PURPOSE OF REVIEW: The incidence of acute rheumatic fever (ARF) in the United States and Western Europe is decreasing and postStreptococcal reactive arthritis (PSRA) is more prevalent. It is not clear whether PSRA is a forme fruste of ARF or a separate disease entity. Therefore, this review explores similarities and dissimilarities in initial symptoms and signs, disease course and underlying pathophysiologic mechanisms.
RECENT FINDINGS: ARF and PSRA present differently. PSRA patients are generally older, have a longer interval between group A streptococcus infection and symptom onset, and res…

Familial Mediterranean fever (FMF) is an autosomal, recessively inherited multisystem disease that affects various groups of people originating from the Mediterranean Sea region, most specifically those of Jewish, Turkish, Armenian, and Arabic ethnicity. Recurrent attacks of fever and sterile polyserositis of the peritoneum, synovial membranes, and pleura are the main clinical features, although the clinical features of FMF have been expanded in recent years to also include severe myalgia, scrotal swelling, cardiac involvement, and protracted febrile myalgia syndrome (PFMS). PFMS is seen in on…

Resurgence of yellow fever epidemics in Côted'Ivoire remains a major problem of public health. To describe this disease through the early alarm system of National Institute of Public Hygiene (NIPH) is convenient to us. This is a cross-sectional study with descriptive aiming, which proceeded with epidemiological surveillance service of NIPH. The data were collected over 7 years (from 2001 to 2007), starting from the cards of notification, reports, and registers of the service. The main findings of our study are that: 1) we listed 1468 suspect cases including 41 confirmed and the lethality rate …

This paper presents epidemiological and control characteristics of yellow fever in Brazil, taking its wild and urban transmission cycles into consideration. No urban cases have been reported in Brazil since 1942, but urban yellow fever cases were reported in Paraguay in 2008, after more than 50 years without registered cases in the Americas. The two main objectives of yellow fever control programs in Brazil are to reduce the number of wild cases and to maintain zero incidence of urban cases. Although there is a consensus regarding control measures that should be applied in areas endemic for th…

The sheep (Ovis aries) is somewhat less common than smaller species in laboratory settings, but personnel who work with sheep or in a facility that houses sheep should be aware that certain zoonotic diseases are common in sheep. They should also know how to prevent transmission of zoonotic disease in facilities that house or work with small ruminants. Knowledge of diseases such as query fever (Q fever), which can cause severe human morbidity (and in some cases death), needs to be especially emphasized. In this paper, the author describes potential causes, transmission and manifestations of Q f…

Q fever is a zoonotic disease caused by the intracellular gram-negative bacterium Coxiella burnetii (C. burnetii), which only multiplies within the phagolysosomal vacuoles. Q fever may manifest as acute or chronic disease. The acute form is generally not fatal and manifestes as self-controlled febrile illness. Chronic Q fever is usually characterized by endocarditis. Many animal models, including humans, have been studied for Q fever infection through various exposure routes. The studies considered different endpoints including death for animal models and clinical signs for human infection. In…

CONTEXT: Typhoid fever in the United States has increasingly been due to infection with antimicrobial-resistant Salmonella ser Typhi. National surveillance for typhoid fever can inform prevention and treatment recommendations.
OBJECTIVE: To assess trends in infections with antimicrobial-resistant S. Typhi.
DESIGN: Cross-sectional, laboratory-based surveillance study.
SETTING AND PARTICIPANTS: We reviewed data from 1999-2006 for 1902 persons with typhoid fever who had epidemiologic information submitted to the Centers for Disease Control and Prevention (CDC) and 2016 S. Typhi isolates sent b…

Salmonella enterica serotype Dublin, a bovine adapted serotype, is the commonest cause of salmonellosis in cattle. Salmonellosis in animals always presents a potential zoonotic threat. Infected cattles serves as a source of infection to humans. We present here Salmonella Dublin Bacteraemia in an elderly patient, with all the clinical details, due to the rarity of its occurrence. He was treated successfully with ciprofloxacin and his follow up period was uneventful.

Familial Mediterranean Fever (FMF) is an autosomal recessive disease which is characterized by recurrent, self-limiting, short attacks of serositis while abdominal pain is the most common symptom. The underlying clinical and pathological picture is that of acute peritonitis. These abdominal signs are often so striking that they mimic an acute abdominal calamity suggesting several possible gastrointestinal, gynecologic or urologic diagnoses. Diagnosis of acute abdomen in pregnancy also remains one of the most challenging conditions as the physiological consequence of pregnancy and nonspecific l…

We describe a patient with late HIV presentation, opportunistic infections and an immune reconstitution syndrome (IRIS) after starting a combination antiretroviral therapy (ART)occurring in this severely immunodeficient patient. In the beginning, a miliary tuberculosis was diagnoses, followed by a second opportunistic infection, i.e. pneumocystis jiroveci pneumonia. After start of ART, further organ manifestations developed, interpreted as immune reconstitution syndrome. The decision about the optimal time point for starting ART in the presence of an opportunistic infection is essential.

Rift Valley fever (RVF) is an expanding zoonotic disease transmitted from ruminant to ruminant by Culicidae mosquitoes. In 2004, a longitudinal serological survey was performed on small ruminants in the Ferlo are (Senegal) to study RVF transmission and compared the results with those obtained from the same study in 2003. The results confirm that the disease is endemic and that the spatial transmission of RVF is highly heterogeneous. The virus could be maintained during dry season by transovarian transmission in Aedes vexans. Further studies are needed to improve the understanding of the epidem…

This study reports results of a cross-sectional study based on interviews and seroepidemiological methods to identify risk factors for yellow fever infection among personnel of a military garrison in the Amazonian rainforest. Clinical symptoms and signs observed among yellow fever cases are also described. Humoral immune response to yellow fever, Mayaro, Venezuelan equine encephalitis, Oropouche, and dengue 2 infection was assessed by evaluating IgM and IgG specific antibodies. A yellow fever attack rate of 13% (44/341, with 3 fatal cases) was observed among military personnel. Signs of digest…

Mediterranean spotted fever or boutonneuse fever is caused by Rickettsia conorii and transmitted by the brown dog tick. The commonest symptoms are pyrexia, a maculopapular rash, lymphadenopathies and an inoculation eschar. Increasingly, it is recognised as a cause of serious illness in southern Europe. Rickettsial infections of the spotted fever group are rarely reported in Belgium. We report the case of a 20-year-old traveller returning from Morocco who presented with fever and a markedly swollen inguinal lymph node. Our case report illustrates the challenges rickettsioses can pose to physici…
