[Gastroduodenal ulcer disease: update on pathogenesis].

Gastroduodenal ulcer disease comprises a heterogeneous group of different diseases resulting uniformly in a mucosal defect reaching beyond the muscularis mucosae. Consequently, a single unifying pathogenesis of ulcer disease does not exist but only a rather general concept that ulcers develop when mucosa-injuring factors outweigh the mucosa-protecting factors. According to this concept, ulcers develop within a broad range of different possibilities in the relation of mucosa-injuring factors to impaired mucosal protection. The main histological and physiological elements for the understanding o

Investigating the Interaction of Helicobacter pylori with the Gastric Mucosa.

Helicobacter pylori chronically infects the gastric mucosa of humans and diseases associated with infection include gastritis, peptic ulceration, and development of gastric cancer. The organism displays a distinct tropism for the gastric mucosa of humans and for the gastric mucin MUC5AC. While the majority of organisms are found in the mucus layer overlying the epithelial cells in the stomach, adherence of the organism to the gastric epithelium is necessary for the development of disease. The interaction of H. pylori with epithelial cells results in subversion of host cell signaling and induct

Prevalence and pre-disposing factors of Helicobacter pylori among patients with gastro-intestinal symptoms attending Mulago Hospital, Kampala, Uganda.

BACKGROUND: Comprehensive data on the prevalence of Helicobacter pylori infection and its predisposing factors among patients with gastrointestinal symptoms remain limited. Generating this evidence would help inform clinical management and improve antibiotic stewardship. H. pylori infection affects a substantial proportion of the global population, with prevalence varying widely across regions. In Uganda, previous studies have documented the presence of H. pylori infection. However, data specific to symptomatic patients are scarce. This study therefore aimed to determine the prevalence of H. p

The prevalence of nonmalignant gastrointestinal disease in patients with iron deficiency anemia: a systematic review and meta-analysis.

OBJECTIVE: Gastrointestinal pathology is a major cause of iron deficiency anemia (IDA). While previous studies have focused on establishing the prevalence of malignant gastrointestinal disease, the prevalence of nonmalignant gastrointestinal disease in patients with IDA has not been analyzed systematically. We performed a systematic review and meta-analysis to determine the prevalence of normal findings, individual nonmalignant findings, and malignancy in patients with IDA. METHODS: We searched Medline, Embase, and Scopus from inception for observational studies that reported the prevalence o

SMYD3-MEK/ERK Axis Contributes to Intestinal Metaplasia and Helicobacter pylori-Associated Gastric Carcinogenesis.

PURPOSE: Chronic Helicobacter pylori infection induces gastric atrophy and intestinal metaplasia (IM), which are key precancerous lesions for gastric cancer. The purpose of this study was to determine the mechanisms by which mitogen-activated protein kinase kinase/extracellular signal-regulated kinase (MEK/ERK) signaling persists from active infection to the posteradication stage and contributes to gastric carcinogenic risk. MATERIALS AND METHODS: We investigated MEK/ERK signaling across different stages of H pylori-associated mucosal injury, from active infection to the posteradication state

Enhanced H. pylori Treatment Using a β-Cyclodextrin-Propolis Complex: A Randomized Clinical Trial.

BACKGROUND: Helicobacter pylori infection poses a significant global health burden, with rising antibiotic resistance and treatment-related side effects highlighting the need for adjunctive therapies. Propolis, a natural resin with known antimicrobial and anti-inflammatory properties, suffers from poor solubility and bioavailability. This study evaluated a β-cyclodextrin-propolis complex designed to enhance delivery and therapeutic efficacy in H. pylori-infected patients. MATERIALS AND METHODS: In a double-blind, randomized clinical trial at Hamadan University of Medical Sciences, biopsy-conf

Helicobacter pylori small RNA HPnc1470 represses HP0499/PldA/CGAT expression and modulates gastric epithelial morphology and inflammation.

Helicobacter pylori persistently colonizes the stomach and contributes to gastric inflammation and carcinogenesis via sustained interactions with gastric epithelial cells. Although bacterial small RNAs (sRNAs) are crucial post-transcriptional regulators, their roles in regulating H. pylori factors that influence host cell responses remain poorly understood. In this study, we characterized HPnc1470, an H. pylori sRNA of unknown function, and identified HP0499/PldA/CGAT, a lipid-modifying factor implicated in H. pylori-epithelial cell interactions, as a gene negatively regulated by HPnc1470. RNA

AuNPs-VS2-Mediated LSPR-SPR-Coupled optical fiber biosensor for sensitive and acid-stable detection of H. pylori.

Early detection of Helicobacter pylori (H. pylori) is critical for preventing gastric diseases and associated carcinogenesis. However, current clinical methods, such as serology and endoscopic biopsy, are often highly invasive or time-consuming, limiting their utility for real-time and in vivo diagnosis. To address this gap, an AuNPs-VS2 composite-sensitized optical fiber surface plasmon resonance (SPR) biosensor was developed for rapid and highly sensitive H. pylori detection. Multilayered 1T-phase VS2 was synthesized to enhance the interfacial electric field and provide abundant sites for an

Efficacy of Potassium-Competitive Acid Blockers Versus Proton Pump Inhibitors in Eradication Regimens for Clarithromycin-Resistant Helicobacter pylori: A Systematic Review and Meta-Analysis.

