Orbital metastasis of endocervical stromal sarcoma: a rare tumor and an uncommon metastasis.

Endocervical stromal sarcoma (ECSS) is a very rare uterine sarcoma. The most common presentation is pelvic mass and vaginal bleeding. The ma … ne week after thoracotomy, she was admitted to the hospital with loss of vision in the left eye. An orbital mass was detected with magnetic resonance imaging. Endometrial and cervical pathology preparations were reassessed and were compatible with ECSS. We performed mammography, thorax, and abdomen and cranial imaging to rule out other malignancies that …

Endoscopic drainage of orbital subperiosteal hematoma secondary to acute rhinosinusitis in a child.

Acute rhinosinusitis is frequently associated with secondary orbital infection, most commonly subperiosteal abscess. Although orbital subperiosteal abscess is a deadly disease that might lead to blind and cavernous sinus thrombosis, recent review of literature showed that immediate surgical intervention might not always be necessary for subperiosteal abscess. Orbital inflammation secondary to sinusitis is common in children, whereas orbital subperiosteal hematoma secondary to sinusitis is extremely rare, with only 11 reported cases, including one case in children. All the cases were treated wi…

Orbital and adnexal tuberculosis: a case series from a South Indian population.

BACKGROUND: Orbital tuberculosis (OTb) is rare and may be regarded as a manifestation of extrapulmonary tuberculosis. We report an interesting case series of six patients with varied presentations of orbital and adnexal tuberculosis in a South Indian patient population. RESULTS: A retrospective, interventional case series of six patients diagnosed with orbital and adnexal tuberculosis on the basis of clinical, radiological and histopathological evaluations between 2010 and 2013 was performed. Among the six patients with histopathologically proven OTb, five were women. The varied presentations…

Orbital solitary fibrous tumor: A rare clinicopathologic correlation and review of literature.

Orbital solitary fibrous tumor (SFT) is a rare tumor and orbits are a very uncommon site. It is mostly noted to arise from mesenchymal structures like pleura and peritoneum. The diagnosis of orbital SFT cannot be made with certainty on clinical or radiological evaluation alone and requires immunohistochemical studies for confirmation. Orbital SFT's usually show an indolent clinical course and a complete cure can usually be achieved with complete resection. We describe clinical presentations, radiological and operative findings, and pathological features of a patient with orbital SFT along with…

Orbital exenteration under trigeminal block: An innovative method of regional anesthesia.

Orbital exenteration is a disfiguring operation involving the removal of the entire contents of the orbit, with or without the eyelids. It is widely felt that such extensive surgery can only be performed under general anesthesia. We report our experience with a patient who underwent orbitalexenteration under trigeminal block with intravenous sedation. A 68-year-old male patient was diagnosed to have orbital cellulitis (mucormycosis), uncontrolled diabetes mellitus, ischemic heart disease, dilated cardiomyopathy with severe left ventricular systolic dysfunction with severe pulmonary artery hype…

Successful treatment of rhino-orbital mucormycosis with posaconazole and hyperbaric oxygen therapy.

Mucormycosis is a rare, but invasive infection caused by ubiquitous molds. Amphotericin B and surgery have been known to help improve the outcome. Sporadic case reports support the use of posaconazole in adults. We report a toddler with acute lymphoblastic leukemia who acquired rhino-orbital mucormycosis caused by Rhizopus species at the end of induction chemotherapy. She was successfully treated with multiple surgical debridements, amphotericin B, posaconazole and hyperbaric oxygen therapy. In conclusion, mucormycosis is a serious infection that requires aggressive surgical and medical therap…

Emergency decompression of tension retrobulbar emphysema secondary to orbital floor fracture.

Orbital floor fractures are generally the result of blowout orbital and may be associated with orbital emphysema leading to proptosis and even to loss of vision. A 49-year-old woman fractured the orbital floor in a fall. After blowing her nose, she developed exophthalmos and severe reduction in vision. She consulted our department and underwent emergency surgical management with orbital drainage. Decompression led to immediate resolution of the exophthalmos and postoperative improvement in visual acuity. Urgent decompression is indicated by the presence of proptosis, elevated intraocular press…

[Give attention to standardized management of orbital development in Chinese with microphthalmos or anophthalmos].

Congenital and acquired microphthalmos or anophthalmos are common ocular disorders that cause facial disfigurement in children. It is important to have timely and reasonable treatment to promote orbital growth. At present status, many patients miss the optimum opportunity for orbital reconstruction because of non-standardized management in China. The correct management for promoting orbital growth in microphthalmos or anophthalmos is thus elaborated. Conformers with progressively increasing size can be used in children at 1-3 years of age; while orbital implants could be used after 3-5 years o…

Management of pure medial orbital wall fracture with autogenous bone graft.

