Unmasking the high-risk phenotype in autoimmune gastritis: A pathologist's roadmap for the clinician.

Li et al's recent work on the risk factors for autoimmune gastritis provides clinical context for the vast majority of gastric neuroendocrine tumors (G-NETs). However, a deeper understanding of the underlying pathology is needed for precise clinical management. Our letter details the predictable stepwise progression of type 1 G-NETs from autoimmune-driven corporal atrophy and hypergastrinemia to a clear microscopic sequence of enterochromaffin-like cell precursor lesions, including linear hyperplasia, micronodular hyperplasia, and dysplasia. We highlight the definitive diagnostic thresholds th

Defining 2 biologically and clinically distinct groups in acute leukemia with a mixed phenotype.

A mixed phenotype (MP) is a characteristic of de novo MP acute leukemia (MPAL), but it can also be found in other leukemias. It poses substantial classification and management dilemmas. Herein, we report a large cohort of acute leukemia with MP and define acute myeloid leukemia with MP (AML-MP) and MPAL as 2 distinct groups by characterizing clinical, genetic, and transcriptomic features. Clinically, patients with AML-MP and MPAL were both treated with either AML- or acute lymphoblastic leukemia (ALL)-directed induction regimens. AML-MP has inferior responses (hazard ratio, 12.5; 95% confidenc…

Phenotype and function of IL-10-producing NK cells in individuals with malaria experience.

P.falciparum infection can trigger high levels of inflammation that lead to fever and sometimes severe disease. People living in malaria-endemic areas gradually develop resistance to symptomatic malaria and control both parasite numbers and the inflammatory response. We previously found that adaptive NK cells correlated with reduced parasite load and protection from symptoms. We also found that murine NK cell production of IL-10 protected mice from experimental cerebral malaria. Human NK cells can also secrete IL-10, but it is unknown what NK cell subsets produce IL-10 or if this is affected b

[Multifocal motor neuropathy following allogeneic hematopoietic stem cell transplantation for mixed-phenotype acute leukemia].

A 56-year-old woman with mixed-phenotype acute leukemia underwent allogeneic hematopoietic stem cell transplantation (allo-HSCT). She developed grade 2 acute graft-versus-host disease (GVHD), which subsequently improved, but did not develop chronic GVHD. Immunosuppressive therapy was discontinued 7 months after allo-HSCT, with no relapse of GVHD. However, around the same time, the patient began experiencing generalized fatigue, left eyelid ptosis, and right upper limb finger extensor weakness. Further examinations led to a diagnosis of Hashimoto's disease and multifocal motor neuropathy (MMN).…

Metabolic-inhibitor profiling links phenotype and transcriptome of Lachancea thermotolerans to wine fermentation chemistry.

We applied targeted metabolic inhibitors to 145 Lachancea thermotolerans strains to uncover fermentation traits with direct relevance to wine quality. Oxamate, a lactate dehydrogenase inhibitor, reduced lactic acid and total titratable acidity by 21% and 26%, respectively, while increasing succinic acid and pH without affecting ethanol levels, offering a promising strategy to fine-tune wine freshness and balance. Notably, industrial grape-associated strains (clusters C4-C6) maintained robust growth under oxamate stress, unlike wild strains, positioning oxamate resistance as a practical marker

KDM4 Orchestrates Epigenomic Remodeling of Senescent Cells and Potentiates the Senescence-Associated Secretory Phenotype.

Cellular senescence restrains the expansion of neoplastic cells through several layers of regulation. We report that the histone H3-specific … l reprogramming of the transcriptomic landscape, which underlie the senescence-associated secretory phenotype (SASP). Selective targeting of KDM4 dampens the SASP of senescent stromal cells, promotes cancer cell apoptosis in the treatment-damaged tumor microenvironment, and prolongs survival of experimental animals. Our study supports dynamic changes of H3K …

Oocyte/zygote/embryo maturation arrest: a clinical study expanding the phenotype of NOBOX variants.

