Prevalence of Hypomagnesemia in ICU Patients at a Tertiary Care Center: A Prospective Observational Study.

BACKGROUND: Magnesium, the fourth most abundant cation in the human body, plays a crucial role in enzymatic activity, neurochemical transmission, and muscular excitability. Hypomagnesemia in critically ill patients has been associated with neuromuscular dysfunction, cardiovascular complications, metabolic disturbances, and increased morbidity. This study aims to determine the prevalence of hypomagnesemia in critically ill patients and evaluate its impact on disease severity, length of ICU stay, ventilator dependency, and mortality. METHODS: This prospective observational study was conducted i…

Essential Roles of Nutrient Factors in Learning and Memory.

Essential nutrient factors, including water- and fat-soluble vitamins, minerals, and essential amino acids, play an important role in brain function. We have investigated the roles of these nutrients in learning and memory in mice. Interestingly, we found that dietary deficiency of vitamin B1 or magnesium, inhibition of the vitamin A signaling pathway, and restricted intake of tryptophan impair hippocampus-dependent memory. Furthermore, magnesium deficiency causes neuroinflammation in the hippocampus. Conversely, dietary heat-killed Lactobacillus species enhance hippocampus-dependent memory. T…

Tetany by transfusion of small volume of blood: a rare case report.

A requisition for two units of packed red blood cells was received for a 54 year female, known case of genitourinary carcinoma. After transfusion of approximately 15-20 ml of blood, a call was received from resident in charge of radiotherapy ward stating that patient had clenching of hands along with circumoral tingling and paresthesias in her limbs. Her investigations showed decreased serum potassium and calcium levels but serum magnesium levels were not available. Multiple electrolyte disturbances probably precipitated tetany even by small volume of blood transfusion. We therefore recommend …

Gitelman syndrome-associated severe hypokalemia and hypomagnesemia: case report and review of the literature.

We report a case of a woman with Gitelman syndrome who presented to our hospital mainly due to hyperemesis. Following her admission, intravenous potassium and magnesium supplementation was commenced to counter the observed hypokalemia and hypomagnesemia. Hyperemesis receded and although serum potassium remained low, she became asymptomatic. Oral potassium and magnesium supplementation was administered throughout pregnancy and biweekly ion level measurements were scheduled. Despite the intensive replacement, ion levels remained constantly low. She delivered at 38 weeks with an elective caesarea…

Changed serum trace element profile in Down's syndrome.

Being cofactors of important antioxidant enzymes superoxide dismutase (SOD) and glutathione peroxidase (GPx), which are significantly modified in Down's syndrome (trisomy 21), serum levels of microtrace elements zinc, copper, and selenium and of macroelement magnesium are reported in 16 subjects with Down's syndrome (DS) and their respective well age- and sex-matched controls. Serum zinc and selenium levels were significantly lowered in DS subjects, whereas copper levels were elevated. Consequently, a marked increase (40%) of the copper/zinc ratio in DS persons was observed. There were no diff…

Taste-Masked orodispersible tablets of alfuzosin hydrochloride: response surface methodology (RSM) based approach for enhanced patient compliance in BPH management.

Alfuzosin hydrochloride, an α1-adrenergic antagonist used in the treatment of benign prostatic hyperplasia (BPH), was formulated as orodispersible tablets (ODTs) to improve patient compliance, particularly in geriatric patients with dysphagia. Tablets were prepared by direct compression using microcrystalline cellulose as a filler-binder, crospovidone as a superdisintegrant, magnesium stearate as a lubricant, and aspartame (2%) as a taste-masking agent. Pre-compression studies confirmed satisfactory flow properties, compressibility, and drug-excipient compatibility. A Box-Behnken Design (BBD) …

Prevention of hypokalaemia and hypomagnesaemia following peripheral stem cell collection - a prospective cohort study.

