Orchestrating the pre-metastatic niche: roles of stromal mediators and immune cells in metastatic progression and therapeutic targeting.

Tumor metastasis is the leading cause of mortality in patients with malignant cancers, and the establishment of the pre-metastatic niche (PMN) is recognized as a pivotal step in this process. With growing insight into the tumor microenvironment, the roles of immune cells and stromal mediators in PMN formation have gained increasing attention. This review systematically summarizes the core functions of stromal components and immune cells in PMN development and their coordinated regulatory mechanisms within the metastatic cascade, encompassing macrophages, tumor-associated fibroblasts, neutrophi

The Immune Biology of the Adrenal Gland Microenvironment and Its Role in Metastatic Progression.

Metastatic lesions are the most common malignant tumor of the adrenal gland. While surgery can have a favorable surgical outcome for isolated adrenal metastatic lesions, most adrenal metastases occur in the context of disseminated disease, and the overall prognosis remains poor. Although data are limited, metastatic lesions from diverse solid tumors to the adrenal gland have typically demonstrated poor response to immunotherapy, particularly immune checkpoint inhibitors with programmed cell death protein 1 (PD-1)/programmed death-ligand 1 (PD-L1) blockade. This apparent resistance to immunothe

[HPV-associated oropharyngeal carcinomas in the 9th edition of the TNM classification - Clinical classification and consequences].

The 9th edition of the TNM classification represents a refinement of the staging system for oropharyngeal carcinoma. Based on the distinction between HPV-associated and HPV-negative tumors introduced with the TNM-8 classification system, TNM-9 aims to capture the prognostically relevant heterogeneity within the HPV-associated subgroup more precisely. In particular, greater emphasis is placed on nodal disease characteristics, including extranodal extension, to improve prognostic discrimination. This CME article reviews the key factors of the TNM-9 classification for HPV-associated oropharyngeal

[Oncologist's perspective from sentinel node biopsy of melanoma - three decades of experience].

Sentinel lymph node biopsy was the first procedure to become the standard of care in the surgical treatment of melanoma patients, serving as a key procedure for determining the pathological stage of the disease. Thirty years ago, at the Albert Szent-Györgyi Clinical Center of the University of Szeged, we were among the pioneers in introducing sentinel lymph node biopsy for patients with melanoma in Hungary. In this publication, we summarize our experience gained during the treatment of two and a half thousand melanoma patients, based on our previously published reports on the subject. Our data

Survival Benefit of Surgical Staging Before Radiotherapy in Locally Advanced Cervical Cancer: A Pooled Analysis.

Lymph node metastasis significantly impacts prognosis and treatment in locally advanced cervical cancer (LACC). Surgical staging offers precise information on node involvement, though its survival benefit is debated. We pooled data from 9 cohort studies involving 2553 patients to evaluate the benefit of pre-treatment surgical staging in patients with locally advanced cervical cancer. Fixed effects models or random effects models were used to calculate the pooled hazard ratios (HRs). The overall pooled results showed no difference in PFS (HR 0.94, 95% CI 0.73-1.22, p = 0.65) or OS (HR 1.00, 95%

Prognostic impact of tumor deposits in colorectal cancer.

We read with great interest the article by Sun et al addressing the prognostic role of tumor deposits (TDs) and negative lymph nodes in N1c colorectal cancer. Their proposal of the NLNTD index is a valuable step toward refining risk stratification in this subgroup. In our recently published population-based cohort of 111106 patients with early-stage colon cancer, TD positivity, classified as N1c according to AJCC definitions, was independently associated with significantly worse overall and disease-specific survival, even after propensity score matching. Taken together, these findings show tha

Integrating model systems and genomic insights to decipher mechanisms of cancer metastasis.

Deciphering metastatic processes is crucial for understanding cancer progression and potential treatment options. Genetic studies of model systems engineered to mimic metastatic disease, including organoids, genetically engineered mice and human cell lines, have had an important role in shaping our understanding of the metastatic cascade and how it can be manipulated. More recently, advances in high-throughput sequencing have enabled human metastases to be studied at single-cell and single-nucleotide resolution, providing insights into metastatic evolution and phenotypes of both cancer cells a

Protocol for the establishment of murine orthotopic lung tumors and the assessment of contralateral pulmonal metastasis by flow cytometry.

Murine orthotopic tumor models can accurately resemble cancer biology characteristics including metastasis, drug sensitivity, and remodeling of the tumor microenvironment. Here, we present a protocol for establishing murine orthotopic lung tumors and assessing contralateral pulmonary metastasis by flow cytometry. We describe steps for marking tumor cells with label retention dyes, preparing cell mixtures in Matrigel, and performing intrapulmonary injection. We then detail procedures for ex vivo processing of lungs followed by fluorescence-activated cell sorting (FACS) data analysis and quantif

[Evidence for the extent and oncological benefit of lymphadenectomy for esophageal cancer].

The prognosis for esophageal cancer is determined in particular by the depth of infiltration (T stage) and lymph node metastasis (N status). In patients with locally advanced tumors, surgical resection is the current standard. The extent of the lymphadenectomy depends on the localization of the tumor, analogous to the choice of surgical technique. For adequate tumor staging and achievement of pN0 status, seven lymph nodes without tumor metastases are necessary by definition but the current guidelines recommend 20 lymph nodes as a benchmark in an expert consensus. Despite the importance of the

Calpain-1 and Calpain-2 Promote Breast Cancer Metastasis.

