Minimally invasive anterior vertebroplasty for C-2 metastatic lesions.

The authors describe a technique for minimally invasive anterior vertebroplasty for treating metastatic disease of the C-2 vertebra and discuss its application in 2 cases. After a 2-cm lateral neck incision is made, blunt dissection is performed toward the anterior inferior endplate of the C-2 vertebra. An 11-gauge needle is introduced through a tubular sheath and tapped into the inferior endplate of C-2, with biplanar fluoroscopy being performed to confirm position. The needle is subsequently advanced across the fracture line and into the odontoid process. Under fluoroscopic guidance, 2 ml of

Cervical CT-guided, selective nerve root blocks: improved safety by dorsal approach.

Cervical transforaminal blocks are frequently performed as a treatment of cervical radicular pain. These blocks are performed mostly under fluoroscopy, but a CT-guided technique also has been described. We describe a modification that leads to a more extraforaminal than transforaminal and equally selective nerve root block.

How has electrophysiology changed the management of cervical spondylotic myelopathy?

Cervical spondylosis occurs universally, with cervical spondylotic myelopathy (CSM) as its most serious consequence. The electrical property of the spinal cord and its susceptibility to injury renders electrophysiology relevant to the management of CSM, as addressed in this review. Somatosensory-evoked potentials evaluate spinal cord integrity with regards to posterior column functions exclusively. Although motor-evoked potentials may be more sensitive than the former and may be utilized intraoperatively, they are susceptible to interference by inhalational anaesthetics. Electromyography may p

Adjacent segment disease after fusion for cervical spondylosis; myth or reality?

Cervical spondylosis is a common cause of radiculopathy and myelopathy, often treated by discectomy and interbody fusion. However, there has been a recent vogue for the use of artificial disc prostheses to decrease the risk of accelerated degenerative disease at adjacent levels. The short-term results of artificial disc replacements have been encouraging, but the long-term justification for using this new technology hinges on whether the incidence of adjacent segment disease decreases. It will also be necessary to demonstrate that movement at the operated levels is maintained and the incidence

Hyperextension cervical spine injuries and traumatic central cord syndrome.

Traumatic central cord syndrome (TCCS), regardless of its biomechanics, is the most frequently encountered incomplete spinal cord injury. Patients with TCCS present with disproportionate weakness of the upper extremities, and variable sensory loss and bladder dysfunction. Fractures and/or subluxations, forced hyperextension, and herniated nucleus pulposus are the main pathogenetic mechanisms of TCCS. Nearly 50% of patients with TCCS suffer from congenital or degenerative spinal stenosis and sustained their injuries during hyperextension as originally described by Schneider in 1954. Immunohisto

Craniocervical junction schwannoma mimicking occipital neuralgia.

Occipital neuralgia is a very well recognized cause of posterior headache. Although hypoesthesia may be found in the individual occipital nerve's territory, the remaining neurologic exam is typically normal. An abnormal neurologic exam is an alert for potential underlying causes of symptoms.

Homologies of the longissimus, iliocostalis, and hypaxial muscles in the anterior presacral region of extant diapsida.

Homologies of muscles of the m. longissimus and m. iliocostalis groups in the dorsal and cervical regions, as well as those of the subvertebral muscles and mm. intercostales externi that continue from the dorsal into the cervical regions, in extant Diapsida are proposed based on detailed dissections and published accounts of lepidosaurs, crocodylians, and birds. The morphology of tendons and innervation patterns suggest that the avian "m. iliocostalis" in the dorsal region include the homologs of both m. longissimus and m. iliocostalis in non-avian diapsids. The conserved nature of the morphol

Refinement of interbody implant testing in goats: a surgical and morphometric rationale for selection of a cervical level. Laboratory investigation.

