Varicella-zoster virus causes varicella and herpes zoster and a wide spectrum of central nervous system (CNS) complications, such as meningitis, encephalitis, myelitis, and stroke. Severe manifestations are more often observed in older adults and patients with an immunocompromised status. CNS involvement may occur without accompanying skin eruptions, making clinical diagnosis challenging. Although the FilmArray® Meningitis/Encephalitis Panel is a useful rapid detection tool, clinicians should be aware that false-negative results are possible, particularly in cases with low viral load or delaye

BACKGROUND: Gynecological cancers are among the most common cancers in women that affect female reproductive organs. The most common gynecological cancers are ovarian, cervical, uterine/endometrial, vaginal, and vulvar cancer. Women's reproductive organs have a dynamic and relative microbial balance. The disruption in the balance of the microbiome could result in numerous gynecological diseases, as well as, gynecological cancers. In this study, we aimed to review new findings on the role of different bacterial infections in various types of gynecological cancers.
RECENT FINDINGS: The role of …

The WHO estimates that one million new sexually transmitted infections (STI) are acquired every day worldwide among people aged between 15-49 years old. Travel is now a well-established risk factor. In Switzerland, a substantial proportion of infections are acquired abroad. Certain populations are at an increased risk, such as men who have sex with men, travelers visiting friends or relatives, backpackers, international truck drivers, and business travelers. In this context, the primary care physician plays a central role. Pre-travel consultations should systematically incorporate STI preventi…

Due to the rapid development of multidrug-resistant (MDR) bacteria due to the inappropriate use and misuse of antibiotics and the ineffective performance of antibiotics against refractory biofilm-associated infections (BRI), there is an urgent need for novel alternative antimicrobials and strategies to combat bacterial infections. Antimicrobial peptides (AMPs) have attracted considerable interest due to their potent activity against MDR pathogens and biofilm-associated infections, coupled with a substantially reduced risk of driving antimicrobial resistance-especially when employed as alternat…

Do viral infections accelerate aging, and does this acceleration scale with pathogenicity? Using transcriptomic aging clocks, we measured biological age in Drosophila melanogaster infected with four enteric RNA viruses spanning a broad pathogenicity range (i.e., reduction of host lifespan). All pathogenic infections accelerated aging and the magnitude of acceleration tracked pathogenicity. This pattern held across oral and systemic infection routes and was conserved in Caenorhabditis elegans where the nonpathogenic Orsay virus produced negligible aging acceleration. Pathway analysis indicated …

Electrochemical biosensors have emerged as promising tools to address major limitations of conventional urinary tract infection (UTI) diagnosis, which depends on slow culture and low-specificity dipsticks. By transducing specific biorecognition events into electrical signals, these platforms can deliver rapid, sensitive, and potentially point of care results directly from urine. This narrative review synthesizes developments in pathogen directed and host response based electrochemical biosensors for UTI detection. DNA biosensor arrays targeting bacterial 16S rRNA enable species specific identi

BACKGROUND: Urinary tract infections (UTIs) are the most common postoperative complications of percutaneous nephrolithotomy (PNL). Besides the significant threat to the patient's life, they represent a surgical, financial, and stressful burden to the healthcare systems. Duration of the procedure, bacterial load in urine, severity of obstruction, and presence of infected stone directly increase the incidence of UTI.
AIM: To identify the perioperative predictors of postoperative UTIs in patients undergoing PNL in Assiut University Urology Hospital, Assiut, Egypt.
METHODS: A prospective study w…

INTRODUCTION: Respiratory tract infections (RTIs) are among the most common causes of medical consultation in children. Although the majority of upper respiratory tract infections are viral and self-limiting, bacterial pathogens remain responsible for a relevant proportion of cases and may require targeted antimicrobial therapy. Accurate microbiological diagnosis is therefore essential to guide clinical management and support antimicrobial stewardship.
AREAS COVERED: This review discusses the etiology and clinical presentation of pediatric respiratory infections and examines current diagnosti…

All organisms need protection against infection. Bacteria are often primarily seen as infectious agents, but they also need protection again … e components, which work closely together to form a very powerful immune system for defense against infections. In contrast to adaptive immunity, the majority of the non-adaptive innate defense mechanisms, such as pattern recognition receptors, antimicrobial peptides (AMPs), iron-binding proteins, the complement system, and lysozymes, can be traced back to e …

