Developing shared understanding of pre-eclampsia in Haiti and Zimbabwe using Theory of Change.

Pre-eclampsia, a complex and multi-system disorder specific to pregnancy, is a leading cause of preventable maternal and perinatal deaths in low-resource settings. Early detection and appropriate intervention with management of hypertension, prevention of eclampsia and timely delivery are effective at reducing mortality and morbidity. Outcomes can be greatly improved with the provision and uptake of good quality care. Cultural contexts of maternal care, social practices and expectations around pregnancy and childbirth profoundly shape understanding and prioritisation when it comes to seeking o

Effects of testosterone on rat placental development.

We investigated the morphological effects of testosterone on placental development in a rat model of polycystic ovarian syndrome (PCOS). Testosterone propionate (TP), which was subcutaneously administered to pregnant rats with 5 mg/animal from gestation day (GD) 14 to GD 18, induced a maternal weight reduction without mortality or clinical signs from GD 19 onwards. A decrease in fetal and placental weight, an increase in intrauterine growth retardation (IUGR) rates, and histological changes in the placenta were observed on GD 21 but not on GD15 or 17. Histopathologically, on GD 21, the trophob

Maternal immune responses and obstetrical outcomes of pregnant women with COVID-19 and possible health risks of offspring.

Coronavirus disease 2019 (COVID-19) pandemic has spread rapidly across the world. The vast majority of patients with COVID-19 manifest mild to moderate symptoms but may progress to severe cases or even mortalities. Young adults of reproductive age are the most affected population by SARS-CoV-2 infection. However, there is no consensus yet if pregnancy contributes to the severity of COVID-19. Initial studies of pregnant women have found that COVID-19 significantly increases the risk of preterm birth, intrauterine growth restriction, and low birth weight, which have been associated with non-comm

Pregnancy-Associated Stroke.

Cerebrovascular accident, or stroke, is the fourth leading cause of death for all women and the eighth leading cause of pregnancy-associated death. The physiologic changes of pregnancy increase the risk of cerebrovascular accident for women. With current incidence rates, a facility with 3300 births per year can anticipate caring for one woman with a pregnancy-related stroke at least every 2 years. All maternity care providers must be able to assess women experiencing stroke-like symptoms and initiate timely care to mitigate brain tissue damage, decrease long-term morbidity, and prevent mortali

Committee Opinion No. 638: First-Trimester Risk Assessment for Early-Onset Preeclampsia.

Hypertensive disorders with adverse sequelae (including preterm birth, maternal morbidity and mortality, and long-term risk of maternal cardiovascular disease) complicate 5-10% of pregnancies. Early identification of pregnant women at risk of developing early-onset preeclampsia would theoretically allow referral for more intensive surveillance or application of preventive therapies to reduce the risk of severe disease. In practice, however, the effectiveness of such triage would be hindered by the low positive predictive value for early-onset preeclampsia reported in the literature. In spite o

Ischemic heart disease in pregnancy.

While ischemic heart disease in reproductive-age women is rare, cardiac disease is a leading cause of maternal mortality. In turn, coronary artery disease is one of the most common causes of maternal cardiac death. The incidence of coronary artery disease in pregnancy may be rising due to the increasing prevalence of comorbid risk factors. Diagnosis and clinical management of ischemic cardiac disease is largely similar in the pregnant and non-pregnant patient, and the majority of medications and diagnostic and interventional procedures are compatible with pregnancy with a few important excepti…

Ductus-dependent fetal cardiac defects contraindicate indomethacin tocolysis.

The hemodynamics of critical aortic stenosis in the fetus make it a ductus-dependent cardiac defect because the ductus arteriosus supplies blood not only to the descending aorta but also to the aortic arch and coronary vessels. In utero closure of the ductus arteriosus has been reported in association with tetralogy of Fallot, truncus arteriosus, maternal use of prostaglandin inhibitors, and as idiopathic events. This is the first report of a ductus-dependent congenital heart defect (critical aortic stenosis) where treatment with indomethacin, a prostaglandin synthetase inhibitor, precipitated

[Diagnostic and therapeutic problems in HELLP syndrome].

The HELLP syndrome is a severe and life-threatening complication of preeclampsia. Despite obstetricians are more aware of this syndrome maternal and perinatal mortality could not be reduced during the last years. The diagnostic problems of the HELLP syndrome are early detection of the disease, problems concerning differential diagnosis to other diseases with similar symptoms, and the different definition of the HELLP syndrome itself. For early detection of hemolysis determination of serum haptoglobin should be included in laboratory screening methods. Manifestation of DIG is not a principal si

The HELLP syndrome (hemolysis, elevated liver enzymes, and low platelets): much ado about nothing?

The question of whether the HELLP syndrome exists as a distinct entity or is part of a spectrum of pregnancy complications, which have in common hemolysis, elevated liver enzymes, and thrombocytopenia, has long been a source of speculation and debate among obstetricians and internists. A review of the literature indicates a definite need for a uniform definition, diagnosis, and management of this syndrome. Patients manifesting this syndrome usually are seen before term (less than 36 weeks' gestation) complaining of malaise (90%), epigastric or right upper-quadrant pain (90%), and nausea or vom

[Hypo- and hyperparathyroidism in pregnancy].

