Differences in opioid use and overdose among younger and older justice-impacted adults.

BACKGROUND: Justice-impacted persons aged 40 and up with substance use disorders (SUD) demonstrate increased health risks and health disparities relative to general population peers. Persons with SUD are less likely to age out of criminal behavior, appearing on criminal dockets, in jails, prisons, and under community supervision throughout the life course, with greater community-level cost burdens as they age. Justice system involvement presents health risks that compound with SUD to undermine well-being, which is amplified as people age and experience age-related health decline. Propensity fo

"I Smoked That Cigarette, and It Calmed Me Down": A Qualitative Analysis of Intrapersonal, Social, and Environmental Factors Influencing Decisions to Smoke Among Youth Experiencing Homelessness.

INTRODUCTION: Approximately 70% of youth experiencing homelessness smoke cigarettes; many try to quit and are interested in formal smoking cessation programs. The purpose of this study was to describe the intrapersonal, social, and environmental contexts associated with the most recent smoking experience among youth experiencing homelessness and (2) identify differences in contextual factors by age and willingness to quit. METHODS: Thirty-six youth experiencing homelessness aged 14-24 years and who reported current smoking were recruited from a drop-in center in a Midwestern city. Semi-struct

People with aphasia share their views on self-management and the role of technology to support self-management of aphasia.

PURPOSE: Self-management can lead to increased self-efficacy and improved quality of life for individuals with chronic conditions; however, there is little research exploring how self-management approaches can be used for chronic communication disorders, such as aphasia. Modifications to self-management approaches would be required to accommodate for aphasia; therefore, the input of people with aphasia (PwA) should be sought. The aims of the present study were: (1) To investigate and document the experiences and perspectives of PwA regarding self-management; (2) To explore how technology can b

Atypical Emotional Electrodermal Activity in Toddlers with Autism Spectrum Disorder.

Past studies in autism spectrum disorder (ASD) indicate atypical peripheral physiological arousal. However, the conditions under which these atypicalities arise and their link with behavioral emotional expressions and core ASD symptoms remain uncertain. Given the importance of physiological arousal in affective, learning, and cognitive processes, the current study examined changes in skin conductance level (ΔSCL) in 41 toddlers with ASD (mean age: 22.7 months, SD: 2.9) and 32 age-matched toddlers with typical development (TD) (mean age: 21.6 months, SD: 3.6) in response to probes designed to i

The effect of a 12-week tele-exercise using immersive virtual reality on functional capacity in adolescents with cystic fibrosis: A randomized controlled, single (assessor) - blind study.

BACKGROUND: Adolescents with cystic fibrosis (CF) have lower levels of physical activity compared to their peers. Risk of cross-contamination limits participation in face-to-face sport activities. Tele-exercise (TE) is a good alternative to deliver exercise remotely, without risk of infection. Immersive virtual reality (VR) could be a motivating method to improve the functional capacity. The primary aim was to evaluate the effect of a 12-week VR exercises on 6-min walk distance (6MWD). The secondary aims were to assess its effects on pulmonary function, muscle strength, quality of life, feasib

Age-related parietal GABA alterations in children with autism spectrum disorder.

GABA is the primary inhibitory neurotransmitter in the brain, and is essential to the balance of cortical excitation and inhibition. Reductions in GABA are proposed to result in an overly excitatory cortex that may cause, or contribute to, symptoms of autism spectrum disorder (ASD). This study employed a cross-sectional design to explore GABA+ differences in ASD and the impact of age, comparing 4-12 year olds with ASD (N = 24) to typically developing children (N = 35). GABA+ concentration was measured using edited magnetic resonance spectroscopy in the left parietal lobe. This study used a mix

Can Cochlear Implantation in Older Adults Reverse Cognitive Decline Due to Hearing Loss?

INTRODUCTION: Older adults with late-onset hearing loss are at risk for cognitive decline. Our study addresses the question of whether cochlear implantation (CI) can counteract this potential influence. We investigated whether cognitive performance in older adults with severe and profound hearing loss improves 12 months after CI to a level comparable to controls with normal hearing, matched for age, sex, and education level. DESIGN: This cohort study was performed at two tertiary referral centers. The study included 29 patients, of age between 60 and 80 years, with adult-onset, severe to prof

Social skills interventions for the autism spectrum: essential ingredients and a model curriculum.

