Idiopathic membranous nephropathy in a patient diagnosed with IgG4-related disease: A case report.

RATIONALE: Immunoglobulin (Ig) G4-related disease (IgG4-RD) is a newly recognized, systemic disease. Membranous nephropathy is the most common glomerular lesion in IgG4- related kidney disease. However, the lack of relationship with IgG4-related kidney disease and monoclonal gammopathy of undetermined significance (MGUS) warrants investigation of the potential mechanisms. PATIENT CONCERNS: A 62-year-old patient was diagnosed with IgG4-RD, tubulointerstitial nephritis, retroperitoneal fibrosis. After 2 years, she was presented with proteinuria, hypoproteinemia, facial, and bilateral lower limb

Ocular Findings in Pontine Tegmental Cap Dysplasia.

PURPOSE: To describe the ocular complications experienced by patients with pontine tegmental cap dysplasia (PTCD) and the management strategies used to care for these children. METHODS: Subjects with PTCD were recruited through social media advertisement and completed a survey gathering information on potential ocular problems related to the patient's PTCD disease and any current or previous treatments. RESULTS: Twenty-two patients or guardians completed the survey. Neurotrophic cornea was the most common ocular diagnosis (82%), followed by facial palsy (59%), dry eye syndrome (59%), and ble

Surgically-Relevant Anatomy of the External Auditory Canal Bulge and Scutum.

HYPOTHESIS: Anatomic study of the external auditory canal's (EAC) anterior bulge, scutum, and ossicular chain will generate knowledge applicable to safe ear surgery and instrument design. BACKGROUND: The EAC contains two structures that obscure view of the middle ear: the anterior bulge and the scutum. The dimensions of these structures and their relationships to the ossicular chain have not been previously described. METHODS: Cadaveric temporal bones underwent computed tomography scanning, and three-dimensional reconstructions were created. Dimensions and angles of the EAC, its anterior bul

The Modern Male Rhytidectomy: Lessons Learned.

BACKGROUND: Cosmetic surgery among male patients has become increasingly popular. The face-lift technique is different in male patients, specifically regarding preservation of hair follicles, restoration of a youthful and well appearance, and reduction of the risk of hematoma. METHODS: A retrospective review of a single surgeon's past 20 years of consecutive male rhytidectomies revealed a cohort of 83 patients. A control group was created by random selection of 83 age-matched female rhytidectomy patients during the same period. Data were collected and analyzed critically for factors contribut

Surgical Timing of the Orbital "Blowout" Fracture: A Systematic Review and Meta-analysis.

OBJECTIVE: The orbital blowout fracture is a common facial injury, carrying with it a risk of visual impairment and undesirable cosmetic results unless treated properly. Optimal timing of the surgical treatment is still a matter of debate. We set out to determine whether a meta-analysis would bring us closer to an answer to this question. DATA SOURCES: PubMed, EMBASE, Web of Science, and the Cochrane Library were searched from January 1980 to August 2014. We applied the following inclusion criteria: isolated blowout fractures, presenting early and late surgery groups (<14 and >14 days). Patie

Multidrug-related leukocytoclastic vasculitis raising suspicion of sexual homicide-things are not always what they seem.

Ambiguous findings during external examination of a deceased in combination with dubious autopsy findings can raise doubts concerning the manner and cause of death. We report the case of a 35-year-old female deceased who had suffered from a borderline personality and depressive disorder with suicidal ideation. At the death scene, the body showed massive facial swelling accompanied by complete reddening of the skin of the face, with patchy skin abrasions on the forehead and neck, and purple bruise-like discolorations distributed symmetrically over both shoulders, elbows, hands, hips, knees, low

Medial maxillary fractures revisited.

BACKGROUND: We consider medial maxillary fractures to be a unique type of nasomaxillary buttress fracture involving the lateral margin of the piriform aperture, the maxillary frontal process and the medial aspect of the infraorbital rim. This article aims to define medial maxillary fractures as a unique type of facial bone fracture. METHODS: Eight patients with medial maxillary fractures were treated at our hospital from May 2010 to June 2013. Every patient was preoperatively evaluated using three-dimensional and multidimensional computed tomographic scans and surgically treated. RESULTS: Th

Influence of the maxillary sinus exposure in reduction malarplasty with an L-shaped osteotomy.

