Is there added risk in resurfacing a femoral head with cysts?

BACKGROUND: Femoral head cysts have been identified as a risk factor for early femoral failures after metal-on-metal hip resurfacing arthroplasty (HRA) based on limited scientific data. However, we routinely performed HRA if less than 1/3 of the femoral head appeared destroyed by cysts on the preoperative radiograph. This study was undertaken to analyze whether there was an added risk of early femoral failures in HRA when femoral head cysts were present. METHODS: This retrospective case-control study included 939 MOM HRAs operated by a single surgeon with use of the posterior minimally invasi

The pedicle screw fixation with vertebroplasty augmentation in the surgical treatment of the severe osteoporotic spines.

STUDY DESIGN: Retrospective clinical study. OBJECTIVE: To present the early clinical results of pedicle screw fixation augmented by vertebroplasty using polymethylmethacrylate in severely osteoporotic patients requiring spine surgery due to the neurologic deficit. SUMMARY OF BACKGROUND DATA: It is postulated that combining a formal vertebroplasty-that is, maximum filling of the trabecular space with polymethylmethacrylate-with pedicle screw placement in osteoporotic vertebrae could result in resistance to pullout forces significantly. METHODS: Between the years 2003 and 2006, pedicle screw

INvestigational Vertebroplasty Efficacy and Safety Trial (INVEST): a randomized controlled trial of percutaneous vertebroplasty.

BACKGROUND: The treatment of painful osteoporotic vertebral compression fractures has historically been limited to several weeks of bed rest, anti-inflammatory and analgesic medications, calcitonin injections, or external bracing. Percutaneous vertebroplasty (the injection of bone cement into the fractured vertebral body) is a relatively new procedure used to treat these fractures. There is increasing interest to examine the efficacy and safety of percutaneous vertebroplasty and to study the possibility of a placebo effect or whether the pain relief is from local anesthetics placed directly on

[Aseptic necrosis of the femoral head following renal transplantation: assessment of a 25-year experience].

From June 1963 to December 1988 aseptic necrosis of the femoral head has been treated surgically in 84 renal transplant recipients (150 surgical procedures). The long-term results of drilling of the neck and head of the femur (16), cup arthroplasty (32), cemented cup (1) and hemiarthroplasty (8) were unsatisfactory, as 23 of these 57 hips underwent a secondary procedure. Total hip arthroplasty progressively became the standard procedure for treatment of hip disease in transplanted patients. Since 1971, 63 renal transplant recipients underwent 92 cemented total hip replacement (THR) as a primar

Percutaneous vertebroplasty for treatment of painful osteoporotic vertebral compression fractures: an evidence-based analysis.

UNLABELLED: OBJECTIVE OF ANALYSIS: The objective of this analysis is to examine the safety and effectiveness of percutaneous vertebroplasty for treatment of osteoporotic vertebral compression fractures (VCFs) compared with conservative treatment. CLINICAL NEED AND TARGET POPULATION: Osteoporosis and associated fractures are important health issues in ageing populations. Vertebral compression fracture secondary to osteoporosis is a cause of morbidity in older adults. VCFs can affect both genders, but are more common among elderly females and can occur as a result of a fall or a minor trauma. T

An innovative approach for retreatment of prosthetically failing implant-supported fixed prostheses with a complete digital workflow: A technique report.

Complications in fixed implant prostheses are frequently encountered and can negatively impact patients' quality of life. This technique outlines a complete digital workflow for the remake of full-mouth, implant-supported, cement-retained restorations necessitated by esthetic and functional impairments. For the digital recording of reused abutments, a contemporary technique was employed in which extraorally scanned abutment data were digitally superimposed onto their corresponding counterparts in the intraoral scans. The jaw relation record procedures were completed in stages. Initially, centr

Conservative Management of Class III Invasive Cervical Resorption Using the Heithersay Technique: A Case Report.

Invasive cervical resorption (ICR) is an uncommon condition involving the gradual loss of cervical tooth structure due to the invasion of fibrovascular tissue because of predisposing factors such as trauma, orthodontics, and intracoronal restorations. A 45-year-old female patient presented with a class III cervical lesion and a missing restoration on the maxillary left lateral incisor following trauma. Radiographic examination revealed a "moth-eaten" appearance and a negative pulp test when cold testing was performed. Since it was a Class III lesion, the Heithersay technique was the choice of

A Step Beyond Convention: Three-Stage Revision with Limb Reconstruction System Fixator for Chronic Infected Total Knee Arthroplasty - A Case Report.

INTRODUCTION: Chronic periprosthetic joint infection (PJI) following total knee arthroplasty (TKA) is a limb-threatening complication, especially in the presence of sinus tract formation, bone loss, and failed prior revisions. While two-stage revision remains the standard of care, its success is limited in complex infections. CASE REPORT: We present a single case of chronic infected TKA with a discharging sinus managed successfully using a three-stage revision protocol. The treatment involved implant removal and antibiotic-loaded cement spacer insertion, interim stabilization using a Limb Rec

Nonsurgical Endodontic Treatment of Type IIIa Dens Invaginatus in an Immature Maxillary Central Incisor: A Case Report.

