Clinical and therapeutic applications of neuromuscular stimulation: a review of current use and speculation into future developments.

In this paper we present an overview of current research into clinical and therapeutic applications of electrical neuromuscular stimulation (NMS). As this is now such a huge subject we have focused our attention on the therapeutic rather than orthotic uses of stimulation and limited the field almost exclusively to upper limb applications in hemiplegia. The evidence that NMS influences motor re-learning and how this may be measured is discussed. We have identified the following as the three most important unresolved issues: 1) an understanding of how NMS modifies the interactions within the ner

Compactotomy in Huntington's chorea.

Advances in neuroradiological and neurosurgical techniques have lead to a growing interest in functional neurosurgical interventions for medically intractable movement disorders. The majority of these procedures are performed in patients with hypokinetic movement disorders, especially Parkinson's disease. However, relatively few interventions were done in hyperkinetic disorders such as Huntington's disease (HD), mainly owing to the lack of an adequate target nucleus. We have recently described the case of a reversible chorea in a genetically confirmed HD patient. We subsequently identified a m

Incorporating breastfeeding care into daily newborn rounds and pediatric office practice.

Integrating breastfeeding care into pediatric practice and thought process results in improved breastfeeding outcomes for patients and significant professional satisfaction for pediatricians. Meeting a family's expectation to help them succeed in their desire to breastfeed their infants is rewarding. Current knowledge regarding the appropriate care of the breastfed infant immediately after birth provides an engaging opportunity to reframe ideas regarding good newborn care. Whatever may have previously been thought regarding newborn care, the true objectives are to maintain optimal physiologic

Prognostic value of motor evoked potentials elicited by multipulse magnetic stimulation in a surgically induced transitory lesion of the supplementary motor area: a case report.

Surgery involving the supplementary motor area (SMA) places the patient at risk of transient motor deficit. To predict outcome in patients with early postoperative hypokinesis would be relevant to both the patient and the surgical team. A 15 year old girl with a large left thalamic tumour removed through a left transcallosal approach is described. Despite intraoperatively preserved muscle motor evoked potentials (mMEPs) from all limbs, elicited by multipulse electrical stimulation, she awoke with a right hemiplegia and mutism. On the first postoperative day, neurophysiological evaluation using

Repeated stimulation of L-type calcium channels in the rat ventral tegmental area mimics the initiation of behavioral sensitization to cocaine.

RATIONALE: A substantial body of evidence indicates that ion flux through L-type calcium channels and N-methyl-D-aspartate (NMDA) receptors contributes to behavioral sensitization to cocaine. OBJECTIVES: The following experiments were designed to evaluate the role of calcium influx through L-type calcium channels or NMDA receptors in the ventral tegmental area (VTA) in the initiation of behavioral sensitization to cocaine. METHODS: The L-type calcium channel agonist BayK 8644, the glutamate agonist NMDA, or vehicle was microinjected into the VTA on 3 consecutive days. Following a 2-week with

St Louis encephalitis: a review of 11 cases in a 1995 Dallas, Tex, epidemic.

OBJECTIVE: To update some of the clinical features of St Louis encephalitis (SLE), a common arboviral infection that occurs in epidemic patterns in the south-central and midwestern United States. METHODS: Eleven patients with SLE from a 1995 epidemic in Dallas, Tex, were studied clinically, radiologically, neurophysiologically, and neuropathologically (in 1 case). RESULTS: The electroencephalograms and magnetic resonance imaging (MRI) scans of our patients revealed features that have received little attention in previous studies. Of the 9 patients who were examined with electroencephalograph

Eye movements affect the perceived speed of visual motion.

Eye movements add a constant displacement to the visual scene, altering the retinal-image velocity. Therefore, in order to recover the real world motion, eye-movement effects must be compensated. If full compensation occurs, the perceived speed of a moving object should be the same regardless of whether the eye is stationary or moving. Using a pursue-fixate procedure in a perceptual matching paradigm, we found that eye movements systematically bias the perceived speed of the distal stimulus, indicating a lack of compensation. Speed judgments depended on the interaction between the distal stimu

Balance disturbances in individuals with vibroacoustic disease.

INTRODUCTION: In previous studies, patients with vibroacoustic disease (VAD) presented hyperintense foci in T2 of the cerebral white matter, brainstem and basal nuclei. The most probable etiology is ischemia. One of the most frequent complaints of these patients is balance disturbances which, in two cases, has threatened the patients' ability to maintain their jobs. The purpose of this study was to compare two methods, one neurophysiological (auditory evoked potentials - AEP) and the other structural imaging (brain MRI), in order to determine to what extent the changes detected with these meth

Excitability of the central masticatory pathways in patients with painful temporomandibular disorders.

Much is unclear about the pathophysiological mechanisms underlying painful temporomandibular disorders. In addition to various other theories, masticatory muscle dysfunction and pain have also been attributed to primary central nervous system hyperactivity. We assessed this possibility in a study using recent neurophysiological techniques. From among outpatients whose diagnosis of temporomandibular disorders had been obtained in stomatognathic facilities, we studied 10 patients with bilateral pain and 15 patients with unilateral pain, in whom electromyographic examination of the trigeminal ref

Clinical and prognostic features in unilateral femoral neuropathies.