PURPOSE: Primary ovarian insufficiency (POI) is an important cause of female infertility, stemming from follicle dysfunction or premature oocyte depletion. Pathogenic variants in genes such as NOBOX, GDF9, BMP15, and FSHR have been linked to POI. NOBOX, a transcription factor expressed in oocytes and granulosa cells, plays a pivotal role in folliculogenesis. Loss-of-function variants in NOBOX are reported in 1-2% of POI women. This study aims to describe the association of novel NOBOX variants with a distinct oocyte, zygote, and embryo maturation arrest (OZEMA) phenotype in infertile women. M…

Snrnp25 is a candidate for the peri-implantation lethal phenotype of the Hba deletions.

Mutations in adult hemoglobin alpha genes in humans lead to blood disorders commonly known as α-thalassemia. In search of a mouse model for this disease, mutagenesis screens have identified several deletions that resemble these phenotypes. The Hbab2(th) deletion, in particular, replicates the characteristics of alpha-thalassemia minor in heterozygous mice but presents a homozygous embryonic lethal phenotype. Previous analyses of Hbab2(th) mice suggested that the deletion affects both Hba genes (Hba-a1 and Hba-a2) and considered epidermal growth factor receptor (Egfr) or rhomboid 5 homolog 1 (R…

KLK7 overexpression promotes an aggressive phenotype and facilitates peritoneal dissemination in colorectal cancer cells.

Colorectal cancer (CRC) incidence and mortality continue to rise globally and new prognostic biomarkers are required for the development of targeted therapies. Several studies have suggested that tissue kallikrein-related peptidases (KLKs), including KLK7, contribute to tumorigenesis. We previously demonstrated KLK7's tumor-promoting role both in vitro and in vivo, but its role in CRC metastasis remains unclear. Here, using the Cancer Genome Atlas (TCGA), we confirmed that KLK7 expression is upregulated in advanced stages of CRC and its association with shorter progression-free survival (PFS)

Disease Phenotype Significantly Influences the Outcome After Discontinuation of Ruxolitinib in Chronic Phase Myelofibrosis.

INTRODUCTION: In patients with myelofibrosis (MF), overall survival (OS) after ruxolitinib discontinuation is poor, with leukemic transformation, clonal evolution and thrombocytopenia as the main factors worsening prognosis. PATIENTS AND METHODS: To assess the impact of disease phenotype on outcome after ruxolitinib discontinuation in chronic phase patients, we performed a sub-analysis of the "RUX-MF" study (NCT06516406), which now includes 1055 MF patients who received ruxolitinib in a real-life context. RESULTS: After a median follow-up of 3.3 years, 397 patients discontinued ruxolitinib t…

Expanded CRB2-related disease phenotype: multisystem involvement and post-transplant complications in monozygotic twins.

BACKGROUND: Congenital nephrotic syndrome (CNS) is a rare disorder caused by mutations in genes essential for podocyte function and glomerular slit diaphragm integrity, including CRB2 (Crumbs Cell Polarity Complex Component 2). CRB2 mutations are linked to focal segmental glomerulosclerosis and ventriculomegaly with cystic kidney disease, but their full phenotypic spectrum remains unclear. We describe the clinical course of monozygotic twins with a homozygous CRB2 mutation, highlighting severe complications following kidney transplantation. METHODS: The twins, who were followed and managed th

From adaptation to exhaustion: defining exposure-related malnutrition as a bioenergetic phenotype of aging.

Aging is increasingly understood not as the passive accumulation of molecular damage, but as the cumulative cost of unresolved physiological adaptation under bioenergetic constraint. This review introduces Exposure-Related Malnutrition (ERM) as a mechanistically grounded and clinically actionable phenotype of early maladaptation. ERM arises from sustained metabolic strain during chronic stress exposure and manifests not through overt weight loss or nutrient deficiency, but through subtle, multisystem declines in physical, cognitive, and regenerative capacity. These include fatigue, impaired re…
