Successful Treatment of Necrotizing Retinitis with Epstein-Barr Virus-Positive Ocular Fluid by Intravitreal Methotrexate Injection.

PURPOSE: To present a case of necrotizing retinitis with Epstein-Barr virus (EBV)-positive ocular fluid in a patient with sudden unilateral vision loss, which was successfully treated with intravitreal methotrexate (MTX) injections. METHODS: Retrospective case report. RESULTS: An 83-year-old female who had been on methylprednisolone for 20 years due to interstitial pneumonia developed vitreous opacity and extensive necrotizing retinitis with retinal hemorrhage sparing the posterior pole in the left eye. Multiplex polymerase chain reaction (PCR) for viral DNA using vitreous sample was positiv

Clinical and Magnetic Resonance Imaging Findings of Neurotoxocariasis.

Human toxocariasis is one of the most prevalent helminthiases worldwide. Toxocara canis larvae can cross the blood-brain barrier leading to the neurotoxocariasis. The clinical presentation consists of a wide spectrum of neurological manifestations, but asymptomatic infection is probably common. Neurotoxocariasis is not a frequent diagnosis probably due to the non-specific nature of its symptoms as well as the lack of confirmatory diagnostic tests. Diagnosis of neurotoxocariasis is based on the presence of a high titer of anti-Toxocara antibody in the cerebrospinal fluid or in the serum, presen

[Voluminous unilateral hydrocele with scrotal ulceration: about a case study conducted at the Regional Hospital of Gao].

Hydrocele is an accumulation of fluid between the testis and its vaginal coat. It may be very abundant. It commonly affects people who live in tropical regions. We report the case of a 56-year old patient, Mr. YM, living in the sahelian zone with very abundant hydrocele and thickening of vaginal coat associated with bag ulcer. The patient presented with big bag associated with a feeling of heaviness as well as annoyance when walking, evolving for about one year with progressive increase in volume. Clinical examination showed voluminous left bag (A,B) with a plague measuring 3 cm (A). Two diagn

[Herpes simplex virus type 2-associated recurrent aseptic meningitis (Mollaret's meningitis) with a recurrence after 11-year interval: a case report].

A 55-year-old woman was diagnosed with aseptic meningitis at the age of 43 and 44. She developed sudden fever and headache, and she showed nuchal rigidity. Cerebrospinal fluid examination revealed pleocytosis (cell count 208/mm3) and was positive for herpes simplex virus type 2 (HSV-2) DNA by PCR. Acyclovir was started on the first day of admission, and she was complete recovery. Preserved cerebrospinal fluid specimen from aseptic meningitis at the age of 44 was also positive for HSV-2 DNA by PCR. She was diagnosed with HSV-2 associated recurrent aseptic meningitis (Mollaret's meningitis) with

A case of hepatocellular carcinoma in an elder man with metastasis to the nasopharynx.

The lung, bone, brain, etc are common metastatic sites of hepatocellular carcinoma. Nasopharynx metastasis of hepatocellular carcinoma rarely occurs and has not been reported yet. Here we report one case of nasopharynx metastasis from liver in a 50-year-old male patient who was diagnosed with primary hepatocellular carcinoma (nodular and diffuse-type) in 2014. 6 and underwent interventional therapy for two times afterward. However, he suffered from severe headache in 2014. 8, and head contrast-enhanced MRI scan did not show clues for brain or skulls metastasis. Then the lumbar puncture was per

Retropharyngeal pseudoabscess manifesting in nephrotic syndrome.

We describe a case of nephrotic syndrome that manifested as a retropharyngeal pseudoabscess. The patient was a 32-year-old man who presented with a short history of throat discomfort and a choking sensation. Laryngoscopy identified bulging of the posterior pharyngeal wall that partially occluded the laryngeal inlet. A lateral neck x-ray revealed that the prevertebral space was widened, and computed tomography detected fluid in the retropharyngeal and parapharyngeal spaces. Neck exploration revealed that the edema had been caused by nonsuppurative fluid. Biochemical analyses revealed marked hyp

[Contact lens associated dry eye. Current study results and practical implementation].

