Autoimmune progesterone dermatitis: diagnostic relevance and epidemiological analysis of 13 cases.

OBJECTIVE: Autoimmune progesterone dermatitis (APD) is a rare hypersensitivity to high levels of progesterone, with an as-yet unknown etiology. The objective of this study is to gather epidemiological data on patients with APD at an outpatient dermatology clinic. METHODS: Medical records of patients diagnosed with APD, from menarche to menopause, seen at the dermatology clinic between 2004 and 2020 were reviewed. RESULTS: Thirteen patients with APD were included. The mean age of symptom onset among APD patients was 24 years. The most prevalent clinical manifestation was urticaria, and the mo…

Patient treatment needs in atopic dermatitis: A real-world cohort study.

BACKGROUND: Atopic dermatitis (AD) substantially affects quality of life, yet standardized severity measures may not capture patient-centred priorities, making systematic needs assessment essential for personalized care. OBJECTIVES: This study aimed to systematically assess and rank treatment needs in adults with moderate to severe AD, identify patient subgroups with distinct need profiles and evaluate how well these needs were met after 2 years of specialized dermatological care. METHODS: In this prospective observational study from the TREATgermany registry, 697 adults with moderate to sev…

Acute radiation dermatitis during head-and-neck radiotherapy: VMAT vs IMPT.

BACKGROUND: Acute radiation dermatitis (ARD) is common during head-and-neck radiotherapy (RT) and may impair skin-related quality of life (QoL). Whether intensity-modulated proton therapy (IMPT) improves patient-reported skin outcomes compared with volumetric modulated arc therapy (VMAT) remains uncertain. We compared longitudinal outcomes of skin toxicity between RT modalities and identified significant predictors of ARD and skin-related QoL. METHODS: This prospective and non-randomized cohort study enrolled adults with newly diagnosed head-and-neck cancer receiving curative-intent VMAT or I…

Pulsed Dye Laser for Sensory Dysesthesias Associated With Chronic Radiation Dermatitis.

Chronic radiation dermatitis (CRD) is a long-term complication of radiation therapy, typically manifesting as scarred atrophic plaques with telangiectasias. In some cases, patients report persistent sensory dysesthesias such as burning, stinging, and pruritus. These symptoms are underrecognized and likely underreported but can significantly impair quality of life and are often refractory to conventional therapies. Pulsed dye laser (PDL) has demonstrated efficacy in treating CRD-associated telangiectasias, but its role in alleviating sensory symptoms has not been reported. We report 2 breast ca…

Delgocitinib Cream for Chronic Hand Dermatitis: Efficacy, Safety and Clinical Application.

Chronic hand dermatitis (CHD), also referred to as chronic hand eczema, is a persistent inflammatory skin disorder characterized by erythema, fissuring, scaling, lichenification, and significant pruritus and pain. The condition is often debilitating, particularly because the hands are constantly exposed to mechanical friction, detergents, irritants, and environmental stressors. Traditional treatments such as topical corticosteroids, calcineurin inhibitors, phototherapy, and systemic immunomodulators are limited by safety concerns, incomplete efficacy, or poor long-term tolerability. Delgocitin…

Altered brain connectivity in sensory and motor cortices underlying atopic dermatitis.

BACKGROUND: Atopic dermatitis (AD) is a chronic inflammatory skin disorder characterized by persistent itching. Growing neuroimaging evidence suggests that chronic itching involves altered brain connectivity within sensorimotor networks. This study aimed to investigate alterations in intrinsic brain connectivity in patients with AD compared to healthy controls, and to assess their association with symptom severity using resting-state functional magnetic resonance imaging (fMRI). METHODS: We defined several regions in sensorimotor and other relevant networks as seeds and compared seed-to-whole…

Skin Lipid Dysregulation in Atopic Dermatitis and Related Inflammatory Skin Diseases.

Epidermal lipids are essential for skin barrier function and actively influence cutaneous immune homeostasis. Recent advances have transform … , long linked to ceramide (CER) deficiency, is actually the β-subunit of acid ceramidase. In atopic dermatitis, this enzyme shifts its substrate specificity to sphingomyelin, depleting the barrier lipid pool. Type 2 cytokines, particularly interleukin (IL)-4 and IL-13, suppress lipid biosynthesis through Janus kinase (JAK)/Signal Transducer and Activator of …

Polynucleotides Attenuate Atopic Dermatitis-like Inflammatory Signaling in Keratinocytes and Macrophages.

Background: Atopic dermatitis (AD) is a persistent and recurring skin disease characterized by epidermal barrier dysfunction, immune dysregulation, and elevated expression of proinflammatory mediators. We investigated the anti-inflammatory potential of polynucleotides (PN), highly purified DNA biopolymers isolated from salmonid gonads, in keratinocyte and macrophage activation models. Methods: RAW 264.7 macrophages were stimulated with lipopolysaccharide (LPS), whereas HaCaT keratinocytes were stimulated with tumor necrosis factor-α (TNF-α) and interferon-γ (IFN-γ). The effects of PN treatment…

Interstitial Granulomatous Dermatitis Associated With Ulcerative Colitis: A Rare Cutaneous Manifestation.

Interstitial granulomatous dermatitis (IGD) is a rare inflammatory dermatosis that has been associated with several autoimmune conditions, including inflammatory bowel disease. Although cutaneous manifestations are well-recognised in ulcerative colitis, IGD remains an uncommon and under-recognised extraintestinal manifestation that can mimic other inflammatory skin disorders, leading to diagnostic uncertainty. This case discusses a 36-year-old woman with a background of ulcerative colitis who presented with erythematous lesions affecting both forearms. Due to the clinical appearance of the les…

Educational Artificial Intelligence Software to Support Assessment of Atopic Dermatitis Severity.

Accurate assessment of the severity of atopic dermatitis is crucial for guiding treatment. However, the Eczema Area and Severity Index (EASI), a widely used clinical assessment tool, relies on subjective visual scoring, which can lead to inter-rater variability. This study aimed to evaluate the performance of convolutional neural network-based artificial intelligence software in assessing atopic dermatitis severity from uncropped, unmasked skin images acquired under uncontrolled conditions. In this prospective, multicenter observational study involving 16 Japanese institutions, 1016 patients w…

Macrophage INSIG1 deficiency drives psoriasiform dermatitis via the SREBP2-STAT1 axis.

Psoriasis is an autoimmune skin disease, and its pathogenesis remains incompletely understood. As central drivers of psoriasis pathogenesis … skin. In vivo, myeloid-specific Insig1 deficiency exacerbates imiquimod (IMQ)-induced psoriasiform dermatitis, characterized by increased epidermal thickening and immune infiltration. By activating sterol regulatory element-binding protein 2 (SREBP2), INSIG1 deficiency leads to the transcriptional upregulation of STAT1, which drives M1 macrophage polarizati …