BACKGROUND AND OBJECTIVES: Citrate-based anticoagulation reduces plasma potassium and free magnesium in patients undergoing peripheral stem cell collections. Whether the effects may be mitigated by pre-procedure oral electrolyte supplements has not been previously assessed. MATERIALS AND METHODS: Results from a historic cohort (2010-2013) guided a systematic prospective intervention in subjects deemed at risk for clinically meaningful hypokalaemia and hypomagnesaemia. From 2015 to 2019, 136 patients were enrolled in the study. Pre- and post-apheresis electrolyte levels were measured, and oral…

Reactivity of the carbodiphosphorane, (Ph3P)2C, towards main group metal alkyl compounds: coordination and cyclometalation.

The carbodiphosphorane, (Ph3P)2C, reacts with Me3Al and Me3Ga to afford the adducts, [(Ph3P)2C]AlMe3 and [(Ph3P)2C]GaMe3, which have been st … dN(SiMe3)2, are also obtained upon reaction of (Ph3P)2C with Zn[N(SiMe3)2]2 and Cd[N(SiMe3)2]2. The magnesium compounds, Me2Mg and {Mg[N(SiMe3)2]2}2, likewise react with (Ph3P)2C to afford cyclometalated derivatives, namely [κ2-Ph3PC{PPh2(C6H4)}]MgN(SiMe3)2 and {[κ2-Ph3PC{PPh2(C6H4)}]MgMe}2. While this reactivity is similar to the zinc system, the magnesium …

Nickel reduces calcium dependent dimerization in neural cadherin.

Cadherins are the transmembrane component in adherens junctions, structures that link the actin cytoskeletons in adjacent cells within solid tissues including neurological synapses, epithelium and endothelium. Cell-cell adhesion by cadherins requires the binding of calcium ions to specific sites in the extracellular region. Given the complexity of the cell adhesion microenvironment, we are investigating whether other divalent cations might affect calcium-dependent dimerization of neural (N) cadherin. The studies reported herein characterize the impact of binding physiological magnesium(ii) or …

Deletion of claudin-10 rescues claudin-16-deficient mice from hypomagnesemia and hypercalciuria.

The tight junction proteins claudin-10 and -16 are crucial for the paracellular reabsorption of cations along the thick ascending limb of Henle's loop in the kidney. In patients, mutations in CLDN16 cause familial hypomagnesemia with hypercalciuria and nephrocalcinosis, while mutations in CLDN10 impair kidney function. Mice lacking claudin-16 display magnesium and calcium wasting, whereas absence of claudin-10 results in hypermagnesemia and interstitial nephrocalcinosis. In order to study the functional interdependence of claudin-10 and -16 we generated double-deficient mice. These mice had no…

Cu/MgAl(2)O(4) as bifunctional catalyst for aldol condensation of 5-hydroxymethylfurfural and selective transfer hydrogenation.

Copper supported on mesoporous magnesium aluminate has been prepared as noble-metal-free solid catalyst for aldol condensation of 5-hydroxymethylfurfural with acetone, followed by hydrogenation of the aldol condensation products. The investigated mesoporous spinels possess high activity as solid-base catalysts. Magnesium aluminate exhibits superior activity compared to zinc and cobalt-based aluminates, reaching full conversion and up to 81 % yield of the 1:1 aldol product. The high activity can be correlated to a higher concentration of basic surface sites on magnesium aluminate. Applying cont…

Water-soluble organo-building blocks of aminoclay as a soil-flushing agent for heavy metal contaminated soil.

We demonstrated that water-soluble aminopropyl magnesium functionalized phyllosilicate could be used as a soil-flushing agent for heavy metal contaminated soils. Soil flushing has been an attractive means to remediate heavy metal contamination because it is less disruptive to the soil environment after the treatment was performed. However, development of efficient and non-toxic soil-flushing agents is still required. We have synthesized aminoclays with three different central metal ions such as magnesium, aluminum, and ferric ions and investigated applicability of aminoclays as soil flushing a…