We have examined the clinical features of patients with femoral neuropathy and the factors that influence the prognosis. Of 80 consecutive patients referred for neurophysiological evaluations of proximal lower limb weakness, 32 fulfilled strict inclusion criteria and had adequate information, including estimates of axon loss (AxL) by stimulation of the bilateral femoral nerve. In 31, the Kaplan-Meier method was used to describe the time course of the outcome, while logistic regression was employed to determine the contributing factors. Excellent, satisfactory, and poor outcomes were seen in 10

Development and reorganization of descending motor pathways in children with hemiplegic cerebral palsy.

Reorganization of descending motor pathways was explored in 33 subjects with hemiplegic cerebral palsy. Subjects were assessed neurologically and surface electromyographic recordings were taken from homologous muscles of both hands. Functional corticospinal projections were assessed using focal magnetic stimulation of the motor cortex. In control subjects this evokes EMG responses in the contralateral hand at short latency. Similar results were seen in 12 of the hemiplegic subjects following stimulation of the undamaged motor cortex. In the remaining 22 subjects novel corticospinal pathways we

[Clinical study of four cases of Rett Syndrome].

Rett Syndrome was first described in 1966 by Andreas Rett. The disorder is characterized by a progressive loss of cognitive and motor skills as well as development o stereotyped hand movements, occurring after an apparently normal development. Authors present three typical cases, and, another one atypical, being all of them female. This study takes into account ten different areas about chronology, age and reasons in the first consultation, some milestones of psychomotor development, the diagnostic criteria--according to Rett Syndrome Diagnostic Criteria Work Group (R.S.D.C.W.G.), they consist

Serotoninergic agents in the treatment of Gilles de la Tourette's syndrome.

A preliminary trial with fluoxetine, a 5-HT reuptake inhibitor, was carried out on two young male patients (21 and 32 years old) affected by Gilles de la Tourette syndrome. They both underwent a complete neurological evaluation also including neuroradiological, neurophysiological and neuropsychological assessment. Both patients had already been treated with benzodiazepines and amitriptyline; the older one was also given haloperidol and chlorimipramine with definite, but short-lasting improvement. During hospitalization a therapeutic trial with fluoxetine (20 mg/day in the younger patient and 4

Motor learning deficits in aged rats are correlated with loss of cerebellar noradrenergic function.

We have demonstrated that aged rats show impairments in learning patterned motor movements. Similar behavioral impairment is observed in rats with noradrenergic lesions. Norepinephrine is known to act as a neuromodulator in the cerebellar cortex because it can augment the action of GABA and other neurotransmitters. This effect of NE to augment the signal to noise ratio of GABAergic inputs to cerebellar Purkinje neurons is a possible substrate for NE's effect on motor learning. Aged rats demonstrate deficits in the modulatory actions of NE to augment GABAergic inhibitions when both substances a

[Dysthymic disorder and its treatment].

Dysthymia is a new nosographic category in the latest American and international classification of mental disorders. Dysthymics were previously considered either as "neurotic depressive characters" or "characterologic depressives." Today it has been demonstrated that the personality and affective traits of these patients are in fact simply different expressions of a constitutive predisposition to clinical depression. Some studies have shown biological and neurophysiological relationships between dysthymia and major mood disorders. We therefore submit that dysthymia constitutes a subaffective e

Neurotoxicity in workers exposed to styrene.

Quantitative electroencephalography (EEG) for 99 workers exposed to styrene in the reinforced-plastics industry was analysed and compared with exposure parameters. The work places and factories from which the subjects were selected had been evaluated previously, and exposure levels were known relatively accurately. That information and data on individual exposures were combined to create an exposure index, which reliably reflected long-term exposure to styrene in various occupational settings. All of the subjects underwent careful medical and neurophysiological examinations. Quantitative EEG w

Chronic treatment with a benzodiazepine agonist in vivo increases the actions of the benzodiazepine partial inverse agonist, FG7142, on the hippocampal slice in vitro.

We have shown previously that chronic treatment of mice with a benzodiazepine agonist, flurazepam, increased the pharmacological actions of the partial inverse agonist, FG7142. We have investigated the neurophysiological basis for this using extracellular recordings of evoked field potentials in area CA1 of isolated hippocampal slices. The slices were prepared 48 h after the end of the chronic in vivo treatment, a time when no evidence of residual benzodiazepine agonist activity was found in the CNS. During perfusion with standard Ringer solution, no significant differences were seen between t

Magnetic transcranial brain stimulation: painless evaluation of central motor pathways. Normal values and clinical application in spinal cord diagnostics: upper extremities.

Painless transcranial magnetic stimulation has been performed on 53 healthy adults in order to obtain normal values of central motor pathway conduction times to cervical segments innervating upper limb muscles. Central motor latency was 5.1 msec when recording from the biceps brachii and 5.2 msec when recording from the abductor pollicis brevis or abductor digiti minimi muscles. The plexus motor latency as the conduction time between the cervical motor root and the proximal nerve trunks in the axillary region was 2.2 msec and 2.6 msec when recorded from the abductor pollicis brevis and abducto

Gating of auditory evoked responses in normal college students.

Several neurophysiological indicators of abnormal sensory processing found in psychosis are also found in lower incidence in normal populations, particularly those at the age of risk for illness. One such parameter, gating of the auditory evoked potential, was evaluated in college students. Auditory stimuli were presented in pairs to 21 normal subjects, who generally showed gating or suppression of the vertex P50 wave to the second stimulus. Since suppression in this conditioning-testing paradigm is often absent in psychotic patients, we attempted to see if particular parameters, perhaps relat