In breast cancer, progression from localized stage I to distant metastatic stage IV disease is associated with a reduction of 5-year survival from nearly 100% to 23.2%. Expression of the calcium-activated protease isoforms calpain-1 and calpain-2 has been correlated with cell migration and invasion in vitro, metastatic potential in preclinical mouse models of cancer, and breast cancer prognosis in patients. It is unclear which of these two calpain isoforms is responsible for the apparent metastatic potential of cancer cells. Here, we demonstrate that while individual CRISPR-Cas9 knockouts of e

To ablate or not to ablate? Outcomes of local ablative treatments (LAT) for oncogene addicted (OA) oligo-metastatic (OM) non-small cell lung cancer (NSCLC): A systematic review.

Oncogene-addicted non-small cell lung cancer (NSCLC) encompasses distinct molecular phenotypes, each associated with specific clinical behavior and variable sensitivity to targeted therapies. Therapeutic advancements in the field enhanced clinical outcomes and overall prognostic improvement. Beyond the molecular profile, disease burden serves as a predictive marker for treatment response and overall prognostic outcomes. The current disease staging categorizes patients based on the number and sites of metastatic involvement. In this context, the addition of local ablative therapies (LAT) is can

FNAC Diagnosis of Rare Port-Site Metastasis in Gallbladder Cancer Post-Cholecystectomy.

BACKGROUND: Port-site metastasis (PSM) following laparoscopic cholecystectomy (LC) for unsuspected gallbladder carcinoma (GBC) is a rare occurrence. Fine Needle Aspiration Cytology is a simple and time-saving technique, thus it has proven useful in diagnosing such metastases. CASE REPORT: We present a case of a 45-year-old female who developed painful, multinodular epigastric swellings 2 years post-LC, initially performed for presumed benign cholelithiasis. FNAC of the port-site swelling revealed hypercellular smears with malignant epithelial cells, leading to a diagnosis of poorly differenti

The new perspective of gasotransmitters in cancer metastasis.

Cancer metastasis is the leading cause of death in cancer patients, which renders heavy burdens to family and society. Cancer metastasis is a complicated process in which a large variety of biological molecules, cells and signaling pathways are involved. Nitric oxide (NO), carbon monoxide (CO) and hydrogen sulfide (H2S) are common air pollutants which are harmful to human bodies and environments. However, recent studies show that these gases, which are collectively termed gasotransmitters, play significant roles in physiological homeostasis and pathogenesis including immunological responses, n

Neutrophil extracellular traps-STC1 positive feedback loop promotes immune evasion and metastasis in bladder cancer.

BACKGROUND: Immune checkpoint inhibitors (ICIs) have transformed the treatment strategy for bladder cancer (BLCA), but primary resistance still occurs in most patients. Recent evidence suggests that neutrophil extracellular traps (NETs) play a key role in cancer therapy resistance, but their specific role in BLCA remains unclear. METHODS: An integrated analysis of multiple clinical cohorts receiving ICIs was performed to explore the link between NETs and patient outcomes. Immunofluorescence staining, in vitro co-culture system, scanning electron microscopy, and mouse lung metastasis model wer

Protocol for iterative in vivo selection of highly metastatic ovarian cancer cell populations and metastasis modeling in mice.

The molecular basis underlying ovarian cancers' multi-route metastasis remains poorly defined. This protocol details iterative in vivo selection of highly metastatic cancer cell populations and metastasis modeling in ovarian cancer. We outline steps for inoculation of ES-2-GFP-Luc, monitoring tumor growth and visceral spread, isolation and culture of tumor-derived derivatives, reinoculation of ES-2-MC1-Hep to establish ES-2-MC2-Hep, and assessment of metastatic colonization efficiency. This protocol enables mechanistic studies of distant metastasis in gynecological cancers. For complete detail

A new proposal of simplified classification of non-small cell lung cancer oligometastases for easy applicability through systematic literature analysis and meta-analysis validation.

INTRODUCTION: Oligometastasis (OM) exhibits wide range of prognosis, which necessitates appropriate classification for optimal therapeutic decision-making. Complementing the ESTRO-EORTC classification which lacked prognostic differentiation and was rather complex, we propose a new and simpler classification based on systematic literature analysis and meta-analysis validation. METHOD: The databases were searched up to April 2024. Inclusion criteria were (1) ≥ 10 Non-small cell lung cancer OM patients, (2) local ablative treatment (LAT) versus control (systemic/supportive treatment), (3) report

Emerging therapeutic strategies for metastatic uveal melanoma: Targeting driver mutations.

Uveal melanoma (UM) is the most common primary malignant intraocular tumor in adults. Although primary UM can be effectively controlled, a significant proportion of cases (40% or more) eventually develop distant metastases, commonly in the liver. Metastatic UM remains a lethal disease with limited treatment options. The initiation of UM is typically attributed to activating mutations in GNAQ or GNA11. The elucidation of the downstream pathways such as PKC/MAPK, PI3K/AKT/mTOR, and Hippo-YAP have provided potential therapeutic targets. Concurrent mutations in BRCA1 associated protein 1 (BAP1) or

Lymphatic drainage of sinonasal malignancies and the role of sentinel node biopsies.

BACKGROUND: Locoregional recurrence is a critical factor in the prognosis of sinonasal malignancies. Due to the rarity of these tumours, as well as the heterogeneity of histologies and anatomical subsites, there is little evidence regarding the rate and location of regional metastases in sinonasal malignancies. Elective regional lymph node dissection in the therapy of sinonasal malignancies has become controversial. On the one hand, elective regional lymph node dissection is considered to be an overtreatment in the cN0 cases. On the other hand, undetected occult lymphatic metastases are associ
