Metabolic dysfunction-associated fatty liver disease and cardiac remodeling in non-ischemic dilated cardiomyopathy.

BACKGROUND AND AIM: Metabolic dysfunction-associated fatty liver disease (MAFLD) represents systemic metabolic dysfunction and is significantly associated with cardiovascular disease. However, its specific impact on cardiac remodeling in non-ischemic dilated cardiomyopathy (NIDCM) remains unclear. This study aimed to investigate the impact of MAFLD and liver fibrosis on biventricular remodeling in patients with NIDCM. METHODS AND RESULTS: We evaluated 293 NIDCM patients using comprehensive echocardiography. Liver fibrosis was assessed via the NAFLD Fibrosis Score (NFS). Associations were anal

Paclitaxel-Coated Balloon Dilation for Adults With Recurrent Bulbar Urethral Stricture: A Health Technology Assessment.

BACKGROUND: Bulbar urethral stricture is the narrowing of the bulbar segment of the urethra, which causes urinary symptoms and difficulty in voiding. Surgical urethroplasty is the gold standard treatment, but usually the first-line treatment is using either a simple (uncoated) balloon, a rigid dilator, or performing direct vision internal urethrotomy, which uses a blade or laser to make a cut in the stricture. Treatment with a balloon that is coated with paclitaxel has been offered as a second-line treatment when the stricture recurs. This health technology assessment looked at how safe, effec

Can Acupuncture Be Effective in Treating Insufficiently Dilated Cervix Following Intrapartum Uterine Torsion in Cows?

BACKGROUND: This prospective clinical field study evaluated the effect of acupuncture on cervical dilatation following manual correction of intrapartum uterine torsion in cows. METHODS: A total of 93 cows of different breeds and ages diagnosed with intrapartum uterine torsion were randomly assigned to two groups: Group I (n = 45), which received standardised acupuncture prior to retorsion, and Group II (n = 48), which served as a control without acupuncture treatment. Acupuncture was applied approximately 15 min before the initiation of the retorsion procedure. During this interval, all anima…

The bidirectional genetic causality between immunocyte phenotypes and dilated cardiomyopathy: A bidirectional Mendelian randomization study.

The association between immunocyte phenotypes and dilated cardiomyopathy (DCM) has been explored, however the exact pathogenesis of the relationship between immune cells and DCM is unclear. This bidirectional two-sample Mendelian randomization (MR) research aims to further validate the causal link between 731 immunocyte phenotypes and DCM. Summary statistics from a genome-wide association study data of individuals with European ancestry were utilized, including 1444 DCM cases and 353,937 controls, as well as 3757 European adults for the 731 immunocyte phenotypes. Causal effects were estimated

Neo-Aortic Root Dilation in Pregnancy After Arterial Switch for Complete Transposition of the Great Arteries.

BACKGROUND: The arterial switch operation (ASO) is standard treatment for D-transposition of the great arteries, and neo-aortic dilation is common afterward. Its behavior during pregnancy remains uncertain, especially the risk of dissection. CASE: We describe a 30-year-old woman (G6P0A5) with neo-aortic dilation (44 mm) during her first pregnancy, which remained stable on beta blockers until an uncomplicated caesarean delivery. In her subsequent pregnancy, an initial measurement of 47 mm raised concern. Expert review highlighted the challenges of accurately measuring asymmetric neo-aortas pos

Safety and Tolerability of Chronic Carvedilol in Pediatric Dilated Cardiomyopathy: A Report from the Pediatric Cardiomyopathy Registry.

Long-term studies on safety of beta-blockers in pediatric heart failure (HF) are scant. We describe the safety profile, dose ranges, and tolerability of carvedilol in pediatric dilated cardiomyopathy (DCM). The Pediatric Cardiomyopathy Registry (PCMR) was used to identify patients with DCM, treated with carvedilol from 1997 to 2005. Descriptive statistics were analyzed and echocardiographic parameters were compared longitudinally using ANOVA. Total 118 patients were included with median age 4.4 years (interquartile range (IQR) 1.0-13.3). The etiology of DCM was idiopathic (66%), myocarditis (1

Evaluating and comparing the signalling and functional landscape of hiPSC-CMs derived from patients with dilated cardiomyopathy.

