OBJECTIVE: To report on a single-stage technique for hypospadias repair based on the combination of several features of established methods and to emphasize the importance of identifying and excising the chordee that is almost always present in the tissues proximal to the urethral opening to ensure the complete correction of this deformity.

Reconstruction of the penis using a free jejunum transplant with microsurgery revascularization was developed initially with an animal model. The technique was found to be successful in the dog and was applied in a 17-year-old patient. The main advantages of the technique are to permit mucomucosal anastomosis between the urethra and the jejunal transplant thus avoiding stenosis. We present a surgical procedure that provides exposure to the medial part of the orbit, the interorbital space, and the nasopharynx. A midline incision of the nose is followed by a pedicled osteotomy of the lateral nasal wall and the medial wall of the orbit in one piece. This transnasal approach provides good exposure to the medial parts of the craniofacial junction to remove foreign bodies or benign tumors.

Certain procedures for facial advancement may carry greater risk than others. Between 1975 and 1988 we observed 169 patients with carcinoma of the cervical esophagus, 85 with a carcinoma involving the hypopharynx and the cervical esophagus, and 27 patients with a carcinoma of the cervical esophageal region arising after laryngectomy for laryngeal cancer. Thirty-one patients with obstructive sleep apnea syndrome underwent uvulopalatopharyngoplasty (UPPP). After three months, the average apnea/hypopnea index (AHI) decreased from 352.2 +/- 138.2 (mean +/- SD) to 182.2 +/- 140.4 episodes/hr and the duration of oxygen desaturation (less than 85%) decreased from 38.4 +/- 48.6 to 13.2 +/- 25.8 minutes. The efficacy of scleral grafts for eyelid lengthening in patients with thyroid related upper and/or lower lid retraction was evaluated in 62 consecutive patients with Graves' ophthalmopathy who underwent lid surgery in the last 3.5 years. Distal hypospadias are the most frequently observed, and represent about 70% of all forms of this condition. We describe two children with congenital deformities due to the early amniotic rupture syndrome. One child who had an amniotic band around the upper arm suffered from a distal nerve lesion. This was relieved following surgery. The second child died shortly after birth of multiple congenital malformations. The most usual findings in this syndrome are constriction of a limb by an amniotic band, scoliosis or syndactyly due to oligohydramnion with compression. We discuss one accepted hypothesis (subscribed by an experimental animal study) of the aetiology of the early amniotic rupture syndrome.
