DNA-testing for BRCA1/2 prior to genetic counselling in patients with breast cancer: design of an intervention study, DNA-direct.

BACKGROUND: Current practice for patients with breast cancer referred for genetic counseling, includes face-to-face consultations with a genetic counselor prior to and following DNA-testing. This is based on guidelines regarding Huntington's disease in anticipation of high psychosocial impact of DNA-testing for mutations in BRCA1/2 genes. The initial consultation covers generic information regarding hereditary breast cancer and the (im)possibilities of DNA-testing, prior to such testing. Patients with breast cancer may see this information as irrelevant or unnecessary because individual geneti

Novel mutations in the SGCA gene in unrelated Vietnamese patients with limb-girdle muscular dystrophies disease.

Background: Limb-girdle muscular dystrophy (LGMD) is a group of inherited neuromuscular disorders characterized by atrophy and weakness in the shoulders and hips. Over 30 subtypes have been described in five dominant (LGMD type 1 or LGMDD) and 27 recessive (LGMD type 2 or LGMDR). Each subtype involves a mutation in a single gene and has high heterogeneity in age of onset, expression, progression, and prognosis. In addition, the lack of understanding of the disease and the vague, nonspecific symptoms of LGMD subtypes make diagnosis difficult. Even as next-generation sequencing (NGS) genetic tes

Significant Linkage Evidence for Interstitial Cystitis/Painful Bladder Syndrome on Chromosome 3.

PURPOSE: Interstitial cystitis/painful bladder syndrome is a chronic pelvic pain condition of unknown etiology. We hypothesized that related interstitial cystitis/painful bladder syndrome cases were more likely to have a genetic etiology. The purpose of this study was to perform a genetic linkage analysis. MATERIALS AND METHODS: We identified interstitial cystitis/painful bladder syndrome cases using diagnostic codes linked to the Utah Population Database genealogy resource and to electronic medical records. For this analysis we used 13 high risk pedigrees, defined as having a statistical exc

Beyond LDL-C: A Case-Based Review of Complex Dyslipidemia.

Dyslipidemia is clinically heterogeneous, and LDL-C alone may not fully capture atherogenic particle burden, metabolic risk, treatment response, or inherited disease. This case-based review presents three dyslipidemia phenotypes that illustrate common diagnostic and management challenges. The first case describes diet-sensitive LDL particle and lipoprotein insulin resistance changes despite LDL-C values near conventional targets in a patient with subclinical coronary atherosclerosis. The second highlights persistent low HDL-C and HDL particle number despite substantial LDL-C reduction with non

VACmap: an accurate long-read aligner for unraveling complex genomic rearrangements.

Sequence alignment is essential for genomic research and clinical diagnostics, yet detecting complex rearrangements such as inversions, duplications, and gene conversions remains challenging due to allele complexity and limitations of current methods. We introduce VACmap, a non-linear mapping approach to enhance the detection and representation of all genetic variations. VACmap improves duplication detection from 20% to 90% in the Challenging Medically-Relevant Genes (CMRG) benchmark and improves characterization of complex inversions in repetitive regions and gene conversion events. It improv

Traveling in a Pack: The Border Cells' Collective Journey Guided by Landscapes and Chemical Cues.

Many cell types migrate collectively, a process critical for animal development and co-opted in some medical disorders. Thus, uncovering the molecular regulation of collective cell migration is of broad interest, yet this process remains understudied as compared to individual cell motility. Both collective and individual cell movements rely on similar mechanisms to change cell shapes and adhesiveness. Although grouped cells face the added challenge of maintaining coordination and communication, they can then leverage group-level advantages, like animals in a pack. How motile cells work togethe

Update on the Etiology and Pathogenesis of Erythrodermic Psoriasis.

Erythrodermic psoriasis(EP) is a rare, life-threatening variant affecting 75%-90% of the body surface area. Characterized by widespread erythema and potential systemic symptoms like fever and lymphadenopathy, it severely impairs patient quality of life. The pathogenesis of erythrodermic psoriasis is not fully understood. It is a multifactorial, multistep process suspected to result from an abnormal immune response induced by both genetic and environmental factors. Key contributors to erythrodermic psoriasis onset include specific gene polymorphisms, altered expression of adhesion molecules, dy

Deep brain stimulation for the treatment of patients with dystonia: advances towards network-based personalised care.

Dystonia is a complex movement disorder that can cause functional disability and reduce quality of life. Deep brain stimulation (DBS) provides meaningful benefits in patients with medically refractory dystonia, but clinical outcomes can vary substantially. This variability likely reflects the heterogeneity in clinical presentations, genetic background, and brain networks involved in motor control. Dystonia is a network disorder involving abnormal interactions across cortico-basal ganglia-cerebellar circuits. Therapeutic effects of DBS therefore arise from modulation of distributed motor circui

Relationships between cannabis use and mental disorders: assessing the coherence of evidence from studies with different methodologies.

Cannabis use often begins in adolescence and young adulthood, when anxiety, depression, psychosis, and bipolar disorder typically first develop. Young people aged in their mid-teens to mid-twenties who engage in daily cannabis use and develop a cannabis use disorder have a higher prevalence of these mental disorders. We assessed the coherence of evidence from epidemiological, genetic, experimental, and preclinical studies to assess relationships between daily cannabis use and the increased incidence, prevalence, and persistence of psychosis, bipolar disorder, anxiety, depression, and suicidal

Comprehensive case report and literature review on perioperative management of multiple pheochromocytoma in a pediatric patient.

