Oesophageal stenting in a district general hospital.

BACKGROUND: Oesophageal carcinoma is the ninth most common malignancy in the UK with five year survival rate of only 8%. Most patients with dysphagia present at an advanced stage. Endoscopic stent insertion is a valid method of palliation. We present the outcomes of patients stented at a district general hospital. METHOD: This is a retrospective study of patients presenting to a district general hospital with inoperable oesophageal obstruction resulting in stent insertion from December 2000 to September 2006. Case notes were reviewed for demographics, diagnosis, treatment, complication and ou

Management of endoscopic retrograde cholangiopancreatography-related perforation.

Endoscopic retrograde cholangiopancreatography (ERCP)-related perforation occurs in 0.1-1.8% of patients after therapeutic ERCP. It is a serious complication with a high mortality. However, little evidence exists in the medical literature and the optimal management remains unclear. This review aims to critically evaluate the optimal management of therapeutic ERCP-related perforation.

Prosthetic repair of incisional hernia combined with elective bowel operation.

BACKGROUND AND AIMS: Incisional hernia repair with mesh is considered a clean operation and it is not recommended to be perfomed at the same time with a potentially contaminated operation. The aim of this study is to assess the short-term results of a group of patients who underwent a colon operation and simultaneous incisional hernia repair with an onlay polypropylene mesh technique. PATIENTS AND METHODS: From Novemberto June 2006, 19 patients underwent incisional hernia repair with polypropylene mesh, with simultaneous colonic operation. In 13 patients reestablishment of bowel continuity af

The Rossetti-Nissen fundoplication--effective in managing gastro-oesophageal reflux disease.

BACKGROUND: The report herein details a prospective audit of a unit's eight-year experience of the Rossetti-Nissen fundoplication using a predominantly laparoscopic technique, selective hiatal repair, no bougie and a standardised protocol of pre- and post-operative functional endoscopic and symptom assessment. METHODS: Three hundred and seventy-eight patients underwent the Rossetti-Nissen fundoplication. All patients had documented data on endoscopy, health related quality of life (HR-QoL), surgical details and complications pre-operatively and at six months post-operatively. Repeat physiolog

The recurrent laryngeal nerve in thyroid surgery.

The recurrent laryngeal nerve (RLN) is an essential nerve for phonation and psychosocial interaction. Throughout the annals oflaryngological history, vocal cord paralysis has been a well recognised disorder. A rich heritage of scientific investigation and research has considered the complexity of vocal cord function, laryngoscopic presentation and surgical rehabilitation. Identification and preservation of the RLN is the major concern in thyroid surgery. Controversy exists regarding the correct pre- and postoperative management in vocal cord evaluation. We fully review this topic and challenge

Nerve injury in lower limb vascular surgery.

OBJECTIVE: Nerve injury is one of the most common complications of varicose vein surgery and is a frequent cause for litigation but its incidence following lower limb arterial surgery has not been well documented. This study was undertaken to determine the incidence of nerve injury following lower limb arterial surgery. This was addressed in relation to long saphenous or femoral vein harvesting, or re-operative surgery. MATERIALS AND METHODS: A total of one hundred patients who had undergone lower limb arterial reconstruction in the previous five years were invited to participate in this stud

Minimally invasive parathyroidectomy and thyroidectomy--current concepts.

The introduction of various techniques for minimally invasive parathyroidectomy (MIP) and minimally invasive thyroid surgery (MITS) have changed both the conceptual and surgical approach to parathyroid disease and single thyroid nodules. Perceived advantages of minimally invasive surgery both among clinicians and patients, have been a major factor in the development of new surgical techniques, as well as refinement in preoperative localisation techniques. Worldwide the number of patients being operated on using MIP or MITS has steadily increased. At some major centres as many as 70% of patient

Proposals for the management of gastrointestinal stromal tumours of the stomach.

BACKGROUND AND AIMS: It has been reported that gastric gastrointestinal stromal tumours (GIST) are aggressive, rare and difficult to treat. Some have advocated radical resection as the only potential cure. We present data to support treatment of gastric GISTs with a limited surgical approach and minimal morbidity. Furthermore, we propose that surveillance for recurrence is unnecessary based upon the follow-up of a cohort of patients with gastric GISTs. METHODS: Database and case notes analysis of 20 patients diagnosed with gastric GIST (1998-2004) and managed by one surgeon in a single centre

Mortality and morbidity associated with late diagnosis of anorectal malformations in children.