The orbit is an irregular conical cavity formed from 7 bones including the frontal, sphenoid, zygomatic, maxillary, ethmoid, lacrimal, and palatine bones. Fractures of the internal orbit can cause a number of problems, including diplopia, ocular muscle entrapment, and enophthalmos. Although muscle entrapment is relatively rare, diplopia and enophthalmos are relatively common sequelae of internal orbital fractures. Medial orbital wall fracture is relatively uncommon and represents a challenge for its anatomical reconstruction. In this context, autogenous bone graft has been the criterion standa…

Extraction of fronto-orbital metallic radio antenna through Stallard orbitotomy.

A 39-year-old man presented after falling on a radio antenna while in his yard. Computed tomography revealed that the antenna entered the right superolateral orbit, penetrating through the right orbital roof and extending into the right frontal lobe. A Stallard orbitotomy was performed to remove the antenna resulting in blepharoptosis.

Hyperplastic intradiploic meningothelial tissue in the orbital roof mimicking metastatic disease.

Osteolytic lesions are highly suggestive of metastatic disease or in rare instances may result from primary extradural meningiomas. Herein, we describe a rare case of a patient who presented with a large area of osteolysis on the right orbital roof. An excisional biopsy revealed that the lesion was associated with the presence of hyperplastic intra-osseous meningothelial tissue. To our best knowledge, this is the first evidence that ectopic nonneoplastic meningothelial cells may induce osteolysis on the orbital walls.

Zygomatico-orbital artery as a recipient vessel for microsurgical head and neck reconstruction.

The selection of recipient vessels in head and neck reconstruction is one of the key factors influencing its difficulty and outcome. We report a case of a microsurgical scalp reconstruction using the zygomatico-orbital artery as a recipient vessel.A 71-year-old woman had intractable skin ulcers on her head after neurosurgeries. Computed tomography angiography findings showed that the superficial temporal artery was obstructed and that the zygomatico-orbital artery ran forward and upward. She underwent scalp reconstruction using a free anterior lateral thigh flap. The zygomatico-orbital artery …

Rare retro-orbital intraconal occurrence of benign schwannoma - a case report.

Schwannomas otherwise referred to as neurilemmomas are benign, slowly progressing, encapsulated tumors arising from Schwann cells in the peripheral nerve sheath. They may grow along any peripheral or cranial nerve. Amongst the cranial nerve schwannomas, the eigth cranial nerve is the commonest site. But orbit is a rare site for their occurrence. Solitary orbital schwannomas account for 1% of orbital neoplasms. Orbital schwannomas arise from 3(th), 4(th), 6(th) cranial nerves and ciliary ganglion. Since complications like optic nerve compression and atrophy are likely to occur, early diagnosis …

Mature teratoma presenting as orbital cellulitis in a 5-month-old baby.

We have presented an interesting case of an unexpected orbital tumor in a 5-month-old female healthy baby who presented initially with a 1-week history of acute left proptosis, swollen eyelid, conjunctival chemosis and exposure keratopathy. She was treated with intravenous antibiotics without any improvement and was referred for the drainage of the left orbital abscess. The procedure was performed with partial resolution, however the proptosis persisted and an underlying orbital pathologic lesion was suspected. Further exploring the left orbital revealed a partially cystic tumor. The histopath…

Orbital phlebography in idiopathic intracranial hypertension and chronic tension-type headache.

BACKGROUND: Pathologic signs in orbital phlebographies have been reported in various neurological diseases. PURPOSE: To study if pathologic signs in orbital phlebography may be markers of inflammation primarily affecting intracranial capillaries, which would cause intracranial hypertension. MATERIAL AND METHODS: Two groups with different intracranial cerebrospinal fluid pressures (Pcsf) were compared as to inflammatory markers in serum and pathologic signs in orbital phlebographies. Nine consecutive patients with idiopathic intracranial hypertension (IIH) with bilateral papilledema and eight…

Orbital apex tumour caused by chronic lymphocytic leukaemia: an unlikely suspect.

An 88-year-old woman with a background of chronic lymphocytic leukaemia (CLL) and presented with unilateral ptosis and dull facial pains for 1 month. Examination revealed a complete right-sided ptosis and pupillary dilation. Vision in her right eye was limited to light perception. She had total external ophthalmoplegia. Her corneal reflex was not present in her right eye and she had lost sensation on the right side of her forehead. MRI revealed abnormal enhancement in the right orbital apex extending posteriorly to the sphenoid sinus. The mass invaded the superior orbital fissure, optic canal …