Pheochromocytomas (PCCs) are rare neuroendocrine tumors originating from the adrenal medulla, particularly uncommon in pediatric populations. This case report presents a 12-year-old boy with a three-year history of fatigue and a one-year history of blurred vision, who was admitted with severe hypertension (230/200 mmHg). Abdominal CT imaging revealed bilateral adrenal masses measuring 40 mm on the left and 12 mm on the right. The diagnosis of pheochromocytoma was confirmed by elevated blood catecholamine metabolites. During perioperative preparation, the patient experienced complications, incl

Cross-species analysis of genetic architecture and polygenic risk scores for non-contact ACL rupture in dogs and humans.

Non-contact anterior cruciate ligament (ACL) rupture is a common serious orthopaedic disease in humans and dogs. Familial risk has been recognized in both species but interactions between genetic effects and environmental risk are not understood. We investigated ACL rupture heritability, genetic architecture, selection pressure, sharing of risk genes and biological pathways, and polygenic risk score (PRS) prediction of disease risk. In both species, ACL rupture has moderate heritability, is likely under negative selection, and has a highly polygenic architecture where thousands of variant effe

Epilepsy surgery for seizure control in individuals with Dravet syndrome: is it a safe and practical option?

INTRODUCTION: Dravet syndrome is a severe developmental and epileptic encephalopathy presenting in childhood with drug resistant seizures and multiple comorbidities including developmental and cognitive delays. While medical therapy has continued to advance, leading to improved seizure control, outcomes such as seizure freedom remain elusive. As the underlying cause of Dravet syndrome is a genetic channelopathy, epilepsy surgery is rarely considered. AREAS COVERED: The authors weigh up the potential benefits of surgical therapy when seizure reduction, but not seizure freedom, is expected, dis

Identifying Determinants of Survival Disparities in Multiple Myeloma Patients Using Electronic Health Record Data.

Multiple myeloma (MM) is one of the most common hematological malignancies. The goal of this study was to analyze the sociodemographic, economic, and genetic characteristics of long-term and short-term survival of multiple myeloma patients using EHR data from an academic medical center in New York City. The de-identified analytical dataset comprised 2,111 patients with MM who were stratified based on the length of survival into two groups. Demographic variables, cancer stage, income level, and genetic mutations were analyzed using descriptive statistics and logistic regression. Age, race, and

Exome sequencing in extreme altitude mountaineers identifies pathogenic variants in RTEL1 and COL6A1 previously associated with respiratory failure.

Adaptation of humans to challenging environmental conditions, such as extreme temperature, malnutrition, or hypoxia, is an interesting phenomenon for both basic and applied research. Identification of the genetic factors contributing to human adaptation to these conditions enhances our understanding of the underlying molecular and physiological mechanisms. In our study, we analyzed the exomes of 22 high altitude mountaineers to uncover genetic variants contributing to hypoxic adaptation. To our surprise, we identified two putative loss-of-function variants, rs1385101139 in RTEL1 and rs10027267

Narcolepsy: an interface among neurology, immunology, sleep, and genetics.

Narcolepsy is a primary disorder of the central nervous system resulting from genetic, environmental, and immunological interactions defined as excessive daytime sleepiness plus cataplexy, hallucinations, sleep paralysis, and sleep fragmentation. The pathophysiology is not entirely known, but the interaction among genetic predisposition, environmental exposition, and immune component with consequent hypocretin-1 deficiency is the model to explain narcolepsy type I. The mechanism of narcolepsy type II is less understood. There is a delay of over ten years for the diagnosis of narcolepsy around

Genetic architecture distinguishes tinnitus from hearing loss.

Tinnitus is a heritable, highly prevalent auditory disorder treated by multiple medical specialties. Previous GWAS indicated high genetic correlations between tinnitus and hearing loss, with little indication of differentiating signals. We present a GWAS meta-analysis, triple previous sample sizes, and expand to non-European ancestries. GWAS in 596,905 Million Veteran Program subjects identified 39 tinnitus loci, and identified genes related to neuronal synapses and cochlear structural support. Applying state-of-the-art analytic tools, we confirm a large number of shared variants, but also a d

Leveraging Mendelian randomization to inform drug discovery and development for ischemic stroke.

Discovery and development of efficacious and safe pharmacological therapies is fraught with challenges. As proteins constitute the majority of drug targets and are encoded by genes, naturally occurring genetic variation within populations can provide valuable insights to inform drug discovery and development efforts. The drug target Mendelian randomization (MR) paradigm leverages these principles to investigate the causal effects of drug targets in humans. This review examines the application of drug target MR in informing the efficacy and development of therapeutics for ischemic stroke preven

Genetic factors, risk prediction and AI application of thrombotic diseases.

In thrombotic diseases, coagulation, anticoagulation, and fibrinolysis are three key physiological processes that interact to maintain blood in an appropriate state within blood vessels. When these processes become imbalanced, such as excessive coagulation or reduced anticoagulant function, it can lead to the formation of blood clots. Genetic factors play a significant role in the onset of thrombotic diseases and exhibit regional and ethnic variations. The decision of whether to initiate prophylactic anticoagulant therapy is a matter that clinicians must carefully consider, leading to the deve