AIM: The aim of this study was to highlight the timing of diagnosis of anorectal malformations and the morbidity and mortality associated with any delay. METHODS: We retrospectively reviewed all patients with anorectal malformations presenting to the Children's Hospital, Leicester between 1998-2005. Patient demographics included age at diagnosis, presenting complaint, type of anorectal malformation and sex. Associated malformations were carefully recorded as well as morbidity and mortality occurring as a consequence of the delay in diagnosis. RESULTS: Out of a total of 52 patients, 28 (53%)

Incentive spirometry decreases respiratory complications following major abdominal surgery.

Pulmonary complications are the leading cause of morbidity and mortality following major abdominal surgery. Chest physiotherapy aims to decrease the likelihood of these complications and hasten recovery. Exercises aimed at maximising inspiratory effort are the most beneficial for the patients. The incentive spirometer is a handheld device that patients use to achieve effective inspiration. In a nonrandomised pilot study of 263 patients we have found that the addition of the incentive spirometer, as part of an intensive post-operative physiotherapy programme, decreased the occurrence of pulmona

Extremity amputation in Nigeria--a review of indications and mortality.

Amputation is a common surgical procedure in Nigeria but there are no national data on the subject. The objective was to determine the common indications for amputation in Nigeria, mortality rate, and regional differences in indications. Results of studies on amputation in Nigeria over a 15-year period were collated and analysed. The most frequent indications for amputation were trauma (34%); complication of traditional bonesetting (TBS) (23%); malignant tumours (14.5%); diabetic gangrene (12.3%); infections (5.1%); peripheral artery disease (2.1%); and burns (2.1%). In the southern regions, t

Diabetic foot ulcers--a comprehensive review.

As the incidence of diabetes mellitus is increasing globally, complications related to this endocrine disorder are also mounting. Because of the large number of patients, foot ulcers developing in the feet of diabetics have become a public health problem. The predisposing factors include abnormal plantar pressure points, foot deformities, and minor trauma. Vulnerable feet usually already have vascular insufficiency and peripheral neuropathy. The complex nature of these ulcers deserves special care. The most useful prognostic feature for healing remains the ulcer depth, ulcers heal poorly if th

One stage procedure in the management of acute sigmoid volvulus without colonic lavage.

BACKGROUND: There is a growing acceptance of one-stage primary resection and anastomosis of left-sided colon obstruction with on-table antegrade colonic lavage to reduce the risk of post-operative infectious complications and anastomotic dehiscence. The purpose of this study was to evaluate the safety of single-stage resection and anastomosis for acute left-sided colonic obstruction due to acute sigmoid volvulus, without intraoperative colonic lavage, in a consecutive series of patients admitted to our department. METHODS: Emergency resection of acute sigmoid volvulus was performed by an expe

The Addis Ababa fistula hospital: an holistic approach to the management of patients with vesicovaginal fistulae.

Ethiopia is amongst the world's poorest countries. It is estimated that approximately 30,000 women have an untreated vesicovaginal fistula, the overwhelming proportion of which are complications of neglected, prolonged or obstructed labour. When unrelieved, the presenting foetal part is impacted against the soft tissues of the pelvis, resulting in a widespread ischaemic injury. This 'field injury' often results in multiple other injuries, in addition to a vesicovaginal fistula. Focusing simply on the 'hole' between the bladder and vagina will ignore many of the other injuries these women have

Intramedullary reamings for the histological diagnosis of suspected pathological fractures.

The aim of this study was to evaluate how effective reamings taken at the time of surgery for fracture were in the diagnosis of malignancy. All histology reports over a five-year period were examined. Ninety cases, where reamings were taken, were identified and included. Reamings are commonly sent for histological diagnosis in orthopaedic practice. A definitive tissue diagnosis was established in 55 samples (60%). No diagnosis was established in 30 (33%), as the tissue sample was deemed inadequate, 17 of which were known to have a malignancy elsewhere. Five case reports were lost. The process

Managing femoral head osteonecrosis in patients with sickle cell disease.

With an increasing number of people immigrating between different countries, sickle cell disease (SCD) is spreading all over the world. Due to improved health care, the life span of SCD patients has increased and many of them live to adulthood and middle-age. Osteonecrosis of the femoral head is one of the most common musculo-skeletal problems in SCD patients. Once osteonecrosis starts in the femoral head, it can progress from early to late stages in just a few years. Managing osteonecrosis of the femoral head in young-adults is a challenging problem and, in many situations, it requires major

A stepped care approach to reduction of anterior shoulder dislocation in the prone position.

There are various methods of reducing anterior shoulder dislocations. The traditionally used methods like Kocher's and Hippocratic are associated with procedural complications and frequently require sedation/muscle relaxants which have their own complications and are associated with longer stay and monitoring requirement in the emergency department. We describe a stepped care approach to shoulder dislocation reduction in the prone position involving four steps and two new manipulation methods taking into consideration the pathological deforming forces in a dislocated shoulder and addressing th
