Bipolar Hemiarthroplasty in Unstable Intertrochanteric Fractures with an Effective Wiring Technique.

PURPOSE: Bipolar hemiarthroplasty has recently been acknowledged as an effective option for treatment of unstable intertrochanteric fracture. Trochanteric fragment nonunion can cause postoperative weakness of the abductor muscle and dislocation; therefore, reduction and fixation of the fragment is essential. The purpose of this study was to perform an evaluation and analysis of the outcomes of bipolar hemiarthroplasty using a useful wiring technique for management of unstable intertrochanteric fractures. MATERIALS AND METHODS: A total of 217 patients who underwent bipolar hemiarthroplasty usi

Deltoid ligament arthroscopic repair in ankle fractures: Case series.

INTRODUCTION: Diagnosis and treatment of ankle medial ligament lesions in malleolar fractures has always been a matter of controversy. Even when deltoid involvement is clear, the direct repair of this structure is not a consensus. Recently, deltoid repair through an arthroscopic technique was described aiming to potentialize better clinical results and minimize complications. OBJECTIVE: Demonstrate safety and functional results on patients with ankle fractures submitted to open reduction and internal fixation and arthroscopic deltoid repair. METHODS: This is a retrospective study in patients

Primary total hip replacement using Burch-Schneider cages for acetabular fractures.

INTRODUCTION: Osteoporotic acetabular fractures are common and pose a difficult technical challenge for the trauma surgeon. Acute total hip arthroplasty (THA) using a Burch-Schneider antiprotrusio cage with immediate postoperative weight-bearing is a method to approach these injuries. This case series reports our outcomes of acute THA using Burch-Schneider cages for acetabular fractures from a UK major trauma centre based on length of stay, radiological outcome, complications and outcome scores. METHODS: Data were collected from all patients who underwent acute THA with a Burch-Schneider cage

No Difference in Return to Sport and Other Outcomes Between Operative and Nonoperative Treatment of Medial Epicondyle Fractures in Pediatric Upper-Extremity Athletes.

OBJECTIVE: Compare outcomes of operative and nonoperatively managed medial epicondyle fractures in upper-extremity athletes. DESIGN: Retrospective chart review and phone survey. SETTING: Level 1, tertiary-referral pediatric hospital. PATIENTS: Propensity scores (probability of operative treatment) were estimated from a logistic regression model that included sex, age, displacement, limb dominance, and injury severity (presence of an additional fracture, nerve injury, or elbow dislocation). These were used to match subjects in the operative group to the nonoperative group. MAIN OUTCOME MEAS

When and how to operate the posterior malleolus fragment in trimalleolar fractures: a systematic literature review.

OBJECTIVES: Whether or not and how to fixate the posterior malleolus fracture seems to depend on the fracture fragment size and its amount of dislocation, but clear guidelines for daily practice are lacking. In this review, we summarize the literature on preferred treatment of the posterior fragment in trimalleolar fractures. METHODS: A systematic review of publications between January 1995 and April 30 2017 on this topic in the PubMed, Embase, and Cochrane databases was performed according to the PRISMA statement. RESULTS: Seventeen (2 prospective and 15 retrospective) of the 180 identified

[Use of a cannulated 3.0 mm AO screw with an intraosseous support washer in osteosynthesis of the scaphoid: results and analysis of problems in 28 cases].

The cannulated 3.0 mm AO/ASIF screw with threaded washer is a new implant for scaphoid osteosynthesis. After insertion into the distal scaphoid pole, the washer serves as an intraosseous support for the head of the cannulated lag screw. From June 1997 to March 1998 the new implant was used in 28 male patients between 14 and 50 years of age. In fourteen patients, acute scaphoid fractures were operated on, including five transscaphoid perilunate fracture dislocations (Herbert type B2 and B4, respectively). Fourteen patients had scaphoid pseudarthroses, among those two re-pseudarthroses after pri…

Talus Fracture of the Medial Tubercle of the Posterior Process: Interposition of the Flexor Hallucis Longus Tendon.