During pregnancy the maternal calcium metabolism is submitted to special loads. Overchallenging of the functional capacity of the parathyroid gland may evoke an insufficiency of the parathyroid gland which on its part induces a fetal secondary hyperparathyroidism. Therefore, the early diagnostic recognition of the insufficiency of the parathyroid gland is of importance, since (also in basic diseases known before pregnancy) now special therapeutic conditions are given. Maternal hyperparathyroidism leads to fetal hypoprarthyroidism with resulting developmental disturbances; it furthers the tende

Contemporary Approach to Postpartum Hemorrhage: Early Diagnosis and Evidence-Based Therapeutic Management.

Postpartum hemorrhage remains a major obstetric emergency and one of the most important contributors to maternal morbidity and mortality worldwide. Effective management depends on early recognition, systematic evaluation, and rapid implementation of evidence-based therapeutic strategies. A structured clinical approach facilitates identification of the underlying cause and supports timely intervention to prevent progression to severe hypovolemia and organ dysfunction. Risk assessment should begin during the antenatal period, as several maternal and obstetric conditions increase the likelihood o

Immunology of Normal Pregnancy and Preeclampsia and the Role of Placental Non-Classical HLA and Decidual NK Cells.

Pregnancy is considered a unique immunological process in which contact between the maternal innate immune system and the placental allograft results in maternal tolerance to paternally derived antigens. A growing body of research indicates that interactions between non-classical placental Human Leukocyte Antigen (NCHLA) and receptors on decidual Natural Killer (dNK) cells from early implantation are crucial to this process. Preeclampsia (PE), one of the leading causes of maternal and fetal morbidity and mortality, is also considered an autoimmune process. Currently, there is no treatment for

Successful preterm cesarean delivery in a woman with right atrial isomerism: a case report.

BACKGROUND: Right atrial isomerism is a rare condition marked by congenital splenic aplasia and associated cardiovascular and visceral malformations. The etiology of this condition remains unclear. Owing to the severity of concomitant cardiovascular anomalies, survival into adulthood is uncommon, and successful pregnancy outcomes are exceedingly rare. CASE PRESENTATION: A 30-year-old woman with a history of right atrial isomerism and a single right ventricle, who had undergone total cavopulmonary connection (TCPC) at age 15, was admitted at 32 weeks’ gestation due to decreased fetal movements.

Acute Fatty Liver of Pregnancy Presenting With Isolated Nausea and Vomiting in the Third Trimester: A Case Report.

Acute fatty liver of pregnancy (AFLP) is a rare but life-threatening obstetric emergency occurring in the third trimester. Its non-specific presentation frequently overlaps with other pregnancy-related conditions, leading to diagnostic delay and increased maternal and fetal morbidity and mortality. We report the case of a 27-year-old primigravida at 38 + six weeks of gestation presenting with isolated nausea and vomiting, without abdominal pain, hypertension, proteinuria, or obstetric complaints. Clinical examination revealed mild jaundice and stable hemodynamics. Initial laboratory investigat

Treatment of Syphilis in Pregnancy and Congenital Syphilis: Current Evidence, Challenges, and Future Directions.

Syphilis remains a global public health concern, with maternal infection posing a substantial risk for congenital syphilis, a preventable condition associated with severe morbidity and mortality. Penicillin, particularly benzathine penicillin G, remains the cornerstone of treatment and the only therapy with proven efficacy in preventing vertical transmission during pregnancy. However, recurrent global shortages, limited manufacturing capacity, mislabeling of penicillin allergy, and the absence of validated alternative regimens for pregnant women and neonates threaten progress toward eliminatio

Pregnancy and postnatal outcomes for women with intellectual disability and their infants: A systematic review.

BACKGROUND: While the perinatal period is a vulnerable time for women and their infants, it is also a window to promote adjustment and support. Women with intellectual disability might be a uniquely vulnerable group owing to pre-existing health and care inequalities. The aim of this paper is to explore the pregnancy and postnatal outcomes of women with intellectual disability and the health and development of their infants. METHODS: Three electronic databases (MEDLINE, PsycINFO, EMBASE) were searched for peer-reviewed papers that reported maternal pregnancy variables and infant outcomes withi

The impact of inadequate prenatal care on maternal anemia and pregnancy outcomes in Romania.

Prenatal care aims to ensure the well-being of both mother and baby by closely watching for potential problems and addressing them promptly. A lack of such care often stems from a combination of issues, including financial hardship, limited education, challenges in accessing healthcare, inadequate medical services, and insufficient social support. The study aimed to assess how iron supplements given during prenatal care affect the prevalence of anemia and whether lack of prenatal care leads to increased maternal and fetal morbidity. This analysis is based on the retrospective evaluation of med

Effective Communication During and After an Amniotic Fluid Embolism.

Amniotic fluid embolism (AFE) is a life-threatening complication occurring during pregnancy, labor, or birth and is characterized by sudden cardio-respiratory collapse and disseminated intravascular coagulopathy. Amniotic fluid embolism is unpredictable, sudden, and complex. Amniotic fluid embolism is one of the causes of peripartum maternal mortality and morbidity in the United States. Nurses and other health care professionals responding to severe maternal events, such as an AFE, may experience trauma, which can lead to reduced executive functions, and challenges with memory, organization, p…
