Cadaveric study of the posterior pedicle nasoseptal flap: a novel flap for reconstruction of pharyngeal defects and velopharyngeal insufficiency.

BACKGROUND: The posterior pedicle nasoseptal flap has been the workhorse for endoscopic reconstruction of medium to large cranial base defects, with excellent outcomes and minimal flap failures. The authors present the anatomical foundations for the use of the nasoseptal flap for reconstruction of soft palate and pharyngeal defects and for surgical treatment of velopharyngeal insufficiency in a cadaveric model. METHODS: Posterior pedicle nasoseptal flaps were endoscopically harvested and transposed to the naso/oropharynx in seven cadavers. The reach and relationships of the flap with nasophar…

Multiple vascular variations at the vicinity of the left kidney.

Although isolated variations of the renal artery, renal vein and testicular artery are commonly described in the literature, the concurrent presence of variations in all three of these vessels has seldom been reported. Here, we report concurrent vascular variations in the vicinity of the left kidney. The left renal vein was found to have a retropelvic tributary, and a communicating vein connecting the inferior vena cava and retropelvic tributary was also noted. In addition, the left renal artery separated into anterior and posterior divisions just before reaching the hilum. The anterior divisi

Survival rates among patients awaiting deceased donor liver transplants at a single high-volume Korean center.

BACKGROUND: Since the establishment of the Korean Network for Organ Sharing (KONOS) in 2000, thousands of patients have been enrolled on the waiting list, but only a small proportion have received a deceased donor liver transplantation. This report on waiting list mortality in Korea based on data from a single institution. METHODS: The 1772 patients enrolled on the waiting list between February 2000 and December 2011 either have not yet received at the time of analysis or have died before receiving an organ. Survival information was obtained in February 2012 by reviewing medical records or by

Contrast-enhanced computed tomographic evaluation of the deep digital flexor tendon in the equine foot compared to macroscopic and histological findings in 23 limbs.

REASONS FOR PERFORMING THE STUDY: Distal deep digital flexor tendinopathy is an important cause of foot lameness in horses that is difficult to diagnose with radiography and ultrasonography. Magnetic resonance imaging is a well-accepted and validated technique for the identification of deep digital flexor tendon (DDFT) lesions, but has some practical and financial drawbacks. Contrast-enhanced computed tomography (CECT) has been proposed as a suitable alternative, but validation studies are currently lacking. OBJECTIVE: To assess the accuracy of CECT for the identification and characterisation

Anatomic study of selective neurectomy of gastrocnemius muscle for calf reduction in Chinese.

BACKGROUND: Selective neurectomy of the innervating nerves of the gastrocnemius muscle is a popular method employed for calf reduction. However, accidental injury to the untargeted adjacent nerves could happen. This study aims to provide detailed morphometry of the motor branches from the tibial nerve innervating the gastrocnemius muscle, the soleus and the medial sural cutaneous nerve in the popliteal fossa. METHODS: 23 lower legs from female cadavers were dissected to explore the origin, length of and the spatial relationship between the four branches given off from the tibial nerve in the …

The surgical anatomy of the infrapatellar branch of the saphenous nerve in relation to incisions for anteromedial knee surgery.

BACKGROUND: Iatrogenic injury to the infrapatellar branch of the saphenous nerve is a common complication of surgical approaches to the anteromedial side of the knee. A detailed description of the relative anatomic course of the nerve is important to define clinical guidelines and minimize iatrogenic damage during anterior knee surgery. METHODS: In twenty embalmed knees, the infrapatellar branch of the saphenous nerve was dissected. With use of a computer-assisted surgical anatomy mapping tool, safe and risk zones, as well as the location-dependent direction of the nerve, were calculated. RE

An interconnected duplicated femoral vein and its clinical significance.

Anatomical variations in the femoral vein are of great clinical importance especially in cases of deep vein thrombosis (DVT). Knowledge of the variable anatomy of the femoral vein is important to minimise false-negative findings on ultrasound examination in patients with DVT and help to explain the 'silent' DVT. Furthermore, the presence of a duplicated femoral vein itself is associated with higher incidence of DVT. These venous anomalies are usually due to the truncular venous malformation. In the present study, while dissecting the right lower limb, we found a case of variation of the femora

Anomalous fascicle of triceps brachii muscle and its clinical importance in relation to radial nerve entrapment.

We present a case of unreported morphological variant of the triceps brachii muscle which was observed while dissecting a cadaveric upper limb at the department of human anatomy. It was noted that an anomalous fascicle present between the lateral and long heads of the triceps. The fascicle was passing just superficial to the spiral groove and was crossing the radial nerve and profunda brachii artery. After the literature search, it was found that the present variation has not been previously reported. The clinical implication of this anomalous fascicle in relation to radial nerve entrapment an…

Cadaveric anatomical comparison of the lateral nasal wall after external and endonasal dacryocystorhinostomy.