Non-ischaemic or dilated cardiomyopathy (DCM) is characterised by abnormal enlargement of the left ventricle, compromising the ability of the heart to pump blood to the body. All patients with DCM are offered the same treatment regimen regardless of individual differences, with highly variable results on disease progression. Some patients fully recover cardiac function, while others continue to deteriorate, requiring heart replacement therapy or palliation. Incomplete molecular knowledge of dilated cardiomyopathy pathophysiology poses challenges for the discovery of new therapeutic agents. To

Dual Intracardiac Thromboses Associated With Dilated Coronary Sinus and Persistent Left Superior Vena Cava.

BACKGROUND: Persistent left superior vena cava (PLSVC) is associated with coronary sinus (CS) thrombosis. CASE SUMMARY: An 85-year-old woman with PLSVC presented with fever due to acute pyelonephritis. Transthoracic echocardiography revealed a mural thrombus within a dilated CS, with a contiguous free-floating thrombus extending into the right atrium. Anticoagulation with warfarin resulted in resolution of the free-floating component; however, the mural CS thrombus persisted. DISCUSSION: This case illustrates the interplay between congenital venous anomalies and transient hypercoagulability

Sudden cardiac arrest in an athlete due to dilated cardiomyopathy masquerading as athlete's heart: a case report.

BACKGROUND: Sudden cardiac arrest (SCA) in athletes is a rare but catastrophic event, most often associated with underlying cardiac pathology. Differentiating physiological remodeling ("athlete's heart") from pathological conditions such as cardiomyopathy or channelopathy remains a major clinical challenge. CASE SUMMARY: A 25-year-old professional athlete presented with recurrent syncope and was found to have frequent premature ventricular complexes with polymorphic ventricular tachycardia and prolonged QT interval. She developed cardiac arrest due to torsades de pointes and was successfully

Optimised two-stage technique for sheath-dilator insertion in peripherally inserted central catheter placement.

BACKGROUND: Peripherally inserted central catheters (PICCs) provide a valuable alternative to traditional central venous access, offering smaller-diameter lines and supporting prolonged infusion therapy. Although recent advancements have addressed many of the initial challenges of PICC insertion, some catheter-related complications remain significant. In cases of mechanical complications, shearing of the peel-away sheath typically occurs while inserting the sheath-dilator, necessitating further dermatotomy and thereby increasing patient morbidity. MATERIALS AND METHODS: The alternative techni

Tooth segmentation and dental crowding diagnosis using two-stage dual-dilated graph convolution.

PURPOSE: Tooth segmentation and diagnosis of dental crowding severity on 3D intraoral scan models are key processes for computer-aided analysis of orthodontic models. Conventional methods are time-consuming, inefficient, and subjective, necessitating more efficient and intelligent approaches. Therefore, we propose a two-stage intelligent workflow. METHODS: In Stage 1, tooth segmentation is performed using an innovative dual-dilated graph convolutional network 1 (DDGCNet1). In Stage 2, Stage 1's output is converted to a point cloud, then processed by DDGCNet2 and post-processing to generate ar

Systematic Review, Meta-Analysis, and Population Study to Determine the Biologic Sex Ratio in Dilated Cardiomyopathy.

BACKGROUND: Dilated cardiomyopathy (DCM) appears to be diagnosed twice as often in male than in female patients. This could be attributed to underdiagnosis in female patients or sex differences in susceptibility. Up to 30% of cases have an autosomal dominant monogenic cause, where equal sex prevalence would be expected. The aim of this systematic review, meta-analysis, and population study was to assess the sex ratio in patients with DCM, stratified by genetic status, and evaluate whether this is influenced by diagnostic bias. METHODS: A literature search identified DCM patient cohorts with d

Recent Trends in Achievement Rates and Time Required for Left Ventricular Reverse Remodeling in Dilated Cardiomyopathy.

BACKGROUND: Left ventricular reverse remodeling (LVRR) is associated with a good prognosis in patients with dilated cardiomyopathy (DCM), so in this study we examined the achievement rates of LVRR, the time taken to LVRR and the factors associated with LVRR in recent cases of DCM. METHODS AND RESULTS: We enrolled 121 patients with DCM. LVRR was defined as a left ventricular ejection fraction ≥40% at follow-up with a ≥10% improvement. LVRR was observed in 82 patients (68%). The median time to LVRR was 208 days. Multivariate analysis revealed that B-type natriuretic peptide (BNP) levels at disc