OBJECT: The authors provide a surgical description of the ventral approach to the cervical spine in a goat model and identify selection of the most appropriate level for testing interbody devices. These constructs are designed for implantation in humans during anterior cervical discectomy and fusion. Such description and guidelines for level selection have never been published in either the medical or veterinarian literature. METHODS: The study comprised three phases: surgical, anatomical, and morphometric. Six goats underwent ventral approaches and were later killed; their necks were dissect

[Treatment of cervical spondylosis of vertebroarterial type with acupuncture regulating the governor vessel].

OBJECTIVE: To explore an effective therapy for vertigo due to cervical spondylosis of vertebroarterial type. METHODS: Ninety-eight cases were randomly divided into a treatment group (n = 50) and a control group (n = 48). In the control group, C3 - C7 Jiaji points (EX-B 2) and Fengchi (GB 20) were selected, and in the treatment group, Fengfu (GV 16), Dazhui (GV 14), Baihui (GV 20), etc. on the Governor Vessel were added besides the acupoints selected in the control group. Their clinical therapeutic effects were observed. RESULTS: The cured rate and the total effective rate were 56.0% and 98.0

Acute non-ambulatory tetraparesis attributable to cranial cervical intervertebral disc disease in a cat.

A 10-year-old domestic longhair cat was presented for acute non-ambulatory tetraparesis. Clinicopathologic diagnostics revealed no abnormalities. Cervical myelogram revealed an extradural compressive lesion consistent with intervertebral disc disease of the C2-C3 intervertebral disc space. Ventral slot decompression confirmed the presence of extruded intervertebral disc material into the vertebral canal of the C2-C3 intervertebral space. The patient succumbed to cardiorespiratory arrest 3 days postoperatively.

Unanticipated difficult endotracheal intubations in patients with cervical spine instrumentation.

We present two cases of unanticipated difficult airway in patients requiring reoperation after cervical spine instrumentation. In both cases, the upper airway examination was normal, and fiberoptic-guided intubation proceeded with the patient sedated and breathing spontaneously. Cord visualization was difficult, but the scope was eventually advanced into the trachea and the endotracheal tube placed safely. Later review of radiographs showed the previously unrecognized protrusion of cervical hardware into the meso- and hypopharynx. We recommend that anesthesiologists review recent radiographic

Do sagittal plane anatomical variations (angulation) of the cervical facets and C2 odontoid affect the geometrical configuration of the cervical lordosis?

Anthropometric and statistical evaluation of measurements from digitization of 252 lateral cervical radiographs were used to investigate any correlation between radiographic measurements of cervical lordosis with sagittal plane facet angulation, articular pillar height, and inclination of the C2 odontoid with respect to the body of C2. Some researchers have hypothesized that facet and odontoid architecture variations can cause a reduction in cervical lordosis. To evaluate this hypothesis, the posterior aspect of the C2 dens, vertebral body corners, and superior and inferior facet surfaces of C

Pars screw fixation of a hangman's fracture: technical case report.

OBJECTIVE AND IMPORTANCE: Traumatic spondylolisthesis of the axis may be treated by external immobilization or surgical fixation. CLINICAL PRESENTATION: We report the case of a 23-year-old man who sustained an Effendi Type II fracture of the axis, for which halo immobilization did not provide adequate stability. INTERVENTION: The unstable fracture was treated by placing lag screws in the pars interarticularis of C2, which reduced the fracture directly but sacrificed no normal spinal motion. The patient developed a solid fusion, and cervical alignment was normal at his 6-month follow-up exami

Isolated cervical esophageal duplication: a rarity.

Duplication of alimentary canal is a well-known entity seen commonly in the abdomen and the thorax. Esophageal duplications are the second most common ones after the ideal duplications, but those in the neck are rarer-only 8 cases have been seen earlier. Cervical esophageal duplication in its isolated form causing torticollis and other chronic pressure symptoms has not been reported previously. Here, the authors present a 3-year-old male patient who had cystic duplication of the esophagus in the neck associated with the cervical vertebral defect and Horner syndrome. The authors hereby discuss