PURPOSE: Much literature has accumulated espousing the relative merits of endonasal and external dacryocystorhinostomy (DCR). However, there is comparatively little information on the relative anatomic differences between these 2 approaches. The purpose of this study is to investigate the anatomic relationships of the lateral nasal wall for endonasal and external DCR. METHODS: Ten cadaver half heads were used in this study. Half were subject to endonasal and half to external DCR procedures. The lateral nasal wall was then dissected and measurements were taken of ostium and anastomosis size an…

Endoscope-assisted supraorbital approach to the retroinfundibular area: a cadaveric study.

The purpose of this study is to investigate and evaluate the exposure and maneuverability of this areas provided by an endoscope-assisted supraorbital approach and to compare that to a microscopic supraorbital approach. We exposed microscopically the optico-carotid and the infrachiasmatic windows after a supraorbital craniotomy executed using an eyebrow incision. We then proceeded to explore the retroinfundibular area using these two windows either using the microscope alone or using the endoscope-microscope combination where the microscope was used to (1) guide instrument and endoscope insert

Intra-articular psoas tendon release alters fluid flow during hip arthroscopy.

While not proven definitively, the hypothesis that intra-articular psoas tendon release allows fluid to track into the retroperitoneal space has been widely accepted. This study attempts to identify the path through which fluid enters the pelvis and retroperitoneal space. Six hemi-pelvis human cadaveric specimens were utilized for this study. 3 specimens underwent a capsulotomy and psoas tendon release, while 3 had only a capsulotomy. Arthroscopy fluid was combined with Barium and Methylene blue, and fluid was run at 50 mmHg for 2 hours. A gross dissection was performed at the end of the arthr…

Anatomical study of fibrous structures of the medial aspect of the canine elbow joint.

Forces exerted by fibrous structures on the medial aspect of the canine elbow joint have been reported to be involved in elbow pathology. The purpose of this paper is to assess the relationships of the ligamentous and muscular structures of this region of the canine elbow joint, and how they relate to the medial coronoid process. Six cadavers of adult German shepherd cross-breed dogs were used in this study to make intra-articular and vascular injections of this region. Some joints were dissected and some were frozen to saw sagittal or dorsal cryosections to assess the relationships of the myo…

Percutaneous acetabuloplasty: a cadaveric study.

Periacetabular osteolysis is a common etiology of prosthesis failure in patients who undergo total hip arthroplasty. These lesions are treated by open and, more recently, percutaneous techniques. The purpose of this study was to determine the relevant surface anatomy and bony landmarks in establishing percutaneous access to periacetabular regions and identifying critical at-risk structures in establishing access. Percutaneous access to the periacetabular region was established superiorly, anteroinferiorly, and posteroinferiorly by using 5 L5-to-mid thigh fixed cadaver pelvises with latex-injec…

Clinico-anatomical report of a rare anomalous disposition of brachioradialis: a possible site for compressing superficial branch of radial nerve.

The topography of the neurovascular structures in the antebrachium assumes relevance during surgical approaches in this region. The aim of the present case report was to describe the clinical importance of a bifid brachioradialis with the superficial branch of radial nerve entrapped between the two slips. The insertion of the two slips was unusual and merits a special documentation in anatomy literature. We propose that the superficial branch of radial nerve could possibly be subjected to compression resulting in the neurological symptoms of varying magnitude. A precise working knowledge of th

Movement associated reduction of spatial capacity of the equine cervical vertebral canal.

Laterolateral radiographs of equine necks are reported to be inaccurate in determining the site of spinal cord lesions even when a myelogram is carried out. The goal of this study was to assess constrictions present in the cervical vertebral canal at any time point throughout the extremes of movement. Sixteen equine cervical vertebral columns without history of cervical disease were used. After removal of the spinal cord, the dura mater was filled with polyurethane foam and during its plastic phase the cervical vertebral column was passively moved in flexion-extension, lateral bending and 30°

Anatomic survey of arachnoid foveolae and the clinical correlation to cranial bone grafting.

INTRODUCTION: When performing in situ harvesting of cranial bone grafts, there is a risk of entering the pericranial-intracranial venous system, either directly or indirectly through the arachnoid foveolae. The aims of this study were to investigate the size and location of arachnoid foveolae and to provide an anatomic road map to prevent penetrating these structures. METHODS: Three hundred dry skulls were selected from the Hamann-Todd osteological collection (Cleveland, OH); skulls were collected between 1912 and 1938. Our study skulls were limited to whites or African American adults. Exclu

Preventing venous congestion of the nipple-areola complex: an anatomical guide to preserving essential venous drainage networks.

BACKGROUND: Venous congestion leading to partial or total nipple necrosis is a relatively uncommon complication of breast reduction and mastopexy procedures but still occurs, particularly in larger reduction procedures. This is largely preventable if the surgeon has an understanding of the venous drainage to the nipple and carefully preserves it. METHODS: An anatomical study was undertaken on 16 fresh female cadaveric breast specimens. The venous drainage of the breast was explored through vascular injection, radiographic, and cross-sectional studies. RESULTS: The venous drainage of the brea…
