Factors influencing women's preferences and decision making regarding the extent of axillary surgery in breast cancer: A systematic review.

Axillary surgery in early-stage breast cancer is increasingly de-escalated. However, variation in practice persists, and the patient level factors influencing treatment decisions remain unclear. This systematic review synthesised evidence on the psychological, clinical, and contextual factors shaping women's preferences and decision making regarding the extent of axillary surgery they receive. A PRISMA-guided search of Ovid MEDLINE, Embase and Cochrane databases was conducted in December 2025. Ten studies involving 861 women were included, comprising observational, qualitative, and mixed metho

Surgical Management of PSMA PET-Positive Lymph Nodes in Prostate Cancer.

PURPOSE OF REVIEW: PSMA PET has reshaped prostate cancer staging by detecting low-volume nodal metastases occult on conventional imaging. This review summarizes current evidence on miN1M0 disease and discusses features that may guide surgical decision-making. RECENT FINDINGS: Evidence remains heterogeneous, and the role of surgery in miN1M0 disease is undefined. Contemporary series suggest that surgery may be feasible within multimodal treatment in selected patients, supporting that miN1M0 disease should not automatically preclude surgery. Surgical candidacy should consider nodal burden and l

Sublobar resection, stereotactic body radiotherapy, and thermal ablation for early-stage non-small cell lung cancer: a systematic review and meta-analysis.

INTRODUCTION: For patients with early-stage non-small cell lung cancer (NSCLC) who are unable to tolerate lobectomy, alternative locoregional treatments, including sublobar resection (SLR), stereotactic body radiation therapy (SBRT), or image-guided thermal ablation (IGTA) are available, yet their relative risks and benefits remain uncertain. This systematic review and meta-analysis aims to synthesize existing evidence and compare the survival outcomes of these modalities in early-stage NSCLC. METHODS: A systematic search was conducted through MEDLINE (PubMed), Embase, and CENTRAL on April 20

Elective neck dissection in clinically early stage (cT1-T2N0) supraglottic squamous cell carcinomas: a systematic review.

BACKGROUND: Early-stage supraglottic squamous cell carcinoma carries a substantial risk of occult lymph node metastasis, yet the benefit and extent of elective neck dissection (END) in clinically node-negative (cN0) patients remain debated. METHODS: This systematic review analyzed studies published between 2000 and 2025 evaluating END versus observation in adults with early-stage (cT1-T2N0) supraglottic tumors, following predefined criteria and PRISMA methodology. RESULTS: Sixteen studies met inclusion criteria. Occult nodal metastasis was identified in 13-37.5% of patients, with level II-es

Management of De Novo miN1 Prostate Cancer in the PSMA-PET Era.

BACKGROUND: Prostate-specific membrane antigen positron emission tomography (PSMA-PET) is reshaping nodal staging in prostate cancer by detecting regional lymph node metastases with greater accuracy than conventional imaging. This has created a new cohort of patients with N1M0 disease detectable only on advanced imaging, raising important questions regarding prognosis, treatment intensification and the applicability of historical management paradigms derived from conventional imaging. The aim of this review is to consider de novo N1M0 management on PSMA-PET and current evidence. METHODS: A co

Should sentinel lymph node biopsy be performed in patients with early-stage breast cancer?

The necessity of sentinel lymph node biopsy (SLNB) in early-stage, node-negative (cN0) breast cancer is increasingly questioned in the era of surgical de-escalation and biologically driven treatment strategies. While SLNB has long replaced axillary lymph node dissection to reduce morbidity, it nevertheless carries a measurable risk of complications, including lymphedema, pain, and reduced arm mobility. Recent high-quality evidence suggests that SLNB may be safely omitted in selected patients without compromising oncologic outcomes. The SOUND randomized trial demonstrated that omission of SLNB

Optimising personalised melanoma care: A national e-Delphi consensus survey exploring expert perspectives on the use of a novel prognostic biomarker for early-stage cutaneous malignant melanoma.

BACKGROUND: Cutaneous malignant melanoma (cMM) incidence continues to rise. AJCC staging provides broad prognostic estimates, potentially leading to overtreatment in early-stage disease. AMBLor™ is a validated IHC-based biomarker that identifies non-ulcerated AJCC stage I/II cMM at lower risk of progression. OBJECTIVES: To obtain consensus on clinical and service challenges in early-stage cMM care and how AMBLor™ could be integrated into decision-making at critical points in the care pathway. METHODS: A four-round modified e-Delphi survey was conducted. Twenty-five UK skin specialist MDT con

Prognostic Value of Estrogen Receptors Versus Progesterone Receptors in Early Breast Cancer: An Analysis of Clinical and Translational Data (2004-2026).