BACKGROUND: Clarithromycin resistance represents the primary obstacle to the success of conventional proton pump inhibitor (PPI)-based triple therapy for Helicobacter pylori (H. pylori) eradication. Potassium-competitive acid blockers (P-CABs) offer faster and more potent acid suppression, which may enhance the efficacy of eradication regimens in this resistant population. The present study aimed to systematically evaluate the comparative efficacy of P-CAB-based versus PPI-based eradication regimens specifically in adult patients with documented clarithromycin-resistant H. pylori infection. The effect of obesity on the coccoid form transformation of Helicobacter pylori and its relationship with gastric inflammation pattern: a retrospective comparative study.

BACKGROUND/AIM: Helicobacter pylori (H. pylori) can transform from its spiral form into a coccoid (viable but non-culturable; VBNC) state under adverse environmental conditions. The coccoid form is associated with antibiotic resistance and eradication failure. This study compared coccoid and spiral form proportions, H. pylori localization patterns (epithelial vs. crypt), and gastric inflammation parameters across three BMI-stratified groups. MATERIALS AND METHODS: This retrospective comparative study included 156 H. pylori-positive cases from the archives of Karamanoglu Mehmetbey University F

Helicobacter pylori modulates macrophage polarization via the glucocorticoid receptor to promote gastric cancer cell proliferation.

BACKGROUND: Helicobacter pylori (H. pylori) is recognized as a major risk factor for gastric cancer (GC). However, the precise mechanism by which H. pylori influences macrophage immune responses in cancer progression remains poorly understood. This study investigates how H. pylori affects macrophage polarization via the glucocorticoid receptor (NR3C1) and its subsequent role in gastric cancer cell proliferation. MATERIALS AND METHODS: NR3C1 expression, prognosis, and immune infiltration in gastric cancer were analyzed using the UCSC, TCGA, and GEPIA databases. NR3C1 expression, activation, ma

The Story behind the Science: The discovery of Helicobacter pylori CagA, the first tumorigenic bacterial protein.

In this article, we reconstruct the history of the science that led to the discovery of CagA, the protein that causes gastric cancer, where teams in the United States and Italy made independent observations that ultimately merged. We also honor the memory of Antonello Covacci, who played an important role in this discovery and passed away prematurely in 2023.

Association between Anti-Helicobacter pylori IgG seropositivity on coronary heart disease and potential pathogenesis: a Mendelian randomization study.

BACKGROUND: Currently, observational studies and clinical trials have shown that the occurrence of coronary heart disease (CHD) is closely related to anti-Helicobacter pylori (H. pylori) IgG seropositivity, but these studies may be affected by confounding factors, resulting in a causal relationship that is still controversial. METHODS: We conducted a Mendelian randomization analysis to clarify the association between anti-H. pylori IgG seropositivity and CHD and explore its potential pathogenesis. Exposure, outcome, and mediator data were obtained all from the genome-wide association studies

Iron Bisphosphonate Metal-Organic Framework Nanoparticles as an Magnetic Resonance Imaging Probe for Spatial Detection of Helicobacter pylori.

Accurate spatial detection of Helicobacter pylori (H. pylori) can potentially allow for the diagnosis and prevention of early stage gastric cancer. However, the hostile gastric environment and protective mucus layer surrounding H. pylori significantly hinder precise imaging. Here, an iron bisphosphonate metal organic framework nanoparticle (Fe-BP NPs) was synthesized for magnetic resonance imaging-based spatial detection of H. pylori. The Fe-BP NPs feature excellent stability under acidic conditions, a size of 120 nm, and a negatively charged surface, enabling rapid penetration through the muc

Helicobacter pylori-positive Colorectal MALT Lymphoma with Repeated Metachronous Recurrence after a Complete Response to Eradication Therapy and Regression after a Watch-and-wait Strategy.

A 77-year-old man underwent a colonoscopy. A submucosal cecal tumor was found, and localized colorectal MALT lymphoma (Lugano stage I) was diagnosed with a Helicobacter pylori infection. The first-line eradication therapy was successful, and the cecal lesion regressed. However, a new lesion was observed in the lower rectum. The rectal lesion regressed after the second-line eradication therapy. Two new lesions were identified in the sigmoid colon. However, no additional treatment was administered and a watch-and-wait strategy was adopted. Both of the lesions regressed. There are no established

Changes in Prooxidant and Antioxidant-Related Trace Element Levels During Helicobacter pylori Eradication Therapy.

Helicobacter pylori (H. pylori) is a spiral-shaped Gram-negative bacterium strongly linked to chronic gastritis, peptic ulcer disease, and gastric cancer. The Maastricht V/Florence Consensus Report recommends eradication therapy to prevent related complications. Trace elements are essential for maintaining oxidative balance, and their alterations during therapy may provide insights into the metabolic mechanisms of H. pylori and the cellular metabolic response to PPI and antibiotic therapy. This study investigated alternations in seven key trace elements and the oxidative stress marker malondia

Peptic ulcer characteristics in oral opium and non-opium user patients with upper gastrointestinal bleeding.

BACKGROUND/AIMS: Upper gastrointestinal bleeding (UGIB) is a frequent medical issue. The primary risk factors for bleeding peptic ulcers are Helicobacter pylori infection and non-steroidal anti-inflammatory drugs. The association between acute gastric/duodenal ulcer and opium use has been previously proposed; however, there is no available data on endoscopic findings of patients with acute UGIB who use opium. MATERIALS AND METHODS: In the present descriptive cross-sectional study, endoscopic data of 50 consecutive patients with oral opium use and 50 consecutive patients without any opium use
