[Acute abdomen from the internal medicine point of view].

The interdisciplinary management of acute abdominal pain involves experienced specialists in both internal medicine and visceral surgery. The essential approach is to shorten the time between begin of symptoms, contact with a medical practitioner, diagnosis and therapy through identification of patients at risk, eg. of mesenteric ischemia. The characteristics of abdominal pain may help to identify the underlying disease. However, in children, in the elderly or in immune-compromised patients the symptoms may be nonspecific. Effective management of acute abdominal pain involves a careful history

Practical considerations in acne treatment and the clinical impact of topical combination therapy.

Acne vulgaris is a highly prevalent, multifactorial disease process affecting the pilosebaceous units of the dermis. The effects of acne are profound both physically and psychosocially across all age groups and ethnicities; therefore, prompt recognition and appropriate treatment are critical. Pharmacotherapeutic approaches range from simple monotherapy to multimodal treatments with a variety of topical and systemic agents. Optimal patient outcomes in acne management, however, are based both on treatment efficacy as well as overall patient satisfaction. In an effort to comprehensively address t

Major hepatobiliary surgery during pregnancy: safety and timing.

Hepatobiliary disease, although rare, may present during pregnancy with potential complications for mother and fetus. We present two cases of choledochal cysts and one case of a hepatic adenoma diagnosed in gravid patients. All three patients had acute events or failed medical management and were successfully treated with open resection, excision, or reconstruction during the second or third trimesters of pregnancy without requiring blood transfusions or tocolytic therapy. Although conservative treatment may be indicated in select patients due to the risk of underlying disease, we recommend su

[Gastroesophageal reflux in the adult: diagnosis and treatment].

Gastro-oesophageal reflux is a frequent problem affecting up to one third of the Belgian adult population. It is most often a benign non evolutive disease. It is however frequently chronic, with a significant negative impact on quality of life. The diagnostic and therapeutic management must be adapted to each individual case. In the majority of cases, the purpose of therapy is the control of symptoms and, in a small number of cases with severe oesophagitis the aim is a complete and persistent healing of lesions. A Franco-Belgian consensus meeting on diagnostic and therapeutic management of gas

The management of diabetes insipidus in adults.

The management of patients with diabetes insipidus can be confusing because of the disorder's variable pathophysiology, the numerous medications used, and the possible complications related to their use. Nevertheless, the primary care physician, rather than the subspecialist, will increasingly be called on to manage patients with such relatively uncommon conditions in the future. If a few basic facts and principles are kept in mind, the care of most patients with diabetes insipidus can be successful. A comprehensive, practical review of the short- and long-term therapy for patients with diabet

Treatment of respiratory failure due to kyphoscoliosis with nasal intermittent positive pressure ventilation (NIPPV).

Nasal intermittent positive pressure ventilation (NIPPV) represents a major advance in the management of respiratory failure. While its role in chronic respiratory failure is well documented, its use in acute respiratory failure is less well defined. We studied four patients with respiratory failure due to kyphoscoliosis, two of whom presented with acute respiratory acidosis. All four patients were on conventional maximum medical therapy prior to treatment with NIPPV. All patients survived to be discharged from hospital on home ventilation and significant improvements were noted in pO2, pCO2 a

Drug-induced and tardive movement disorders.

Drug-induced and tardive movement disorders represent a large number of extrapyramidal disorders seen in neurologic practice. Iatrogenically induced, most commonly by neuroleptics, these disorders can be characterized by any abnormal body movement including tremor, chorea, athetosis, dyskinesias, dystonia, myoclonus, tics, ballismus or akathisia. Parkinsonism, dyskinesias and dystonia tend to be the most common. Management of patients with drug-induced or tardive syndromes is complex. Prognosis is frequently poor as patients usually need the offending agent to manage their underlying psychiatr

Transvenous vena caval filtration and pulmonary embolectomy.

The diagnosis of deep vein thrombosis or pulmonary embolism can be made accurately using angiographic and radioisotopic techniques. Utilizing hemodynamic data, it is possible to classify patients with pulmonary embolism into five groups, which is helpful for planning therapy and assessing prognosis. Surgery for pulmonary embolism has evolved to include intraluminal methods of vena caval filtration for prevention of recurrent pulmonary embolism and transvenous extraction of pulmonary emboli. Though the majority of patients surviving pulmonary embolism can be managed medically with anticoagulati

Committee opinion no. 506: expedited partner therapy in the management of gonorrhea and chlamydia by obstetrician-gynecologists.

Expedited partner therapy is the clinical practice of treating the sex partners of patients, in whom sexually transmitted infections are diagnosed, by providing prescriptions or medications to the patient to take to his or her partner(s) without the health care provider first examining the partner(s). The American College of Obstetricians and Gynecologists supports expedited partner therapy in the management of gonorrhea and chlamydial infections when the partner is unlikely or unable to otherwise receive in-person evaluation and appropriate treatment. The legality of expedited partner therapy

Resources and care structures for management of urea cycle disorders in Japan and the United States: A system-level comparison.

