The chlorpromazine-sensitive GTPase from the cell membrane of rat cerebral cortex was purified to homogenity by using DEAE Bio-Gel A agarose, hydroxyapatite and heparin agarose chromatography. The purified chlorpromazine-sensitive GTPase was purified 370-fold to obtain a final specific activity of 40 mumol GTP hydrolyzed2min/mg protein. The purified enzyme was inhibited by chlorpromazine but not by compound 48/80. Magnesium was required for its activity instead of calcium. The purified enzyme had an apparent pH optimum of 8.0, and molecular weight was estimated to be 58,000.

253 outpatients were included in a prospective randomized study. All patients had incidence of injuries of the lower limb immobilized by a plaster cast. 126 patients (group I) received a subcutaneous injection of Fraxiparin daily, and 127 patients (group II) received no thromboprophylaxis. Nine patients (six men, three women) with extracranial vertebral artery dissection are described. Their mean age was 39.1 years (range 17-66). In four cases dissection was "spontaneous"; in the other five cases there was a history of trivial trauma. Three patients had fibromuscular dysplasia, two were migraineurs, one had elastorrhexis. An infant of a diabetic mother (IDM) presented with evidence of multiple thromboses. Body parts clinically involved with the thromboses included: kidney, brain, lung, forearms, and fingers. A circulating anticoagulant against bovine, equine, guinea pig, and sheep plasmas developed in a 15-year-old cardiac patient. He had been exposed to both bovine and porcine heparin over a period of 13 years, and had a porcine valve placed four years before the anticoagulant was noted. There was no anticoagulant activity detected against human, rat, or porcine plasma, and an equivocal reaction against rabbit plasma. There was no apparent clinical significance to the anticoagulant. It did, however, confuse the interpretation of coagulation factor assays based on animal substrate plasmas.

Defibrination is a rare event in acute lymphoblastic leukaemia (ALL). This paper describes four cases of acute defibrination in a series of 52 adult patients with ALL. All four patients had blood clotting tests consistent with or suggesting defibrination prior to therapy, but haemorrhages and unequivocal laboratory evidence of defibrination developed only after leukaemic blast cells had been destroyed early during therapy for remission induction. Eight patients with E. coli septicaemia had oliguric renal failure which was associated with haematological evidence of intravascular coagulation. Five of these patients also had the characteristic blood picture of microangiopathic haemolytic anaemia. Thrombosis of the jugular vein and upper extremities are much rarer than thrombosis of the lower extremities. The diagnostic tool of choice is duplex sonography of the vessels. We report the case of a patient who developed paraplegia after mitral valve repair and maze procedure. The first day after surgery, marked weakness of both lower extremities was noted. Neurologic examination showed almost complete loss of sensory and motor function below the level of the first thoracic vertebrae. Magnetic resonance imaging showed intramedullary hemorrhage ranging from the C7 to Th2 segments. Preoperative anticoagulation therapy and general heparinization during heart surgery may cause this rare complication.

A 7-year-old boy suffered a tonsillar lesion due to trauma and presented to hospital with a GCS of 15. Some hours later he developed hemiparesis with loss of consciousness. Angiography displayed a left-sided carotid dissection associated with thrombosis of the arteria cerebri media. After initial treatment by heparinization, the increase of intracranial pressure was treated by craniectomy. The neurological deficits diminished partially after days and reimplantation of the skull cap was possible. The initial hemiparesis decreased slightly during rehabilitation after 5 weeks.

OBJECTIVE: To improve the clinician's ability for emergency treatment of priapism.
METHODS: Both cases received 2 mg to 8 mg of metaraminol injection at the root of cavernous body, and perfusion of heparinized saline at glans and root of cavernous body of the penis by contrecoup, but they had not good response to the above therapy. At last surgery was performed.