Gram-positive infections continue to pose a therapeutic challenge owing to their invasive nature (e.g., Staphylococcus aureus and Streptococcus pyogenes) and eradication difficulties (e.g., methicillin-resistant Staphylococcus aureus [MRSA] and vancomycin-resistant enterococci [VRE]). Long-acting lipoglycopeptides (laLGPs) present an effective and conveniently dosed option for the treatment of most invasive Gram-positive pathogens. Improved compliance monitoring associated with their infrequent intravenous dosing, that is not available to such degree with more frequently dosed antimicrobials o…

Anorectal bacterial sexually transmitted infections, predominantly chlamydia, gonorrhoea and syphilis, represent a substantial and evolving global health challenge, with incidence rising despite widespread testing availability. In this Review, we examine the epidemiology and burden of anorectal sexually transmitted infections, explore the pathogenic mechanisms underlying infection and discuss advances in diagnosis, screening and prevention strategies. Anorectal infections are frequently asymptomatic, particularly among men who have sex with men, in which they often exceed the prevalence of uro…

BACKGROUND: Deep neck infections are potentially life-threatening conditions that can spread rapidly due to the complex anatomy of the cervical fascial spaces. They most commonly originate from infections of the upper aerodigestive tract or odontogenic sources and remain associated with significant morbidity and mortality despite advances in treatment.
OBJECTIVE: The aim of this study is to outline the pathophysiology, diagnosis, and management of deep neck infections, emphasizing the importance of timely intervention in accordance with the principle of "time is tissue."

Why revisit our understanding of relapsing fever and their causative spirochaetes? We have seen many existing paradigms of relapsing fever borreliosis challenged over recent years, largely facilitated through technological molecular biological improvements. Concepts that were believed for many years are now being questioned; from the origins of B. recurrentis, the epidemic form of relapsing fever, through to the one-tick-one Borrelia model. Furthermore, the description of relapsing fever borreliae such as B. miyamotoi transmitted by hard tick species and increasing reports of new borrelial spe

Maternal sepsis remains a major and largely preventable contributor to maternal morbidity and mortality, with intra-amniotic infection and postpartum endometritis remaining common obstetric sources of sepsis. Intra-amniotic infection complicates approximately 2%-5% of term deliveries, and postpartum endometritis occurs in up to 7% of births. Both conditions are typically polymicrobial, arising from ascending infection involving gram-positive, gram-negative, and anaerobic organisms. Despite rising antimicrobial resistance, the recommended antibiotic regimens for obstetric infections have remain…

Neutrophil extracellular traps (NETs) are crucial innate immune components that ensnare and neutralize pathogens. Inspired by this, we engineered a novel peptide, RFC, designed to mimic NETs' "trap-and-kill" strategy against Staphylococcus aureus infections. RFC integrates an antimicrobial peptide (KR12), a self-assembling motif (KLVFF), and a Staphylococcus-targeting sequence (CARGGLKSC). In vitro, RFC exhibited potent broad-spectrum activity (minimum inhibitory concentration (MIC) as low as 4 μM), fast bactericidal kinetics (>3-log10 reduction within 2 h at 1× MIC), inhibited biofilm formati…

This review brings together global data on bacterial, fungal, nontuberculous mycobacterial (NTM) and viral infections that complicate the course of individuals following the diagnosis of pulmonary or extrapulmonary tuberculosis (TB). It shows how residual structural lung damage (bronchiectasis, cavitation, residual fibrotic lesions) and persistent immune dysregulation create niches for pathogens, most notably Pseudomonas aeruginosa, Aspergillus spp., Mycobacterium avium complex and herpes zoster virus. Although bacterial infections in post-TB bronchiectasis are the most consistently documented…

Invasive non-typhoidal Salmonella (iNTS) disease, primarily manifesting as bloodstream infections and dominated by Salmonella Typhimurium sequence type 313 (ST313), represents a major global health challenge, particularly in sub-Saharan Africa. However, the transmission landscape and evolutionary driver of ST313 remain incompletely understood. Here, we compile the global dataset to date, comprising 11,361 human-derived iNTS genomes and 3115 ST313 genomes from diverse sources, enabling identification of a highly antimicrobial-resistant sublineage 2.4 and an emerging lineage 4. Phylogenomic anal…

PURPOSE OF REVIEW: Invasive mold infections (IMIs) remain a major cause of morbidity and mortality in patients with hematological malignancies, particularly when critical illness develops. Advances in hematological therapies and intensive care medicine have increased the number of high-risk patients admitted to intensive care units (ICUs), where invasive aspergillosis and other mold infections represent important causes of acute respiratory failure and sepsis. This review summarizes current knowledge on IMIs in critically ill hematological patients, focusing on epidemiology, diagnosis, and man…
