Utility of Immunocytochemistry in Cerebrospinal Fluid of B-ALL Patients for CNS Involvement: A Pilot Study.

BACKGROUND: Central nervous system (CNS) involvement in paediatric B-acute lymphoblastic leukaemia (B-ALL) significantly impacts relapse risk and survival. Conventional cytomorphology (CM) of cerebrospinal fluid (CSF) is rapid and specific but has low sensitivity because of limited cellularity and morphological overlap with reactive lymphocytes. OBJECTIVE: To evaluate the diagnostic utility and resource efficiency of immunocytochemistry (ICC) as an adjunct to CM in detecting leukaemic infiltration of CSF in paediatric B-ALL. METHODS: In Cohort 1 (n = 45), all CSF samples underwent CM and ICC

[Treatment of migrant children with acute lymphoblastic leukemia in Argentina].

INTRODUCTION: The probability of survival for children diagnosed with acute lymphoblastic leukemia (ALL) is below 60%, in Paraguay, Peru, and Bolivia, while in Argentina achieves 80%. Patients from neighboring countries migrate to Argentina looking for better curative options. The objective was to analyze the clinical characteristics, event-free survival (EFS), overall survival (OS), and treatment response in migrant patients with ALL, with or without previous treatment, and to compare them with Argentine patients. MATERIALS AND METHODS: A retrospective, cross-sectional, observational, and de

Differential Expression and Target Gene Analysis of PBMC-Derived microRNAs as Prognostic Biomarkers in Acute Lymphoblastic Leukemia.

Acute lymphoblastic leukemia (ALL) is a clinically diverse cancer in which microRNA (miRNA)-mediated post-transcriptional regulation contributes to leukemogenesis and subtype heterogeneity. In this study, miRNA expression profiling by microarray was performed on ALL cases (B-ALL and T-ALL) and healthy controls. Data were normalized and analyzed for differential expression using false discovery rate (FDR)-adjusted p-values. Differentially expressed miRNAs were further examined using unsupervised visualization to assess overall disease-related expression patterns. To explore their biological sig

Single-session therapeutic plasma exchange as salvage therapy for pegaspargase-induced severe acute pancreatitis accompanied by multiple organ dysfunction in a pediatric patient with B-cell precursor acute lymphoblastic leukemia: a case report.

BACKGROUND: Pegaspargase-related severe acute pancreatitis (SAP) combined with persistent multiple organ dysfunction syndrome (MODS) is a life-threatening adverse reaction in children receiving acute lymphoblastic leukemia (ALL) chemotherapy. Standard intensive supportive care often fails to halt progressive organ damage, while clinical data regarding therapeutic plasma exchange (TPE) as a rescue intervention in pediatric populations remain scarce. Guidelines released by the American Society for Apheresis (ASFA) do not recommend routine TPE administration for all patients diagnosed with acute

Use of Universal Premedication in the Prevention of Allergic Reactions to PEGylated-Asparaginase in Children and Adolescents With Acute Lymphoblastic Leukemia and Lymphoblastic Lymphoma.

Asparaginase is an established chemotherapeutic agent for acute lymphoblastic leukemia (ALL) and lymphoblastic lymphoma (LL) treatment. Pegylated asparaginase (PEG-Asp) is first-line but may lead to allergic reactions in ∼30% of recipients, leading to substitution by Erwinia -derived asparaginase. Studies demonstrate mixed results on the impact of universal premedication on the incidence of infusion reactions. Our center adopted universal premedication with H1 and H2 antagonists and nadir serum asparaginase activity (SAA) level monitoring in 2019. We assessed the impact of universal premedicat

Long-term oral and dental effects in childhood acute lymphoblastic leukemia survivors: a cross-sectional study comparing early and late post-treatment periods.

BACKGROUND: Childhood cancer survivors face significant long-term oral health challenges following antineoplastic treatment. The primary aim of this study was to examine the effect of time elapsed since completion of antineoplastic treatment on oral health parameters - caries experience, salivary function, and dental developmental anomalies - in childhood ALL survivors. Secondary objectives included comparing oral health parameters between survivors and healthy sibling controls and identifying independent predictors of caries outcomes. METHODS: This cross-sectional study included 38 ALL survi

clinTALL: machine learning-driven multimodal subtype classification and treatment outcome prediction in pediatric T-ALL.

BACKGROUND: Childhood T-lineage acute lymphoblastic leukemia (T-ALL) is an aggressive hematologic malignancy with poor prognosis. Differently from B-cell precursor ALL, T-ALL lacks effective risk stratification strategies. A recent study has integrated whole genome and whole transcriptome data to define 17 distinct molecular subtypes with prognostic significance. However, clinical translation of this knowledge remains challenging due to the complexity of interpreting high-dimensional multi-omics-based data. METHODS: Here, we present clinTALL, a deep learning based multi-task pipeline for pedi

Gasoline stations and risk of childhood cancer: a population-based cohort study in Quebec, Canada.

Gasoline (petrol) stations emit benzene and other volatile organic compounds (VOCs) into ambient air, potentially increasing childhood cancer risk, though epidemiological evidence remains limited. We investigated whether residential proximity to gasoline stations at birth was associated with childhood cancer incidence. We conducted a population-based cohort study of 824,414 newborns in Quebec, Canada. We constructed three exposure metrics at the birth residence: A) number of gasoline stations within 250 m; B) distance to the nearest gasoline station; C) sum of inverse distances to all gasoline

Effect of high-dose methotrexate infusion duration on 48-hour drug levels and toxicity in children with acute lymphoblastic leukaemia: a quality-of-care study.

BACKGROUND: High‑dose methotrexate (HD‑MTX) is a cornerstone of pediatric acute lymphoblastic leukaemia (ALL) therapy. However, deviations from protocol-mandated infusion durations need regular monitoring as part of standard quality care. Hence, an observational quality assessment study was planned with a secondary objective to evaluate the acute toxicity in patients who did not receive the infusion within the recommended time period. METHODS: This study enrolled 42 children (75 cycles) who received HD-MTX (3-5 g/m2 over 24 h) according to the ICiCLe ALL-14 protocol. The actual infusion durat

Blinatumomab administration in the outpatient setting: Safety and health care utilization.

BACKGROUND: Blinatumomab, a bispecific CD19 × CD3 antibody is effective for the treatment of B-cell acute lymphoblastic leukemia (ALL). Because of its immune toxicities, namely cytokine release syndrome (CRS) and immune effector cell-associated neurotoxicity syndrome (ICANS), the US Food and Drug Administration (FDA) recommends preemptive hospitalization for the initiation of cycle 1 (C1) and cycle 2 (C2). The necessity of this approach is unclear. METHODS: The authors conducted a retrospective cohort study among all patients treated with blinatumomab between 2012 and 2025 at two academic can

Bridging the Gap: Cost-Effective Strategies for Detecting Ph-Like B-Lineage ALL in Resource-Limited Settings.

Acute lymphoblastic leukemia (ALL) is a complex hematologic disorder primarily affecting children, characterized by genetic mutations that disrupt normal lymphoid cell differentiation and promote abnormal proliferation. A particularly high-risk subtype, Philadelphia chromosome-like ALL (Ph-like ALL), mirrors the genetic profile of Philadelphia chromosome-positive (Ph-positive) ALL but lacks the BCR::ABL1 fusion gene. While Ph-like ALL has been extensively studied in high-income countries (HICs), it remains under-researched in low- and middle-income countries (LMICs), where resource limitations
