[Principles of controlled clinical trials].

The recovery of the patient should be facilitated as the result of therapeutic research. The basic rule for every therapeutic-clinical trial mist involve a comparison of therapeutic approaches. In acute conditions, such as acute infectious diseases, infarcts, etc., comparisons should be made between two or more groups: the collective therapeutic comparison = the between patients trial. The formation of groups, to be compared one with the other can be justified only if one is reasonably sure that a pathogenic condition indeed exists. In chronic diseases, which extend essentially unchanged over

Neuromuscular Blockade Depth and Oculocardiac Reflex in Pediatric Strabismus Surgery: A Randomized Clinical Trial.

BACKGROUND: Although neuromuscular blockade reduces oculocardiac reflex (OCR) incidence compared with no neuromuscular blockade, the effect of blockade depth on OCR has not been prospectively evaluated. This study aimed to evaluate whether the depth of neuromuscular blockade influences the incidence of the OCR during pediatric strabismus surgery. METHODS: In this prospective, double-blinded randomized controlled trial, 204 pediatric patients aged 3 to 18 yr undergoing strabismus surgery under general anesthesia were randomly assigned to moderate to deep neuromuscular blockade (MD-NMB; train-o

Ianalumab for the Treatment of Patients with Sjögren's Disease.

BACKGROUND: Sjögren's disease is a complex, systemic autoimmune disease with no approved targeted therapies. Ianalumab is an IgG1 monoclonal antibody with a dual mechanism of action, depleting B cells through antibody-dependent cellular cytotoxicity and inhibiting B-cell activation and survival through blockade of B-cell activating factor receptor. METHODS: We conducted two 52-week, double-blind, randomized, placebo-controlled trials involving patients with active Sjögren's disease. Patients in the NEPTUNUS-1 trial were assigned in a 1:1 ratio to receive subcutaneous ianalumab (300 mg monthly

Load and breath-hold: impact of 7-day creatine monohydrate loading on breath-hold exercise performance - a double-blind randomized controlled trial.

BACKGROUND: Creatine supplementation enhances anaerobic energy availability and buffering capacity; however, its impact on muscle work under voluntary breath-hold (BH) remains unexplored. BH exercise increases reliance on anaerobic ATP production and creates a metabolic environment in which phosphocreatine-derived ATP may be particularly valuable. The aim of this study was to determine whether creatine monohydrate loading improves BH exercise performance in trained BH divers. METHODS: A double-blind, randomized, placebo-controlled study assessed the effects of a 7-day creatine monohydrate loa

Post-Exercise R-1,3-Butanediol did not Improve Markers of Fatigue or Recovery During a Multi-Stage Mountain Bike Race.

Evidence suggests that exogenous ketosis (EK) induced by ketone monoester (KME) supports recovery and attenuates physiological stress during endurance exercise. However, the potential benefits of the ketone precursor R-1,3-butanediol (BDO) during real-world multi-stage cycling remain unknown. We investigated the effects of post-exercise BDO supplementation on physiological strain, dietary intake, hydration, cognitive function, sleep, and subjective recovery during the 2025 Absa Cape Epic mountain bike race. In this double-blind, placebo-controlled study, 18 trained mountain bikers (10 teams) c

Short-term lithium treatment modulates excitation/inhibition balance in resting-state electroencephalography (EEG) among survivors of medically severe suicide attempts.

Short-term lithium treatment may reduce impulsivity and suicide risk by stabilizing cortical signaling in high-risk individuals. The present double-blind, placebo-controlled, crossover study enrolled survivors of medically severe suicide attempts and recorded 2-min eyes-open resting-state electroencephalography at baseline (drug-naïve), after 4 weeks of lithium carbonate, and after 4 weeks of placebo (n = 10 completers). Analysis focused on spectral power across traditional frequency bands (delta through gamma) and the aperiodic slope of the power spectrum, a marker of excitation/inhibition ba

Gut microbiota modulation via repeated donor fecal transplantation improves motor and gastrointestinal symptoms in drug-naïve Parkinson's disease: a randomized phase 2 trial.

The gut-brain axis is increasingly recognized as a critical contributor to Parkinson's disease (PD) pathogenesis, yet the therapeutic impact of microbiota modulation remains unclear due to lack of clinical trials in drug-naïve patients. We conducted a randomized, double-blind, placebo-controlled phase 2 trial to evaluate the safety, tolerability, and efficacy of repeated donor fecal microbiota transplantation (dFMT) in de novo PD. FMT was administered for seven days (200 mL on days 1-3; 50 mL on days 4-7) per 4-week cycle. Seventy-two patients were randomized 1:1 to receive dFMT or autologous

N-acetylcysteine reduces incidence and duration of linezolid-associated thrombocytopenia in critically ill patients: A randomized controlled trial.