Contact lens-associated dry eye symptoms have mostly been therapeutically addressed using artificial tears containing hyaluronic acid to supplement tear film volume with often unsatisfactory results. However, the main reason for contact lens-associated dry eye is not the lack of tear fluid but the lack of tear film stability due to meibomian gland dysfunction leading to reduction of the lipid film of tears. This is associated with increased evaporation of the aqueous phase of tear fluid and a measurable hyperosmolarity of the residual "denatured" tear film. A subsequent inflammatory reaction o

Advances in peritoneal dialysis in acute kidney injury.

Peritoneal dialysis (PD) is a simple, safe, cheap, and efficient renal replacement therapy method. It can correct metabolic disorders and fluid overload in acute kidney injury (AKI) patients both in and out of the intensive care unit. Use of PD in AKI is enhanced by placement of a Tenckhoff catheter, which can be safely accomplished at the bedside. Some PD modalities, such as high-volume PD and continuous-flow PD, can provide dialysis doses and efficiency comparable to extracorporeal blood purification methods. PD is particularly suitable for neonates, children, and patients with refractory he

[Pneumopericardium after pericardiocentesis: a case report].

Pneumopericardium is defined by the presence of air in the pericardial sac. We present a 61-year-old cachectic woman who developed pneumopericardium after pericardiocentesis. She presented with complaints of fatigue and shortness of breath. The chest X-ray showed an increased cardiothoracic ratio, and echocardiographic examination showed a marked pericardial effusion. Pericardiocentesis was performed and a total of 860 ml hemorrhagic pericardial fluid was aspirated. At the end of the first week after removal of the catheter, control chest radiography showed air-fluid levels in the pericardial

An unusual case of knee monoarthritis.

A 61-year-old woman, a known case of diabetic nephropathy on chronic ambulatory peritoneal dialysis, presented with acute onset severe pain in the right knee, of 7 days duration. She had red hot and tender knee and was unable to bear weight. The lower half of the right thigh was swollen and tender. Right knee synovial fluid revealed 130 cells/microL (all lymphocytes). The fluid was negative for crystals and bacterial cultures. Three week later she returned with worsening of her symptoms despite receiving non-steroidal anti-inflammatory drugs and intra-articular glucocorticoid. Magnetic resonan

Inpatient management of patients with volume overload and high filling pressures.

The majority of patients with acute decompensated heart failure are admitted with symptoms of congestion. The classic symptoms of "congestive" heart failure reflect fluid overload, that is, orthopnea, paroxysmal nocturnal dyspnea, and peripheral edema; these symptoms can be so dramatic that it is not surprising that patients seek hospitalization. Activation of the renin angiotensin system coupled with sympathetic hyperactivity results in marked sodium retention and high filling pressures that ultimately bring about these congestive symptoms. The treatment goal of patients hospitalized with vol

Acute eosinophilic pneumonia complicating Coccidioides immitis pneumonia: a case report and literature review.

Though peripheral eosinophilia commonly accompanies infection with Coccidioides immitis, pulmonary eosinophilia in patients with C. immitis and multilobar infiltrates has been reported in only three prior cases to our knowledge. To enhance clinicians' suspicion of an unusual complication of C. immitis pneumonia and the treatment implications, we present a patient with an initial diagnosis of acute eosinophilic pneumonia [based on bronchoalveolar lavage (BAL) fluid analysis and transbronchial biopsy] followed several days later by a diagnosis of accompanying C. immitis pneumonia when BAL fluid

Management of patients receiving antithymocyte globulin for aplastic anemia and myelodysplastic syndrome.

Antithymocyte globulin (ATG) is used commonly in patients with severe aplastic anemia and those undergoing renal transplant. Its utility also is being explored in the treatment of myelodysplastic syndrome, conditioning regimens for hematopoietic stem cell transplant, and prophylaxis of graft-versus-host disease. As indications for ATG expand, knowledge regarding its administration and management of associated toxicities is needed. These toxicities range from life-threatening anaphylaxis associated with the infusion to flu-like symptoms that occur one to two weeks after the infusion. Adverse ef

Trapped lung.