Dens invaginatus is a developmental anomaly resulting from the infolding of enamel and dentin into the dental pulp. Oehlers' Type IIIa dens invaginatus poses significant challenges due to its intricate anatomy and frequent association with immature teeth. This case report outlines the nonsurgical management of a nonvital immature maxillary central incisor with Type IIIa dens invaginatus, presenting with a chronic apical abscess and a history of trauma to the associated primary incisor. Treatment involved thorough chemomechanical debridement followed by bio-obturation using calcium-enriched mix

[Radial Head Arthroplasty: Pearls and Pitfalls].

Fractures of the radial head are among the more common injuries of the upper extremity. Their incidence is increasing, and these fractures are often associated with ligamentous injuries or occur as part of complex injury patterns, such as the terrible triad, Monteggia-like lesions, or Essex-Lopresti injuries. Radial head fractures are classified according to the Mason/Johnston system. In cases where the fracture is not reconstructable, or when osteosynthesis fails, radial head arthroplasty may be indicated.Several prosthetic designs are available, each offering unique features. These include d

Erythrocyte Exchange Transfusion Enabling Simultaneous Bilateral Total Hip Arthroplasty by a direct anterior approach in a Young Patient with Sickle Cell Disease.

Sickle cell disease (SCD) is a known risk factor for femoral head osteonecrosis, often leading to early joint degeneration. We report a 17-year-old male with homozygous SCD and bilateral femoral head collapse who underwent simultaneous bilateral total hip arthroplasty through a direct anterior approach. Preoperative optimization included erythrocyte exchange transfusion effectively reducing hemoglobin S from 76% to 26%. The procedure was completed without complications, and the patient recovered uneventfully, with no sickling crises or acute chest syndrome. Cemented implants were used due to p

Juan Miguel Acosta: His Revolutionary Contribution to Our Understanding of the Pathophysiology of Gallstone Pancreatitis.

It is not every day that true scientific pioneers come along. Fortunately, the early 20th century gifted us with immensely talented professionals like Dr Eugene Opie, who set the groundwork for Dr Juan Acosta and his associates to make revolutionary advancements on the pathophysiologic origin and proper management of acute biliary pancreatitis. Amidst a modest hospital in the city of Rosario, Argentina, Dr Acosta pioneered numerous studies to validate his hypothesis that transient gallstone obstruction of the lumen was the true source of acute biliary pancreatitis. His findings, along with his

Biological evaluation of ZrO2 composites modified with different ceramics additives.

In this work, zirconia (ZrO2) composites modified with bioactive hydroxyapatite (HAp), hexagonal boron nitride (hBN), bioglass (BG), and bioglass with copper (BGCu) via the hydrothermal method were synthesized. The aim was to obtain highly bioactive and cytocompatible materials that could combine beneficial properties of inert and bioactive ceramics. Such materials could be applied as fillers for tooth extraction cavities, guaranteeing osseintegration without the need to introduce additional bone cements or other adhesives. It was proven that while all materials were favourable towards cells a

Salvage Treatment for a Periprosthetic Humeral Nonunion: A Case Report.

The patient, a 69-year-old female, presented one year after receiving a total elbow arthroplasty with a nonunion periprosthetic fracture of the humerus. Due to the patient's severe osteoarthritis of the ipsilateral shoulder and significant humeral deformity, a procedure linking the total elbow arthroplasty to the reverse shoulder implant via a cemented allograft-composite linkage sleeve was performed. Previous literature suggests upper extremity salvage surgery using large-scale allografts is successful in treating large tumor or infection-derived defects, though data is lacking as to whether

Treatment of Esthetic Complications with Peri-implant Soft Tissue: A Description of the "Apical Approach" Surgical Technique and Presentation of Two Clinical Cases.

Peri-implant soft tissue deformities are an increasingly common phenomenon in the esthetic zone. While the most widely studied esthetic complications are peri-implant soft tissue dehiscences, there are other esthetic concerns in routine clinical practice that require investigation and treatment. Thus, this report on two clinical cases describes a surgical approach using the apical access technique to treat peri-implant soft tissue discoloration and fenestration. In both clinical scenarios, the defect was accessed via a single horizontal apical incision without removing the cement-retained crow

Juvenile aggressive ossifying fibroma.

Ossifying fibroma, also the cement ossifying fibroma radiographically, represents a well-demarcated mixed radiolucency/radio-opacity with smooth and often sclerotic borders. These lesions are usually solitary and most commonly seen in the mandible; cases in the maxilla have also reported. Histologically, they contain a relatively avascular cellular fibrous stroma with reticular bone trabeculae and cementum-like spherules. We present a case report of an 11-year-old boy with a 2-month history of an asymptomatic swelling seen on the left side of the angle of the mandible. An initial diagnosis of

Minimally invasive endodontics: a new era for pulpotomy in mature permanent teeth.

Emerging clinical and histologic evidence is challenging the long-established dogma that root canal treatment (RCTx) is the only therapeutic option for preservation of vital mature permanent teeth diagnosed with irreversible pulpitis or carious pulp exposure. Vital pulp therapy procedures like pulpotomy are not only technically simpler and more economical, but also afford patients a host of other benefits over conventional RCTx. This narrative review provides an update on the contemporary understanding of pulp pathophysiology and defence mechanisms, the proposed new diagnostic terminologies fo
