Stopping extremity hemorrhage: More than just a tourniquet.

Major extremity hemorrhage is a surgical emergency, and the physical examination is essential to help dictate appropriate clinical decision making. Hard signs that require immediate surgical intervention include ongoing bleeding, expanding hematoma, ischemic limb, as well as partial/complete amputation. Packing, compression, balloon tamponade, and tourniquets are very helpful to temporize major hemorrhage. Mangled extremities are very challenging to manage and require a multidisciplinary approach. Temporary vascular shunts are excellent tools for vascular/orthopedic damage control and for temp

Topical sirolimus: Difference between compounded preparations and commercial oral solution.

Topical sirolimus has become a crucial treatment option for many dermatologic disorders. Because an FDA-approved topical formulation is not commercially available, sirolimus creams, ointments, and gels are professionally prepared by compounding pharmacies. Also, the topical use of a commercially available sirolimus solution approved for oral administration is described regularly. To better guide providers in their decision-making when topical sirolimus is being considered, this article highlights the substantial pharmaceutical and clinical differences between commercial oral solution and compo

[Borderline and malignant mesenchymal tumors of the abdomen].

BACKGROUND: Soft tissue sarcomas are most common mesenchymal malignancies in the abdomen. Their radiologic features are often unspecific and establishing a differential diagnosis to other pathologies can be difficult. OBJECTIVES: This review presents a selection of the most common mesenchymal abdominal tumors and their imaging characteristics, as well as the associated differential diagnoses. Knowledge of fundamental pathologic, anatomic and radiologic features of these malignancies is crucial for the differential diagnosis and key in clinical decision making and subsequent selection of an ap

Disclosing Secondary Findings from Pediatric Sequencing to Families: Considering the "Benefit to Families".

Secondary findings for adult-onset diseases in pediatric clinical sequencing can benefit parents or other family members. In the absence of data showing harm, it is ethically reasonable for parents to request such information, because in other types of medical decision-making, they are often given discretion unless their decisions clearly harm the child. Some parents might not want this information because it could distract them from focusing on the child's underlying condition that prompted sequencing. Collecting family impact data may improve future policy determinations.

Vaginal vault dehiscence and increased use of vaginal vault brachytherapy: what are the implications?

Literature reports describe the potential for increased incidence of vaginal vault dehiscence after minimally invasive surgery, and incidental reports of vaginal vault dehiscence with vaginal vault brachytherapy. This review explores the risk of increased vaginal complications in a setting of greater use of both minimally invasive surgical techniques and adjuvant vaginal vault brachytherapy in early endometrial cancer. The impact of associated patient-related and tumor-related risk factors on clinical decision making is evaluated in selecting therapy that provides optimal tumor control while m

[Epidemiology of dizziness and vertigo].

Dizziness and vertigo rank among the most common symptoms in medical practice and belong to the 10 most common reasons for a neurological examination. Epidemiological data on dizziness, vertigo and underlying specific disorders of vestibular origin are useful for clinical decision making, may contribute to a better understanding of disease mechanisms and help evaluate the state of patient care. This article gives an overview on the epidemiology of dizziness/vertigo and of four specific vestibular disorders: benign paroxysmal positional vertigo, vestibular migraine, vestibular neuritis and Meni

Laboratory testing in ethanol, methanol, ethylene glycol, and isopropanol toxicities.

Toxicity from ethanol, methanol, ethylene glycol, and isopropyl alcohol varies widely, and appropriate use of the available laboratory tests can aid in timely and specific treatment. Available testing includes direct measurements of serum levels of these alcohols; however, these levels often are not available rapidly enough for clinical decision making. This article discusses the indications and methods for both direct and indirect testing for ethanol, methanol, ethylene glycol, and isopropanol toxicity. Also discussed are the costs, availability, and turn-around times for these tests.

One normal angiogram is not always diagnostic after subarachnoid hemorrhage: case report and review of the literature.

How many angiograms should be obtained after subarachnoid hemorrhage if the first one fails to identify an aneurysm? We recently encountered this common clinical problem: a patient with subarachnoid hemorrhage confirmed by computed tomography required three angiograms to identify an aneurysm at the posterior communicating artery. Review of the literature reveals conflicting recommendations as to proper management of this common and sometimes perplexing problem. Decision-making must be applied on a case-by-case basis, but persistence is probably warranted when the index of suspicion is high.

Another look at acute appendicitis in tropical Africa: and the value of laparoscopy in diagnosis.

Acute appendicitis is believed to be one of the commonest causes of the acute abdomen in tropical Africa. Negative appendicectomy rates are usually above 20%, which is now considered unacceptably high because of increased risk to patients and the availability of diagnostic facilities to aid clinical decision-making. Our negative appendicectomy rate over a 4-year retrospective period was 29.7% in males, and 47% in females. These reduced to 11% and 10%, respectively, after the introduction of laparoscopy for doubtful cases of acute abdominal pain.

Antibiotics in Mucogingival Surgery for Recession Treatment: A Narrative Review.

Gingival recession is a common problem, particularly affecting oral health and esthetics, and its treatment involves surgical root coverage procedures. The aim of this narrative review is to evaluate the role of systemic antibiotic therapy in mucogingival surgery for recession treatment. The available literature does not support routine antibiotic use in systemically healthy patients undergoing recession coverage surgery. Indications for prophylactic antibiotics are restricted to individuals at high risk of infective endocarditis and immunocompromised patients with elevated susceptibility to s

Evaluation and Management of Aortic Stenosis in Chronic Kidney Disease: A Scientific Statement From the American Heart Association.

Aortic stenosis with concomitant chronic kidney disease (CKD) represents a clinical challenge. Aortic stenosis is more prevalent and progresses more rapidly and unpredictably in CKD, and the presence of CKD is associated with worse short-term and long-term outcomes after aortic valve replacement. Because patients with advanced CKD and end-stage kidney disease have been excluded from randomized trials, clinicians need to make complex management decisions in this population that are based on retrospective and observational evidence. This statement summarizes the epidemiological and pathophysiolo

ELSO 2025 Narrative Guideline on the Use of ECMO for Accidental Hypothermia.

These guidelines on extracorporeal membrane oxygenation (ECMO) are intended for educational use to build the knowledge of physicians and other health professionals in assessing the conditions and managing the treatment of patients undergoing ECMO and describe what are believed to be useful and safe practices for ECMO. Guidelines aim to help clinicians make informed decisions about their patients. However, adherence to a guideline does not guarantee a successful outcome. Healthcare professionals must make their own treatment decisions about care on a case-by-case basis, after consultation with