Trapped lung is one of the outcomes of fibrinous or granulomatous pleuritis and is a cause of chronic, benign, unilateral pleural effusion. It is characterized by inability of the lung to expand and fill the thoracic cavity due to a restricting fibrous visceral pleural peel. The resulting chronic pleural space is fluid filled, and the persistence of the fluid is solely due to hydrostatic equilibrium. Historically recognized as a complication of therapeutic pneumothorax for treatment of tuberculosis, it is today most commonly a consequence of inadequately treated parapneumonic effusion, but it

[Peritoneal dialysis in children after cardiac surgery].

The authors analyse their experience with the use of peritoneal dialysis (PD) in children with acute renal failure after cardiac surgery. From 1995 through 1998, 1246 children were operated in Pediatric Cardiocenter 38 of them developed acute renal failure and required peritoneal dialysis. Indications for PD were oliguria, anuria, hyperkalemia and fluid overload with generalized tissue oedema. PD was instituted at mean 33.9 (3-132) hours after surgery and lasted mean 53.7 (6-264) hours. None of the patients developed peritoneal infection. Hyperglycaemia developed in 8 patients and was treated

How to manage patients with hard-to-recognize postnasal drip?

Postnasal drip (PND), commonly regarded as a phenomenon wherein nasal fluid drips into the pharynx, is one of the main symptoms of chronic sinusitis and other nasal lesions. This definition is controversial, however, because some patients have PND with no evidence of fluid either in the oropharynx or around the choanae. Among 220 patients in whom PND was diagnosed at the University of Tokyo Hospital between January 1996 and December 1997, 19 (8.6%) had hard-to-recognize PND on routine examination. Through careful observation, responsible lesions could be identified in all patients with hard-to

Pericardial tamponade as the first manifestation of dermatopolymyositis.

Pericardial involvement in polymyositis and dermatomyositis (PM/DM) is rare, usually asymptomatic and exceptionally part of the initial presentation. We describe a 37-year-old patient hospitalized for overt acute pericarditis, revealing dermatomyositis, which to our knowledge has not been previously reported. Pericardial puncture relieved the patient. The pericardial fluid was citrine yellow, exsudative, and mainly contained lymphocytes. Search for LE cells and anti-DNA antibodies was negative, whereas the antinuclear antibodies and the rheumatoid factor were present in the pericardial fluid.

[Pulmonary cryptococcosis during HIV infection. 15 cases].

We reviewed the records of 15 Human Immunodeficiency Virus (HIV) infected patients with pulmonary cryptococcosis (PC). PC was the first AIDS-defining manifestation in nine patients. HIV infection was identified simultaneously with the onset of PC in 4 patients. The CD4+ lymphocyte count was low in all cases (median, 24/m3). Chest radiography showed interstitial infiltrates in 13 instances, associated with pleural effusion in 5 cases and hilar adenopathy in 2 cases. In one case, chest-X-ray showed isolated pleural effusion and was normal in one patient. For 11 of 12 patients, bronchoalveolar la

Hartmannella vermiformis isolated from the cerebrospinal fluid of a young male patient with meningoencephalitis and bronchopneumonia.

Meningoencephalitis and bronchopneumonia were documented in a patient from Peubla, Mexico. The patient began with symptoms and signs of a common flu and 12 days after the onset of his disease he was admitted to the hospital presenting symptoms and signs of meningoencephalitis. The clinical course evolved into an endocraneal hypertension syndrome with bronchopneumonia, coma and death. Wide-spectrum antibiotics, immunosuppressive and anti-tuberculosis therapy were unsuccessfully administered. Important antecedents were degree I malnutrition and repeated contact with polluted water. Gram-negative bacillary meningitis in adults: a recent six-year experience.

Thirty-eight cases of Gram-negative bacillary meningitis in adults have been identified over the past six years at the Veterans General Hospital, Taipei. Twenty cases were associated with head trauma and/or neurosurgery, while 18 cases occurred spontaneously. The overall mortality was 58%. Within the spontaneous meningitis group, 13 cases (72%) were due to Klebsiella pneumoniae, 11 cases (61%) were associated with bacteremia and eight cases (44%) with diabetes mellitus. In spite of the administration of third-generation cephalosporins, most cases of spontaneous meningitis (15 patients, 83%) di