BACKGROUND: The zygomatic bone forms the prominence of the midface and plays an important role in determining the human facial attractiveness. The Oriental face is mesocephalic, with a prominent zygomatic body and wide zygomatic arch. Nevertheless, as an oval face is considered ideal, reduction malarplasty is always requested by Oriental people. Currently, L-shaped zygomatic osteotomy is often reported in literature yet no previous study has documented the exact outcome of this approach after exposure of the maxillary sinus. The objective of the study was to investigate the outcome and complic

A new rabbit model of myocardial infarction without endotracheal intubation.

INTRODUCTION: When establishing a myocardial infarction model in rabbits, artificial ventilation under endotracheal intubation is required to prevent pulmonary collapse caused by pneumothorax. However, it is difficult to carry out endotracheal intubation due to the rabbit anatomy. We therefore attempted to develop a myocardial infarction model in rabbits without endotracheal intubation. MATERIALS AND METHODS: A facial oxygen mask with a flow rate of 200 mL/min was placed on anesthetized rabbits (n = 17). The sternum was carefully incised along the midline to avoid injury to the parietal pleur

Localized facial flushing in infancy. Auriculotemporal nerve (Frey) syndrome.

BACKGROUND: Patients with auriculotemporal nerve syndrome (Frey syndrome) have facial flushing, sweating, or both localized to the distribution of the auriculotemporal nerve that occurs in response to gustatory stimuli. In adults, the syndrome usually results from surgical injury or trauma to the parotid gland. The condition is rare in infants, but usually manifests during infancy with the introduction of solid food, thus leading to the misdiagnosis of food allergy by physicians unfamiliar with the syndrome. OBSERVATIONS: We describe 8 children with auriculotemporal nerve syndrome who manifes

LigaSure-assisted Retaining Ligament Release during Composite Deep Plane Face and Neck Lift: A Three-Sign Structured Framework.

Complete release of the retaining ligaments during deep plane face and neck lift is essential for durable rejuvenation. This step demands particular caution to avoid nerve injury, however, as branches of the facial nerve course in close proximity to the retaining ligaments. During ligament release, surgeons currently rely on uncodified visual and tactile clues or separate neuromonitoring equipment which introduces an instrument change between identification and division. We describe a three-sign sequential identification framework in which the LigaSure vessel sealing system (Medtronic, Minneap

Posttraumatic iatrogenic meningoencephalocele following tympanoplasty: illustrative case.

BACKGROUND: Iatrogenic meningoencephalocele of the temporal bone is a complication of middle ear surgery and is typically associated with inflammation-related defects. Trauma-related iatrogenic meningoencephalocele is rare, and its pathogenesis remains poorly understood. OBSERVATIONS: A teenage girl sustained a head injury with a left temporal bone fracture that extended to the mastoid tegmen. Owing to sensorineural hearing loss and facial nerve palsy, she underwent mastoidectomy and tympanoplasty without exposure of the dura. Twelve months later, imaging revealed a meningoencephalocele herni

Isolated abducens nerve involvement as a revealing sign of temporal bone fracture: A case report.

Isolated abducens nerve palsy is an uncommon yet clinically significant manifestation of temporal bone fracture, often following blunt head trauma. We report the case of a 52-year-old woman who sustained a right-sided head injury after a domestic fall under the influence of alcohol. She presented with diplopia, vertigo, and right-sided conductive hearing loss. Clinical examination revealed impaired abduction of the right eye, anisocoria with mydriasis, right retroauricular ecchymosis, and facial hypoesthesia in the maxillary (V2) and mandibular (V3) territories. Otoscopy showed hemotympanum, a

A multichannel electromyography dataset for continuous intraoperative neurophysiological monitoring of cranial nerve.

Continuous Intraoperative Neurophysiologic Monitoring (cIONM) is a widely used technology to improve surgical outcomes and prevent cranial nerve injury during skull base surgery. Monitoring of free-running electromyogram (EMG) plays an important role in cIONM, which can be used to identify different discharge patterns, alert the surgeon to potential nerve damage promptly, etc. In this dataset, we collected clinical multichannel EMG signals from 11 independent patients' data using a Neuromaster G1 MEE-2000 system (Nihon Kohden, Inc., Tokyo, Japan). Through innovative classification methods, the
