Association of Serum Biochemical Biomarker Profiles of Joint Tissue Inflammation and Cartilage Metabolism With Posttraumatic Osteoarthritis-Related Symptoms at 12 Months After ACLR.

BACKGROUND: Anterior cruciate ligament injury and anterior cruciate ligament reconstruction (ACLR) are risk factors for symptomatic posttraumatic osteoarthritis (PTOA). After ACLR, individuals demonstrate altered joint tissue metabolism indicative of increased inflammation and cartilage breakdown. Serum biomarker changes have been associated with tibiofemoral cartilage composition indicative of worse knee joint health but not with PTOA-related symptoms. PURPOSE/HYPOTHESIS: The purpose of this study was to determine associations between changes in serum biomarker profiles from the preoperative

In-line skating injuries in children: a 10-year review.

In-line skating is an increasingly popular recreational activity for both children and adults. As with any high-speed sport, traumatic injury is an associated hazard. It was the purpose of this study to focus on the injuries in children associated specifically with in-line skating with an emphasis on education and prevention. During a 9-year period, 331 injuries were identified, with 105 families completing an in-depth epidemiologic questionnaire. Sixty-one percent of the victims were boys, with an overall average age of 12 years. The upper extremity was most commonly injured (64%); the head a

Six-month post-surgical elevations in cartilage T1rho relaxation times are associated with functional performance 2 years after ACL reconstruction.

The current study looks to: (i) investigate postural stability following anterior cruciate ligament (ACL) reconstruction, as assessed by Y-Balance Test, by comparing single-leg balance of the injured limb against those of controls and the uninjured limb; (ii) analyze the relationship between postural stability symmetry with localized cartilage matrix changes and the Knee Injury and Osteoarthritis Outcome Score (KOOS). Bilateral knee MRI of 36 patients who underwent ACL reconstruction were performed before surgery, 6 months, 1 year, and 2 years, postoperatively. Postural stability was evaluated

A clinical composite score accurately detects meniscal pathology.

PURPOSE: Five common tests were used to diagnose meniscal tears when used as a composite score. We evaluated how effectively the composite examination, when performed in the presence of an anterior cruciate ligament (ACL) injury or degenerative joint disease (DJD), determined the presence of meniscal tears. METHODS: Data were collected prospectively on all patients at our clinic with a primary knee complaint. Independent variables included the presence or absence of the following: (1) a history of "catching" or "locking" as reported by the patient, (2) pain with forced hyperextension, (3) pai

[Minor head injury can result in fracture of the anterior wall of sinus frontalis].

A 17 year-old boy suffered a minor head injury after a somersault in which his knee hit the forehead. He had a swelling over the right frontal sinus with unbroken skin. A computed tomography revealed a fracture of the anterior wall of the frontal sinus with a displaced bone fragment. The posterior wall was undamaged, and no intracranial injury was evident. The nasofrontal duct was intact. The fragment was repositioned surgically without complications. The frontal sinus is considered resistant to low-energy trauma. This dogma, however, has exceptions.

Common peroneal nerve palsy in a UH-60 aviator.

A case of common peroneal nerve palsy in a UH-60 Blackhawk U.S. Army helicopter pilot is reported. A review of the literature revealed several reports of common peroneal nerve palsy, although there were no published reports of this injury secondary to performing flight duties in the UH-60 cockpit. A common practice among Blackhawk pilots is to brace the "collective" with their left knee, subjecting the common peroneal nerve to possible injury. This action should be considered as a possible cause of common peroneal nerve palsy in this select group of aviators.

Rotational displacement of the distal tibial epiphysis without fibular fracture.

Rotational displacement of the distal tibial epiphysis, a rare injury, is reported in a 14-year-old girl. A high clinical index of suspicion and roentgenograms of the entire leg, including knee and ankle, are necessary for diagnosis. Long-term followup is indicated to determine if epiphyseal closure will eventuate as it did in our patient.

Semitendinosus and biceps femoris long head active stiffness response until failure in professional footballers with vs. without previous hamstring injury.

This study sought to examine the active stiffness of semitendinosus (ST) and biceps femoris long head (BFlh) during a knee flexor isometric contraction at 20% of maximal voluntary isometric contraction until failure in elite footballers (n = 50, age: 22.3 ± 5.3 years; height: 1.82 ± 0.08 m; body mass: 74.7 ± 9.0 kg). Active stiffness was assessed using ultrasound-based shear wave elastography by means of shear modulus quantification. Comparisons were performed between limbs with (n = 11) vs. without (n = 89) previous hamstring injury. A similar time until failure in the knee flexor fatigue tas

Isolated posterior cruciate ligament tears in monozygotic twins: A case report.

