Complexities of the charitable and philanthropic landscape within Mexico's children's hospitals.

In today's Mexico, approximately 50% of children experience multidimensional poverty, with Indigenous children particularly affected; half of them live in extreme poverty and nearly all lack access to social security. The current political landscape threatens to exacerbate these issues. Reductions in public spending and the dismantling of federal child health programmes are likely to further hinder children's development, well-being and health by limiting access to vital services like childcare, education and healthcare. Children's hospitals in Mexico may increasingly depend on philanthropy an

Emergency department management of children with tracheostomies, feeding tubes, and cerebrospinal fluid shunts.

Children with medical complexity represent a growing population in emergency medicine, and many of these children depend on medical technology. This issue provides an evidence-based framework for the emergency management of children with 3 commonly encountered technologies: tracheostomies, enteral feeding tubes, and cerebrospinal fluid shunts. Distinct risks, including obstruction, dislodgement, infection, and mechanical failure, are discussed. Strategies for early recognition, stabilization, and incorporation of caregiver input are reviewed to improve outcomes and reduce medical errors. By un

Emergency department management of sentinel injuries in infants.

Sentinel injuries are often found incidentally on infants presenting with medical concerns or minor trauma as the chief complaint. Failure to recognize these findings as a potential indicator of child physical abuse may result in devastating outcomes, including disability and death. This issue reviews the most common sentinel injuries, including bruises, subconjunctival hemorrhages, oral injuries, and burns. Evidenced-based strategies are provided for evaluation and management of infants presenting to the emergency department with sentinel injuries.

Pediatric Spinal Imaging: Using Tools Wisely.

Spinal injuries in children are a clinical challenge for emergency medicine providers. On one hand, spinal injuries are relatively rare and children are at an increased risk for an oncologic process after radiation exposure. On the other hand, spinal injuries can cause significant morbidity and mortality. A thorough history and physical examination can help risk-stratify imaging assessment to detect serious injury. This evidence-based, state-of-the art review will help clinicians understand best practices in imaging decision making for cervical and thoracolumbar spinal injuries in children.

The Impact of Clinical Documentation Integrity Programs on Diagnosis Documentation.

BACKGROUND: Clinical Documentation Integrity (CDI) programs aid in documentation of precise diagnoses. In this study, the authors assessed the rate at which certain diagnoses were present in the Pediatric Health Information System (PHIS) database and evaluated changes in the hospital-specific case mix index (CMI) pre- and post-CDI program inception at six children's hospitals. METHODS: Between 2011 and 2017, CDI programs were implemented at six children's hospitals. The authors queried the PHIS database to compare the following diagnoses in 2010 and 2021: acute respiratory failure, chronic re

Addressing substance use disorder in a hospital prenatal setting.

Pregnancy can be a time of joy and hope but, for birthing parents struggling with a substance use disorder (SUD), it can be challenging. Social stigma, shame, and the potential legal ramifications present barriers to individuals seeking the care they need. Marginalized groups, in particular, face challenges that put them at even greater risk for substance misuse. Substance use during pregnancy can further impair the individual's level of functioning, and it has also been associated with problems in the social, emotional, and cognitive development of their children. Pregnancy and addiction each

A closer look to the timing of orchidopexy in undescended testes and adherence to the AWMF-guideline.

BACKGROUND: To lower the risk of testicular malignancies and subfertility, international guidelines recommend orchidopexy for undescended testis (UDT) before the age of 12-18 months. Previous studies reported low rates of 5-15% of timely surgery. Most of these studies are based on DRG and OPS code-based data from healthcare system institutions that do not distinguish between congenital and acquired UDT. METHODS: In a retrospective study data of all boys who underwent orchidopexy in a university hospital and two outpatient surgical departments from 2009 to 2022 were analyzed. The data differen

Oral health promotion in acute hospital setting: a quality improvement programme.

Tooth extraction is the most common hospital procedure for children aged 6-10 years in England. Tooth decay is almost entirely preventable and is inequitably distributed across the population: it can cause pain, infection, school absences and undermine overall health status. An oral health programme (OHP) was delivered in a hospital setting, comprising: (1) health promotion activities; (2) targeted supervised toothbrushing (STB) and (3) staff training. Outcomes were measured using three key performance indicators (KPI1: percentage of children/families seeing promotional material; KPI2: number o