This article outlines the ingredients the authors feel are critical to making social skills interventions successful for children with autism spectrum disorders. The authors described basic principles for teaching social skills that capitalize on the strengths of such children, while specifically addressing their deficits. The authors applied these widely used principles to group social skills intervention. In particular, social skills groups for children with ASD need to break down complex social behaviors into concrete steps and rules that can be memorized and practiced in a variety of setti

Caregiver feeding practices, dietary intake and weight status of very low birthweight infants compared to term-born peers at school-entry.

Very low birth weight (VLBW; <1250g) infants have an elevated risk of early onset cardiometabolic disease. While likely multi-factorial, the role of early caregiver feeding practices is unknown. We explored associations between primary caregiver feeding practices, diet, and weight of VLBW vs. socio-economically matched term-born peers at school-entry. VLBW children (n = 74; 61 % female; median [IQR] birth gestation of 27.8 [26.2, 29.1] weeks) and term-born controls (n = 74; 57 % female) participated at 5-7 years of age (school entry). We evaluated associations between caregiver-observed (video

Revealing disparities in representation in knowledge generation and guideline development.

BACKGROUND: Multidrug-resistant tuberculosis (MDR/RR-TB) is a major global health challenge, disproportionately affecting low- and lower-middle-income countries (LLMICs). The World Health Organization (WHO) generates guidance to address the problem. Here, we explore the extent to which guidance and related knowledge are generated by experts living in the most-affected countries and consider the results in the context of the movement to decolonize global health. METHODS: We examined the composition of World Health Organization (WHO) MDR/RR-TB treatment Guideline Development Groups (GDGs) from

Validity and reliability of an accelerometer-based assessgame to quantify upper limb selective voluntary motor control.

INTRODUCTION: Current clinical assessments measure selective voluntary motor control (SVMC) on an ordinal scale. We introduce a playful, interval-scaled method to assess SVMC in children with brain lesions and evaluate its validity and reliability. METHODS: Thirty-one neurologically intact children (median [1st-3rd quartile]: 11.6 years [8.5-13.9]) and 33 patients (12.2 years [8.8-14.9]) affected by upper motor neuron lesions with mild to moderate impairments participated. Using accelerometers, they played a movement tracking game (assessgame) with isolated joint movements (shoulder, elbow, l

Prevalence of permanent pacemaker implantation after conventional aortic valve replacement-a propensity-matched analysis in patients with a bicuspid or tricuspid aortic valve: a benchmark for transcatheter aortic valve replacement.

OBJECTIVES: Elective treatment of aortic valve disease by transcatheter aortic valve replacement (TAVR) is becoming increasingly popular, even in patients with low risk and intermediate risk. Even patients with a bicuspid aortic valve (BAV) are increasingly considered eligible for TAVR. Permanent pacemaker implantation (PMI) is a known-frequently understated-complication of TAVR affecting 9-15% of TAVR patients with a potentially significant impact on longevity and quality of life. BAV patients are affected by the highest PMI rates, although they are frequently younger compared to their tricus

Physical activity in children aged 7-13 years with juvenile idiopathic arthritis. Is it significantly altered? A case-control study.

INTRODUCTION: Juvenile idiopathic arthritis (JIA) is a chronic inflammatory disease in children under 16 years of age, often associated with joint damage. The etiopathogenesis of JIA involves an abnormal immune response triggered by the interaction of genetic, immunological, and environmental factors. Pain and swelling associated with JIA can lead to mobility issues and reduced physical activity (PA). Regular PA, tailored to the child's current health and functional status, is recommended for JIA patients. We aimed to assess whether JIA influences the choice of PA in children aged 7-13 years.

Joint kinematics and SPM analysis of gait in children with and without Down syndrome.

BACKGROUND: Individuals with Down syndrome (DS) walk with altered gait patterns compared to their typically developing (TD) peers. While walking at faster speeds and with external ankle load, preadolescents with DS demonstrate spatiotemporal and kinetic improvements. However, evidence of joint kinematic adjustments is unknown, which is imperative for targeted rehabilitation design. RESEARCH QUESTION: How does increasing walking speed and adding ankle load affect the joint kinematics of children with and without DS during overground walking? METHODS: In this cross-sectional observational stud
