Juvenile idiopathic arthritis: some progress but still many uncertainties.

(1) Juvenile idiopathic arthritis is a chronic inflammatory rheumatic disorder of unknown cause. Spontaneous remission is frequent. Between 10% and 20% of patients have ocular involvement. (2) Nonsteroidal antiinflammatory drugs and physiotherapy are the cornerstones of patient management. Intra-articular steroid injection generally provides several months of relief. The place of immunosuppressants is difficult to ascertain, given their uncertain benefits and established long-term risks in children. There is no curative treatment at present.

[The physicochemical characterization of the strains isolated during the outbreak of epidemic neuropathy in Cuba. II].

The presence of 2 agents such as a Cox A9 strain and another mild cytopathogenic effect-producing strain, both isolated from patients presenting with epidemic neuropathy is reported in this paper. A mild cutopathogenic effect which was propagated in successive dilutions was developed in the dilution 10(-4) by means of the neutralization test of a Coxsackie A9 virus with its homologous hyperimmune serum. A gradient in saccharose was performed in a mild cytopathogenic effect-producing strain and a typical cytopathogenic effect of an Enterovirus developed from one of the fractions passed in tissu

Posterior segment manifestations of inflammatory bowel disease.

Thirteen patients with inflammatory bowel disease and posterior segment disease were subject to a retrospective review. Eight patients had Crohn's disease and five had ulcerative colitis. In six patients, the inflammatory bowel disease was active when ocular inflammation occurred. Patients had one or more posterior segment findings that included serous retinal detachment (8), choroidal infiltrates (6), retrobulbar neuritis (1), papillitis (1), retinal pigment epithelium disturbance (1), and choroidal folds (1). Posterior segment disease responded to systemic and periocular corticosteroids in 9

Uremic optic neuropathy.

A rare case of documented reversible bilateral optic neuropathy as the presenting manifestation of chronic renal failure is discussed. Repeated hemodialysis in the early stages of the disorder was useful for recovery of vision.

Research approaches on the diagnosis of multiple sclerosis by CSF examination.

The diagnostic value of CSF oligoclonal IgG bands, intrathecal IgG synthesis, cytology and markers of myelin breakdown in the cerebro-spinal fluid were evaluated in multiple sclerosis. None of the various immunological alterations were unique to MS. The diagnosis depends therefore mainly on course and clinical symptoms, but can be supported by CSF analysis.

Visual changes mediated by beer in retrobulbar neuritis--an investigative case report.

A patient with established retrobulbar neuritis and Uhthoff's phenomenon who claimed that his visual acuity improved after drinking beer was assessed by detailed quantitative psychophysical tests. Studies of electrophysiological responses and biochemical levels of blood serum before and after drinking on several occasions were also made. The results of tests of contrast sensitivity, spectral sensitivity, flicker sensitivity, and visually evoked potential confirmed his observation. Some mechanisms for this phenomenon are considered.

Pattern electroretinograms and visual-evoked potentials in glaucoma and multiple sclerosis.

Steady-state visual-evoked potentials and electroretinograms were simultaneously recorded in four patients with glaucoma and in five patients with multiple sclerosis. The stimuli included a homogenous field and a 2.3 cycles per degree sinusoidal grating that were counter-phase modulated at the rate of 7.5 Hz. We used narrow bandwidth spectral analysis to measure the response amplitudes and signal-to-noise ratios. Transient pattern visual-evoked potentials (1 Hz) were also measured for latency in each eye. We found abnormal pattern electroretinograms, based on the absence of a significant secon

Rod- and cone-mediated visual function in multiple sclerosis.

Visual thresholds and perceptual latencies were determined in patients with multiple sclerosis (MS) and in normal control subjects. Measurements were made under light- and dark-adapted conditions, with stimuli chosen to stimulate rod and cone receptors selectively. More abnormalities in perceptual latency and luminance threshold were recorded in the light-adapted condition than in the dark-adapted condition, but this result was not specific to the rod or cone systems. Possible underlying pathophysiological processes are discussed, and it is suggested that reduced conduction velocity in the dem

Visual acuity and contrast sensitivity in multiple sclerosis--hidden visual loss: an auxiliary diagnostic test.

In 48 patients with multiple sclerosis sine-wave gratings were used to test visual sensitivity for coarse, medium, and fine detail rather than measuring visual acuity for fine detail only, as in conventional clinical tests. In 20/48 patients the test revealed a visual defect of neural origin, qualitatively different from that caused by refractive error. In 11 of these 20 patients, visual sensitivity to detail of medium coarseness was markedly degraded, even though sensitivity to both coarse and fine detail was unimpaired. In 3 of these 20 patients visual sensitivity to coarse detail was select

Role of Tocilizumab in Severe CNS Inflammatory Presentations in Children.

OBJECTIVES: Anti-interleukin 6 receptor (anti-IL6R) therapies have been proven to reduce relapse rates in patients with neuromyelitis optica … -TCZ administration. Outcomes included rapid intracranial pressure reduction, recovery in relapsing optic neuritis and myelitis, and improvement in RANBP2-related acute necrotizing encephalopathy. No acute adverse effects were observed. One patient died despite immunotherapy. CSF cytokine panels showed elevated IL-6 in 5 patients, with normal serum IL-6 concentr …

Molecular Mechanisms of Retinal Damage in NMOSD via Müller Glial Cell Stimulation with Patient Sera.

Neuromyelitis optica spectrum disorder (NMOSD) is a rare autoimmune CNS disease that frequently causes severe optic neuritis, yet the molecular mechanisms driving retinal damage remain incompletely understood, especially across different NMOSD subgroups. Müller glial cells, which maintain retinal water-ion homeostasis through AQP4 and Kir4.1 channels, may represent a primary retinal target of circulating NMOSD-related autoantibodies and serum factors. We investigated how sera from AQP4-IgG+, MOG-IgG+, and double-seronegative (DSN) NMOSD patients affect the expression and localization of key Mü…

[Rare very late-onset neuromyelitis optica].

BACKGROUND: Neuromyelitis optica (NMO) and its related forms, collectively referred to as neuromyelitis optica spectrum disorders (NMOSD), are rare but often severe autoimmune diseases of the central nervous system (CNS). NMOSD predominantly affects women, most commonly between the ages of 30 and 40 years. Aquaporin-4 antibodies (AQP4-IgG) are detectable in approximately 80% of patients with NMOSD.The cardinal clinical manifestations include transverse myelitis, optic neuritis (ON), as well as brainstem and diencephalic involvement. Most NMOSD cases follow a relapsing-remitting course without p…

Highlighting Ophthalmic Adverse Effects Associated with Isotretinoin.

Isotretinoin is a highly effective treatment for moderate-to-severe acne vulgaris but is associated with a broad spectrum of ophthalmic adverse effects. While dry eye disease secondary to meibomian gland dysfunction is the most common ocular complication, less familiar manifestations include nyctalopia, corneal abnormalities, retinal changes, idiopathic intracranial hypertension, and optic neuritis. Current evidence suggests that isotretinoin induces structural and functional changes in the meibomian glands, resulting in tear film instability and evaporative dry eye disease. Although many of t…