The accurate determination of hormone-receptor status is foundational to risk stratification and therapeutic decision-making in breast oncology. While the estrogen receptor (ER) is universally recognized as the primary predictive biomarker for a tumor's response to endocrine therapy, the independent prognostic value of the progesterone receptor (PR) remains a subject of ongoing clinical debate. This study evaluates the relative prognostic weight of PR expression compared to ER expression in early-stage breast cancer, clarifies the biological mechanisms driving PR loss, and assesses the diagnos

[Melanoma - an overview of the disease and its treatment].

Melanoma arises from malignant transformation of melanocytes and accounts for approximately 3500 new cases per year in Switzerland. The most important risk factors include intermittent UV exposure, fair skin, a high number of nevi, five or more atypical nevi, and a positive family history. The most common type is the superficial spreading melanoma, followed by the nodular type. Diagnosis is primarily clinical, for example using the ABCDE rule, and is supported by dermoscopy performed by dermatologists. If there is a suspicion, a narrow complete excision should be performed to confirm the diagn

Multidisciplinary treatment for advanced gastric cancer.

Outcomes for advanced gastric cancer are improving with multidisciplinary treatment combining surgical resection and perioperative chemotherapy. In Asia, the standard of care for pathological Stage (pStage) II gastric cancer involves tegafur-gimeracil-oteracil potassium for 1 year postoperatively after gastrectomy with D2 lymph node dissection or capecitabine plus oxaliplatin for 6 months postoperatively. For pStage III gastric cancer, a combination of tegafur, gimeracil, and oteracil potassium combined with docetaxel remains the standard postoperative treatment. However, perioperative chemoth

Multimodal De-Escalation Strategies in Early Breast Cancer.

The excellent cure rates documented in clinical trials today constitute a very high bar for attempts to de-escalate therapy for early-stage breast cancer (EBC). Moreover, any therapy de-escalation must be made in the context of an optimal multimodal treatment concept as de-escalation of one modality should not be met by escalation of another. In surgery, omission of sentinel lymph node biopsy can now be safely offered for patients with low-risk hormone receptor-positive/human epidermal growth factor receptor 2-negative (HER2-) disease. After neoadjuvant therapy, adapting surgical and/or radiat

[A Case of cy1 Advanced Gastric Cancer That Achieved CR after Chemotherapy Via an Enterostomy].

A 67-year-old male presented with severe epigastric pain in May 2023 and was diagnosed with Stage Ⅲ advanced gastric cancer(cStage Ⅲ)in June. Due to indistinct boundaries between lymph node #8a and the pancreas, non-resectional chemotherapy and enterostomy were performed during a diagnostic laparotomy. The diagnosis was updated to sStage Ⅳ with cy1. Beginning in July, the patient was administered SOX and nivolumab. Following 6 courses, CT scans in December revealed clearer boundaries, allowing for conversion surgery in January 2024. This surgery resulted in a pathological complete response(pCR

Efficacy and Safety of Four Weeks of Moderately Hypofractionated Chemoradiation for Unresectable, Stage 3 Non-Small Cell Lung Cancer: A Systematic Review.

Shortening treatment time with moderately hypofractionated radiotherapy benefits patients by reducing inconvenience and costs, but its use in the definitive treatment of unresectable Stage 3 non-small cell lung cancer is controversial due to lack of level one evidence and toxicity concerns. Pivotal systemic therapy trials utilize conventionally fractionated chemoradiation at 2 Gy per fraction given over 6 weeks. In practice, 4 weeks of chemoradiation at 2.75 Gy per fraction is sometimes employed to reduce the treatment burden for selected patients, especially those who are older or have comorb

Maintenance of resection margins in gastrectomy for cancer: simple in procedure, influential in survival-current status and literature review on optimal lengths and positive margins.

PURPOSE: The prognostic significance of the proximal margin length (PML) and positive resection margins (PRMs) in gastric cancer (GC) remains controversial. International guidelines for PML differ widely, reflecting a lack of consensus on optimal surgical margins. While positive RMs are associated with poor survival, their impact varies according to the tumor stage and nodal involvement. METHODS: A comprehensive review of the relevant literature was conducted to evaluate guideline recommendations on PML, factors influencing pathological negativity, the prognostic impact of PMLs and PRMs, and