Linezolid-associated thrombocytopenia (LAT) limits its use in critically ill patients. This double-blind, randomized, placebo-controlled trial evaluated intravenous N-acetylcysteine (IV NAC) for LAT prevention in 250 critically ill adults receiving linezolid for ≥ 48 h. Patients received IV NAC (600 mg every 12 h) or placebo. The primary endpoint was LAT incidence (platelet count <150 × 10 ³/mm³ or >50 % reduction from baseline). NAC significantly reduced LAT incidence (16.8 % vs. 41.6 %; p < 0.001), platelet transfusions (1.6 % vs. 11.2 %; p = 0.003), and linezolid discontinuations (6.4 % vs.

A single session of high-definition transcranial direct current stimulation does not modulate effects of knee two-point discrimination training or sensorimotor function in healthy adults: Double-blind randomised controlled trial.

BACKGROUND: Benefits of sensory-training interventions on sensorimotor outcomes are inconsistent, and it is unclear whether transcranial direct current stimulation (tDCS) can enhance proprioception, which is fundamental to neuromuscular control. Existing evidence is dominated by upper-limb studies, so transferability to the knee is unclear. This study investigated whether a single session of anodal high-definition (HD)-tDCS, alone or combined with brief two-point discrimination (TPD) training, enhances knee sensorimotor function and performance in healthy adults. METHODS: In a double-blind ra

Motor imagery-based neurofeedback in older adults: neural signatures and feasibility in a randomized controlled trial targeting age-related cognitive decline.

BACKGROUND: Neurofeedback (NF) is a non-invasive endogenous stimulation technique that enables individuals to voluntarily modulate brain activity, which has shown potential to induce neuroplasticity. Among its promising applications is cognitive enhancement in healthy older adults to prevent age-related cognitive decline. However, its efficacy remains controversial due to methodological limitations and a high proportion of individuals unable to achieve effective self-regulation, known as non-responders. This study aimed (1) to evaluate the feasibility of motor imagery (MI)-based NF training in

Feasibility and safety of transcranial direct current stimulation in the treatment of adolescent depression in a naturalistic inpatient setting: a double-blind randomized controlled trial.

Transcranial direct current stimulation (tDCS) has potential as a treatment for adult depression, but its effectiveness in adolescents remains unexplored. This study evaluated the feasibility, safety, and efficacy of tDCS in young in-patients with depression. In a randomized, double-blind, sham-controlled trial, 34 adolescent in-patients (mean age 15.48 years) received ten sessions of either tDCS or sham stimulation over two weeks, alongside standard treatment. Depression severity, quality of life, emotional and behavioral issues, and executive functions were assessed before, immediately after

Neurofeedback of somatosensory alpha oscillations modulates sensory and affective pain through dissociable neurocognitive pathways.

Elevated somatosensory α-oscillations are associated with reduced pain sensitivity, motivating efforts to modulate these rhythms for analgesia. Neurofeedback enables voluntary regulation of neural activity through real-time feedback. Here, we examined whether neurofeedback targeting somatosensory α-oscillations modulates pain and explored the associated neurocognitive processes. In a double-blind, sham-controlled design, healthy participants ( n = 80) underwent neurofeedback training during capsaicin-induced tonic pain. Real neurofeedback provided feedback contingent on contralateral somatosen

Infra-low-frequency neurofeedback alters EEG network efficiency: exploratory evidence from healthy volunteers.

Infra-Low-Frequency Neurofeedback (ILF-NFB) combines classic frequency-band (FB) and infra-low-frequency (ILF) EEG components in implicit training protocols and is increasingly applied in clinical contexts. Yet, the neurophysiological mechanisms underlying ILF-NFB remain to be further elucidated. In this randomized, sham-controlled and double-blind study, we explored the online impact of a one-session ILF-NFB application on EEG correlates in healthy participants (39 analyzed datasets). Continuous 31-channel EEG was recorded during verum and sham feedback in a double-blind, randomized crossover

Safety and Pharmacokinetics of SRN001, a Novel siRNA Drug Targeting Amphiregulin via the SAMiRNA Platform.

INTRODUCTION: SRN001 is a small interfering RNA (siRNA) drug targeting amphiregulin. It was developed using a novel SAMiRNA® platform, in which the siRNAs are protected within a self-assembled micelle. This platform eliminates the need for additional excipients to reduce unintended immune reactions. Amphiregulin plays a critical role in tissue repair and immune modulation. By silencing it, SRN001 is expected to be a therapeutic option for various pathologically fibrotic and cancerous diseases. This study aimed to evaluate safety and pharmacokinetics (PK) of SRN001 in humans for the first time.
