A variety of surgical procedures are available in the treatment of the carcinoid syndrome, the aim being the destruction of liver secondaries and amelioration of symptoms. The urinary excretion of N,N-dimethyltryptamine (DMT) was higher in patients with severe liver disease than in normal subjects. This difference remained significant when patients with all grades of hepatic encephalopathy were excluded. Patients with liver disease whose mental states were normal excreted amounts of DMT similar to those of patients with a hospital diagnosis of schizophrenia.

Liver biopsies in nine of 70 male alcoholics seen during a 12 month period showed predominantly portal fibrosis with fatty change, but little or no alcoholic hepatitis. Patients with severe acute or chronic liver disease were found to have a high mean plasma free tryptophan, an abnormal urinary excretion pattern of tryptophan-kynurenine metabolites and low circulating levels of the vitamins required for tryptophan degradation, i.e. pyridoxine, thiamine and ascorbic acid. Two cases of hemangiosarcoma of the liver and spleen are described. In the first case, a great number of cavernous blood vessels has been found and this, according to the authors, may support the idea that the tumour aroses within the benign hemangioma. In the other case a thrombocytopenia, a rare complication of the liver hemangioblastoma, has been found. It should be stressed that in spite of a malignant nature of these tumours, neoplastic foci were limited only to the liver and the spleen.

This study is based on the experience with 51 cases of hepatic alveolar echinococcosis underwent operative treatment up to the end of 1976. Hepatic resection was carried out in 28 cases with overall operative mortality of 25 per cent, but no death occurred in the last 10 years period during which 12 cases were subjected to the procedure. This apparent improvement of the result is ascribed to the establishment of the strict criteria for operative intervention, i.e., when less than three segments are involved, the hepatic hilum is not highly involved, and the inferior vena cava is not invaded.