BACKGROUND: Urea cycle disorders (UCD) are rare, nutrition-dependent inborn errors of metabolism in which outcomes depend on pharmacological treatment and sustained access to specialized formulas, low-protein medical foods, and dietary counseling. System-level support for dietary management varies across countries, yet comparative analyses are scarce. METHODS: We conducted a structured comparative analysis of dietary management systems for UCD in Japan and the United States (US) using clinical guidelines, policy documents, industry information, and professional reports. The review examined fo

Preoperative management of diabetes mellitus: A comparative narrative review of the recommendations of three professional organizations with Hamad Medical Corporation guidelines.

BACKGROUND: Diabetes mellitus (DM) is a prevalent condition that requires careful management in the perioperative setting to reduce surgical risks and optimize patient outcomes. The preoperative care of diabetic patients is complex because glucose control must be balanced with minimizing hypoglycemic or hyperglycemic events during surgery. A variety of diabetic medications such as insulin, dipeptidyl-peptidase-4 inhibitors, glucagon-like peptide-1 receptor agonists, sodium-glucose cotransporter-2 inhibitors, and biguanides such as metformin present unique challenges and considerations due to t

Psychopharmacology of COVID-19.

BACKGROUND: With the rapid, global spread of severe acute respiratory syndrome coronavirus 2, hospitals have become inundated with patients suffering from coronavirus disease 2019. Consultation-liaison psychiatrists are actively involved in managing these patients and should familiarize themselves with how the virus and its proposed treatments can affect psychotropic management. The only Food and Drug Administration-approved drug to treat COVID-19 is remdesivir, and other off-label medications used include chloroquine and hydroxychloroquine, tocilizumab, lopinavir/ritonavir, favipiravir, conva

Management of epithelial ovarian cancer in sub-Saharan Africa: a survey of practicing physicians.

The purpose of this study was to evaluate current practices in the management of epithelial ovarian cancer (OC) in sub-Saharan Africa (SSA). A 27-question survey was distributed to SSA-based physicians who managed OC and were members of the African Organisation for Research and Training in Cancer (AORTIC) via Research Electronic Data Capture. Questions evaluated diagnostics, imaging, surgical capacity, guideline adherence, access and barriers to systemic therapies, genetic testing and palliative care. Survey responses from 58 providers in 25 SSA countries were analysed. Clinical/medical oncolo

Effectiveness of a short duration program combining supervised exercises, nutritional advice and therapeutic education in patients with peripheral artery disease: A retrospective cohort study.

OBJECTIVES: Supervised exercise training (SET) for at least 3months is a cornerstone in the management of lower extremity peripheral artery disease (PAD). However, optimal duration of SET is discussed. This study aimed to evaluate the effectiveness of a short-duration program combining SET, nutritional advice and therapeutic education on maximal walking distance (MWD). Secondary objectives included assessing the impact of this program on smoking cessation, low-density lipoprotein cholesterol (LDL-c) levels, optimization of medical therapy, and to identify factors associated with MWD improvemen

Diagnostic challenges in culture-negative infective endocarditis: a case of histoplasma endocarditis: a case report.

INTRODUCTION: Histoplasma capsulatum is a fungal infection primarily associated with respiratory diseases and conditions that compromise the immune system. Cardiac involvement is extremely rare and presents a significant diagnostic challenge, mainly owing to often inconclusive results from blood cultures, histopathology, and serological tests. This case not only highlights a rare causative organism but also examines an atypical presentation of endocarditis linked to Histoplasma capsulatum. It underscores the importance of thoroughly evaluating the patient's medical history and underlying condi

Catheter ablation compared to medical therapy for ventricular tachycardia in sarcoidosis: nationwide outcomes and hospital readmissions.

BACKGROUND: Catheter ablation (CA) for ventricular tachycardia (VT) can be a useful treatment strategy, however, few studies have compared CA to medical therapy (MT) in the sarcoidosis population. OBJECTIVE: To assess in-hospital outcomes and unplanned readmissions following CA for VT compared to MT in patients with sarcoidosis. METHODS: Data was obtained from the Nationwide Readmissions Database between 2010 and 2019 to identify patients with sarcoidosis admitted for VT either undergoing CA or MT during elective and non-elective admission. Primary endpoints were a composite endpoint of inpa

Impact of SARS-CoV-2/COVID-19 on Provision of Medical Care to Patients With Systemic Autoimmune Rheumatic Disease and the Practice of Rheumatology.

The SARS-CoV-2 pandemic has had a significant impact on the healthcare field that resulted in changes to the way safe and effective medical care is delivered. The effects range from service disruption including ambulatory clinic closure due to both patient and provider concerns, to lack of capacity in hospital services. In rheumatology, there were other effects including viral infection-related autoantibody production, concerns about the use of systemic immunosuppression in the presence of an infectious pandemic and even concerns for viral infection-induced flares of rheumatic disease.