Talus fracture of the medial tubercle of the posterior process is rare. This type of fracture can be easily missed, because it is difficult to identify on plain radiographs of the ankle. Oblique radiographs with external rotation, computed tomography, and magnetic resonance imaging (MRI) of the ankle are useful for making an accurate diagnosis. However, even with an early diagnosis, the treatment guidelines for talus fractures of the posterior medial tubercle have not yet been established. The flexor hallucis longus (FHL) tendon, which passes through the groove between the medial and lateral t

Reduction techniques in distal radius fractures.

External skeletal fixation is an important minimal invasive procedure in the management of fractures at the distal end of the radius. Attention to detail is important not only in the recognition of indications and function of the external fixator but also in its specific application. To improve anatomical restoration (e.g. palmar tilt) multi-planar ligamentotaxis is recommended. The demonstrated dynamic fixator is easy to apply and the double ball joint facilitates reduction after mounting by multi-planar ligamentotaxis. This transarticular unilateral external fixation system permits restorati

[Damage of the upper femoral epiphysis. Long time results following trauma to the coxal end of the femur (author's transl)].

Traumatic lesions of the coxal end of the femur during growth often lead to irreparable damage of the upper femoral epiphysis. These lesions as well as their influence to the femoral epiphysis are described and classified according to our own experiences and a study of the literature is given as well. Our own results are fractures of the neck of femur, transcapital femoral fractures, pelvic fractures and traumatic luxations during growth. For complementation of diagnostic examinations possibilities of non-traumatic damage of the epiphysis of the femoral head during growth are given.

Reverse total shoulder arthroplasty in patients 80 years and older: a national database analysis of complications and mortality.

BACKGROUND: Although reverse total shoulder arthroplasty (RSA) is considered a safe surgical option in elderly patients, large-scale analyses of complications and mortality after RSA in patients 80 years and older are scarce. The goals of the current study were to identify revision, complication, and early mortality rates after RSA in patients 80 years and older and compare these to younger patients. METHODS: The PearlDiver Database, which contains services rendered to Medicare, Medicaid, and commercial insurance patients, was queried for patients undergoing RSA using International Classifica

Axillary artery injury combined with delayed brachial plexus palsy due to compressive hematoma in a young patient: a case report.

INTRODUCTION: Axillary artery injury in the shoulder region following blunt trauma without association with either shoulder dislocation or fracture of the humeral neck has been previously reported. Axillary artery injury might also be accompanied with brachial plexus injury. However, delayed onset of brachial plexus palsy caused by a compressive hematoma associated with axillary injury after blunt trauma in the shoulder region has been rarely reported. In previous reports, this condition only occurred in old patients with sclerotic vessels. We present a case of a young patient who suffered axi

Spinal cord injury without radiographic abnormality in children--the SCIWORA syndrome.

Spinal cord injury in children frequently occurs without fracture or dislocation. The clinical profiles of 55 children with spinal cord injury without radiographic abnormalities (SCIWORA) are reported in detail to illustrate features of this syndrome. No patient had vertebral fracture or dislocation on plain films and tomographies. There were ten upper cervical (C1-C4), 33 lower cervical (C5-C8), and 12 thoracic cord injuries; of these, 22 were complete or severe lesions and 33 were mild lesions. The mechanism of the neural injury probably relates to the inherent elasticity of the juvenile spi

Risk factors for post-traumatic osteoarthritis following surgical treatment of acetabular posterior wall fractures: a retrospective study.

Posterior wall fractures, commonly caused by high-energy mechanisms like motor vehicle collisions or falls, are frequently associated with hip dislocation and complex intra-articular injuries. Despite surgical advances, post-traumatic osteoarthritis (PTOA) remains a prevalent complication, often leading to significant functional impairment and eventual joint replacement. Identifying potentially modifiable risk factors may help improve long-term outcomes. This study aimed to identify independent risk factors associated with the development of post-traumatic osteoarthritis (PTOA) following surgi

Diagnostic performance of a gravity varus stress computed tomography protocol in detecting instability in isolated coronoid fractures.

BACKGROUND: Instability associated with isolated coronoid fractures can be subtle and frequently missed on routine imaging. A stress-based computed tomography (CT) protocol may improve detection of occult instability in isolated coronoid fractures. The purpose of this study was to evaluate the diagnostic performance of a gravity varus stress CT protocol in detecting instability in isolated coronoid fractures and compare it with routine static CT. METHODS: A retrospective cohort study of patients with isolated coronoid fractures who underwent either gravity varus stress CT or routine static CT