INTRODUCTION AND IMPORTANCE: Posterior cruciate ligament (PCL) tears are not as frequent as anterior cruciate ligament (ACL) tears. They are rare as an isolated injury and more commonly occur in a multi-ligament-injured knee. We reported a case of rupture of PCL in monozygotic twins. CASE PRESENTATION: A 19-year-old female presented with giving away of her left knee since 4 months ago. She fell from stairs previously. Physical examination demonstrated positive posterior drawer test. Magnetic resonance imaging showed ruptured PCL and lateral meniscal tear. Two months later, her identical twin,

Impact of Psychological Factors on Rehabilitation After Anterior Cruciate Ligament Reconstruction: A Systematic Review.

CONTEXT: Anterior cruciate ligament (ACL) tears are a common orthopaedic injury, and the incidence of ACL reconstruction (ACLR) continues to increase. Current clinical practice guidelines (CPGs) recognize the role of psychological factors in rehabilitation, but patient-reported outcome measures (PROs) and psychological readiness are rarely incorporated into rehabilitation. OBJECTIVE: The purpose of this review was to highlight the importance of psychological health after ACL injury, understand the current metrics used to monitor psychological recovery, and outline how psychological recovery c

Integrative analysis of miRNA in cartilage-derived extracellular vesicles and single-cell RNA-seq profiles in knee osteoarthritis.

Extracellular vesicular miRNAs (EV-miRNAs) play essential roles as intercellular communication molecules in knee Osteoarthritis (OA). We isolated cartilage-derived extracellular vesicles (EVs), to perform miRNA sequencing, which revealed EV-miRNA profiles and identified differentially expressed miRNAs (DE-miRNAs) between cartilage injury and cartilage non-injury groups. The target genes of known and novel DE-miRNAs were predicted with multiMiR package in 14 miRNA-target interaction databases. Meanwhile, single-cell RNA sequencing (scRNA-seq) was performed to identify chondrocyte clusters and t

Total Knee Arthroplasty versus Osteochondral Allograft: Prevalence and Risk Factors following Tibial Plateau Fractures.

Orthopaedic surgeons commonly have the misconception that patients with tibial plateau fractures will likely go on to posttraumatic knee arthritis requiring total knee arthroplasty (TKA). In younger patients, osteochondral allograft (OCA) transplantation is an alternative method to address posttraumatic knee arthritis. The purpose of this study was (1) to identify our institutional failure rate following tibial plateau open reduction and internal fixation (ORIF) (failure was defined as conversion to TKA or OCA); (2) to determine if there are patient- or injury-related risk factors predictive o

Effects of an aromatherapy-integrated exercise program on knee pain and physical function in community-dwelling older adults with osteoarthritis.

Osteoarthritis (OA) is a major cause of chronic pain in older adults and negatively affects physical function and daily life. Aromatherapy is a complementary therapy with potential pain-relieving effects. However, evidence supporting its use alongside exercise in older adults is limited. In this single-blind randomized controlled trial, participants with knee pain ≥ 40 mm on the visual analog scale (VAS) were assigned to either an aromatherapy combined with exercise group (ACG) or an exercise control group (ECG). Both groups participated in 60-minute rehabilitation sessions twice weekly for 8

Experimentally Induced Knee Effusion Does Not Simulate Sustained Ground Reaction Force Profiles During Overground Walking.

Less dynamic limb-level loading is observed bilaterally in individuals who have undergone unilateral anterior cruciate ligament reconstruction (ACLR) and is linked with the development and progression of knee osteoarthritis. Experimentally induced knee effusion (EIKE) models have been used to study biomechanical effects of knee injury, showing decreased quadriceps activation and lesser peak limb-level loading (ie, vertical ground reaction force [vGRF]) during stair descent and decreased knee extensor torque during overground walking. However, it is unknown whether EIKE acutely induces less dyn

Total Hip Arthroplasty after McMurray's Osteotomy - A Case Report and Review of Literature.

INTRODUCTION: Opening-wedge high tibial osteotomy (OWHTO) is a widely used procedure to improve knee function, decrease knee pain and delay arthroplasty in unicompartmental knee osteoarthritis in young patients. However, faulty technique can lead to patella baja and maltracking of extensor mechanism- leading to poor outcome. Modified biplanar downsloping OWHTO - where tibial tuberosity (TT) remains attached to proximal osteotomy fragment prevents patella baja by preventing TT to slide down during opening of osteotomy. Biplanar osteotomy also prevents rotation of distal tibial fragment. Biplana

Outcomes of a conservative care program for knee osteoarthritis through translation of research into practice.

INTRODUCTION: Evidence-based guidelines suggest a conservative, nonsurgical approach as first-line treatment for knee osteoarthritis. However, previous literature has documented underutilization of the fundamental components of condition management emphasized in the guidelines. The intervention aim is to apply organized conservative components of care for knee osteoarthritis in an evidence-based management program through the translation of research into practice with a target to observe meaningful functional improvement in a distinct population. METHODS: The program, modeled after the nonsur
