Parent/Caregiver Perspectives on the Healthcare Decision-Making Involvement of Young People With Intellectual Disability: A Qualitative Study.

BACKGROUND: Young people with intellectual disability often have complex healthcare needs. Parent/caregivers can enhance their child's involvement in healthcare decision-making to improve treatment adherence and health outcomes. Healthcare decision-making is embedded within health literacy skillsets, and for people with intellectual disability additional time and assistance are needed to ensure preferences are included. This study aims to explore parent/caregiver perspectives on the different ways that young people with intellectual disability participate in healthcare decision-making and fact

Caregiver experiences of an integrative patient-centered digital health application for pediatric type 1 diabetes care: Findings from a pilot clinical trial.

Diabetes technology generates vital health data, but healthcare professionals (HCP) and patients must navigate multiple platforms to access it. We developed a digital health platform, co-designed with patients and families living with type 1 diabetes (T1D) and their HCPs, that aim to support a collaborative care experience through shared access to diabetes data, clinical recommendations, and resources. We describe caregivers' views on the platform's impact on clinic visits and child self-management in children with T1D. A six-month observational pilot study at BC Children's Hospital Diabetes C

Herd immunity in older adults from a middle-income country: A time-series trend analysis of community-acquired pneumonia mortality 2003-2017.

BACKGROUND AND AIMS: Community-acquired pneumonia is responsible for substantial mortality, and pneumococcus is commonly accepted as a major cause of pneumonia, regardless of laboratory confirmation. Child immunization programs have reported success in decreasing pneumonia mortality: directly in young children and indirectly (herd immunity) in unvaccinated adult populations in some countries. We assess changes in mortality trends for all-cause pneumonia in older adults associated with the introduction of pneumococcal vaccination for children in Peru. METHODS: This is a secondary analysis on a

Conceptualising the empowerment of caregivers raising children with developmental disabilities in Ethiopia: a qualitative study.

BACKGROUND: Caregivers of children with developmental disabilities (DDs) in Ethiopia experience stigma and exclusion. Due to limited existing services and substantial barriers to accessing care, they often lack support. Caregiver empowerment could help address injustices that hinder their capacity to support their child as they would like. The aim of this study was to explore the meaning and potential role of empowerment for caregivers raising a child with a DD and how empowerment was situated in relation to other priorities in service development. METHODS: This was a qualitative phenomenolog

Post-Traumatic Growth in Parents following Their Child's Death in a Pediatric Intensive Care Unit.

Background: Although bereaved parents suffer greatly, some may experience positive change referred to as post-traumatic growth. Objective: Explore the extent to which parents perceive post-traumatic growth after their child's death in a pediatric intensive care unit (PICU), and associated factors. Design: Longitudinal parent survey conducted 6 and 13 months after a child's death. Surveys included the Post-traumatic Growth Inventory-Short Form (PTGI-SF), a 10-item measure with range of 0-50 where higher scores indicate more post-traumatic growth. Surveys also included the Inventory of Complicat

Modelling allocation of resources in prevention and control of obstetric fistula in Ugandan women.

In spite of reliable and skilled healthcare resources, the prevalence rate of obstetric fistula in Uganda is high. The risk factors for obstetric fistula cut across due to high poverty rates and cultural barriers. The main objective of this study was to assess the impact of inability to access skilled healthcare at delivery and implications to the economy. The specific objective was to determine the best way of investment in getting women access to skilled healthcare before, during and after child birth. The question to be answered was whether it was more economical to invest in getting women

Improved access to early infant diagnosis is a critical part of a child-centric prevention of mother-to-child transmission agenda.

BACKGROUND: Prevention-of-mother-to-child-transmission (PMTCT) programs have made it possible to achieve dramatic reductions in the rate of vertical HIV transmission. However, high attrition, particularly after delivery, has limited the impact of these interventions for HIV-exposed infants who remain at risk through the end of breastfeeding. DESIGN AND METHODS: A review of current literature on early infant diagnosis (EID) testing and country experience in low-and middle-income countries. RESULTS: While PMTCT programs report reduced rates of infection among infants tested at 2 months of age,

"Subutex is safe": perceptions of risk in using illicit drugs during pregnancy.

BACKGROUND: The dominant biomedical discourse stresses the physiological risks to the foetus or newborn posed by the prenatal use of illicit drugs. There is also a strong moral incentive for pregnant women to abstain from drugs. Yet few researchers have explored how pregnant, drug-using women themselves perceive the risks involved. The present paper investigates the reasoning by women about risks involved in prenatal drug use. Theoretically, a socio-cultural approach to risk is taken. METHODS: The paper is based on fourteen ethnographic interviews with women who had used illicit drugs during

Green space and early childhood development: a systematic review.

Background The urban environment presents significant health challenges for children, such as discouraging physical exercise and increasing exposure to air pollution, excessive noise and higher temperatures. Reducing exposures to these negative environmental factors can have great benefits on a child's well-being and lower their risk of developing chronic diseases later in life. There is increasing evidence that suggests that the presence of urban green space can offer benefits to human health and well-being. While studies have reported the impact green space exposure has on the individual hea

Systems, Structures, and Support: Using a Socioecological Framework to Improve Health Equity for Children in Immigrant Families.

Children in immigrant families (CIF) represent one in four children in the United States and face disproportionate health risks shaped by immigration policy, structural racism, trauma, and barriers to care. Using a socioecological framework, this article reviews the current state of pediatric immigrant health and outlines opportunities for pediatricians to advance equity at the individual, organizational, and structural levels. Clinicians can follow expert clinical care guidelines, practice trauma-informed care and safe documentation practices, and partner with qualified medical interpreters.

Data-Driven Sequential Uptake Pattern Discovery for Family Planning Studies.

Family planning is a crucial component of sustainable global development and is essential for achieving universal health coverage. Specifically, contraceptive use improves the health of women and children in several ways, including reducing maternal mortality risks, increasing child survival rates through birth spacing, and improving the nutritional status of both mother and children. This paper presents a data-driven approach to study the dynamics of contraceptive use and discontinuation in Sub-Saharan African (SSA) countries. We aim to provide policymakers with discriminating contraceptive u

Obese mother, obese child: A convergent mixed methods study of intergenerational obesity in low-income households in Malaysia.

Maternal and child overweight and/or obesity (OW/OB) are growing public health concerns, especially in low-income households, where they contribute to intergenerational transmission and increased financial burden. Using a convergent mixed methods design, this study aimed to investigate the determinants of intergenerational obesity among low-income families in Malaysia. Quantitative data were derived from the Malaysian National Health and Morbidity Survey for the years 2006, 2011, and 2015, while qualitative insights were obtained through in-depth interviews. The quantitative study included 2,0

Changes in Child Behaviour Following a Brief Parenting Intervention (Triple P) for Families of Children with Phenylketonuria (PKU): A Case Series.

Evaluating psychosocial outcomes in families of children with chronic health conditions is complicated by heterogeneity in impact across and within conditions. This study used goal attainment scaling to assess individual families' progress towards child behaviour goals following brief parenting support for families of children (2-11 years) with phenylketonuria (PKU). Twelve hospital-recruited families nominated up to two behaviour goals and completed baseline monitoring before attending two 2-hr Healthy Living Triple P group sessions. Monitoring was repeated at 4-weeks post-intervention and 4-
