and Afghanistan conflicts, also known as

Vulnerability to posttraumatic stress disorder in adult offspring of Holocaust survivors.

OBJECTIVE: There has been considerable controversy regarding the impact of the Holocaust on the second generation, but few empirical data are available that systematically document trauma exposure and psychiatric disorder in these individuals. To obtain such data, the authors examined the prevalence of stress and exposure to trauma, current and lifetime posttraumatic stress disorder (PTSD), and other psychiatric diagnoses in a group of adult offspring of Holocaust survivors (N=100) and a demographically similar comparison group (N=44). METHOD: Subjects were recruited from both community and c
