Attaining altered states of consciousness is described as a basic human motive. The substance dependent population is distinguished from other populations because they pursue these states destructively by inappropriate use of alcohol and drugs. We observed locomotion and motion sickness in 10 normal adults wearing reversing goggles while moving outdoors. Horizontal reversal of the visual field produced moderate to severe ataxia and motion sickness in all subjects except one. There was marked variability in the sensitivity among different subjects. Acute renal failure (ARF) is not listed as a usual form of presentation in hypoxanthine-guanine phosphoribosyltransferase deficiency, despite the gross uric acid overproduction in the defect. Five cases are presented of bilateral simultaneous rupture of the quadriceps tendon and the English literature of six isolated case reports is reviewed. All the patients were men, and most were over 50 years old. Neurological and psychiatric side effects of cimetidine are reviewed in 47 cases from the literature, and 3 further cases are described. Confusion, psychomotor restlessness, hallucinations and disorientation, stupor and coma were the main features; some had convulsions and a few exhibited focal neurological deficits or neuropathies. This paper describes how fears develop on the basis of one or other of two distinct processes--direct autonomic conditioning or the establishment of new cognitive associations to already existing fears. The dichotomy relates both to the appropriate fears of everyday life and to neurotic fears. Seventeen children with unilateral hyperlucent lungs were referred for investigation. Of the 11 who had a referring diagnosis of possible Macleod's syndrome only two were shown to have post-viral bronchiolitis. Three of the 11 had conditions that required surgical treatment and a further two with brochiectasis were treated medically.
