Radiation Therapy in Node-Positive Prostate Cancer: Current Evidence and Future Directions.

The management of node-positive prostate cancer (N1 PCa) remains a subject of ongoing debate, with growing evidence supporting the role of radiation therapy (RT) alongside systemic therapy. Traditionally, N1 disease was considered metastatic; however, advancements in imaging and treatment have redefined its clinical relevance. This narrative review evaluates current evidence, focusing on recent literature and ongoing clinical trials in N1 PCa, both in the definitive and postoperative setting. Prostate-specific membrane antigen (PSMA) positron emission tomography-computed tomography (PET/CT) ha…

XFBLD-Platform: a crystallographic fragment-screening platform at Shanghai Synchrotron Radiation Facility.

Fragment-based lead discovery (FBLD) is an efficient and effective method for identifying novel chemical scaffolds that have advantages in drug development. X-ray crystallography has an inherent advantage in recognizing low-affinity fragments and integrates fragment identification with complex structure determination, making it an increasingly important tool for screening fragment compounds. Here, we introduce a crystallographic fragment-screening platform developed by the biological macromolecular crystallography group at Shanghai Synchrotron Radiation Facility, named the XFBLD-Platform. This…

Radiation Exposure During Contrast-Enhanced Videofluoroscopic Swallow Studies in Dogs and Cats.

Contrast-enhanced videofluoroscopic swallow studies are considered the gold standard diagnostic for determining underlying etiologies of dysphagia. Swallow studies are commonly performed procedures utilizing fluoroscopy in which personnel are exposed to ionizing radiation. The study objective was to document radiation exposure to individual personnel involved in performing gastrointestinal swallow studies in dogs and cats, compare radiation exposure based on the localization of disease, and evaluate radiation exposure based on the experience (i.e., year of training) of the diagnostic imaging r…

TRAIL agonists rescue mice from radiation-induced lung, skin, or esophageal injury.

Radiotherapy can be limited by pneumonitis, which is impacted by innate immunity, including pathways regulated by TRAIL death receptor DR5. We investigated whether DR5 agonists could rescue mice from toxic effects of radiation and found that 2 different agonists, parenteral PEGylated trimeric TRAIL (TLY012) and oral TRAIL-inducing compound (TIC10/ONC201), could reduce pneumonitis, alveolar wall thickness, and oxygen desaturation. Lung protection extended to late effects of radiation including less fibrosis at 22 weeks in TLY012-rescued survivors versus unrescued surviving irradiated mice. Wild…

Radiation-induced congenital malformations in Fukushima after the Fukushima Daiichi Nuclear Disaster.

The Fukushima Daiichi Nuclear Disaster (FDND) occurred in 2011, which occurred after the Great East Japan Earthquake. However, how the incidence of radiation-induced malformations in Fukushima has been affected by FDND remains to be elucidated. To address this, we analyzed birth data from Fukushima and other areas in Japan from the International Clearinghouse for Birth Defects Surveillance and Research Japan Center, including information on birth defects between January 2010 and December 2022. Among the registered birth defects, microcephaly, microphthalmia, and neural tube defects were classi…

Radiation-induced cerebral cavernous malformation following stereotactic radiosurgery for a brain metastasis.

Radiation-induced cerebral cavernous malformations (RICMs) are rare complications of stereotactic radiosurgery (SRS). We present a woman in her 70s who underwent Gamma Knife radiosurgery for breast cancer brain metastases. She subsequently developed a haemorrhagic lesion in the left parietal lobe, confirmed as RICM by histopathological examination. Postoperatively, the patient recovered well, with reduced headache severity and no new neurological deficits. This case underscores the importance of long-term clinical and radiological follow-up following SRS and the role of resection in symptomati…

Synergistic Genotoxic Effects of Gamma Rays and UVB Radiation on Human Blood.

Exposure to ionizing and non-ionizing radiation from environmental and clinical settings can significantly threaten genomic stability, especially when combined. This ex vivo study investigates the potential combined effects of gamma radiation and ultraviolet B (UVB) exposure on human peripheral blood mononuclear cells (PBMCs) from healthy donors by exposing whole blood and isolated PBMCs to 1 Gy of gamma rays, to an absolute dose of approximately 100 J/m2 of UVB, or to their combination. Combined exposure resulted in significantly elevated γH2AX foci formation and chromosomal aberrations relat…

IntraOperative Cryoablation Therapy (IOCT): A Novel Alternative to Post-Lumpectomy Radiation Therapy.

OBJECTIVE: Today, breast cancer patients have two choices for initial treatment of their breast cancer: 1) surgical excision of the cancer followed by radiation therapy or 2) removal of the cancerous breast (mastectomy). SenoGuard, an investigational intraoperative cryoablative therapy (IOCT) administered by a surgical team as an essential element of breast sparing surgery, offers a third option that would eliminate the need for radiation. We aimed to evaluate the feasibility and safety of cryoablation of the surgical cavity using a new 2 cm cryoablation probe and cryogenic system as a potenti…

Radiation-induced injury and the gut microbiota: insights from a microbial perspective.

Although radiotherapy is the second most effective cancer treatment, radiation injuries limit its use. About 80% of abdominal-pelvic radiotherapy patients develop acute radiation enteritis, with 20% discontinuing radiotherapy. The lack of effective mitigation measures restricts its clinical application. Recent studies have proposed gut microbiota as a potential biomarker for radiation injuries. However, the interaction between gut microbiota and radiation injuries remains poorly understood. This review summarizes two forms of interaction between gut microbiota and radiation injuries based on t…

Distribution of macrophages in the cochlea following radiation-induced sensorineural hearing loss.

Radiation-induced sensorineural hearing loss (RISNHL) is a common and irreversible complication of radiotherapy for head and neck tumors, with currently no effective treatment. While traditional research has primarily focused on the direct effects of radiation on cochlear hair cells (HCs), this alone does not explain the early onset of hearing loss observed before HC degeneration. Emerging evidence from our preliminary study suggests that the pathogenesis of RISNHL may be driven by significant microenvironmental alterations within the cochlea, particularly involving dysfunction of the stria va…

Radiation Decreases Bone Density while Diabetes Increases Osteoarthritis in Non-Human Primates.

Total-body irradiation has long-term effects and may cause joint damage, especially in individuals with diabetes. Lowered bone mineral density and arthropathy related to radiation can contribute to the development of osteoarthritis. Monitoring these conditions in humans is challenging, but non-human primate models allow for longitudinal tracking of metabolic and degenerative changes. This study investigated the effect of radiation on joint health in non-human primates, including bone mineral density, and examined how diabetes influences the development of osteoarthritis. We hypothesized that j…

Taurine-loaded covalent organic frameworks for alleviating post-radiation bone regeneration disorder.

Radiation-induced jaw injury, characterized by inflammation, bone damage, and infection, is a frequent complication of radiotherapy for head and neck malignancies. However, effective biomaterials designed based on the microenvironmental characteristics of irradiated jaws for alleviating bone regeneration disorders remain lacking. Taurine (Tau) and hypotaurine metabolism are significantly upregulated in irradiated mandibular stromal cells, and orally administered Tau can markedly promote jaw regeneration. In order to increase local taurine accumulation and augment therapeutic outcomes, this stu…

Therapeutic radiation drives leptomeningeal dissemination of medulloblastoma through an innate immune process.

Leptomeningeal metastases are the most important source of morbidity and mortality for medulloblastoma patients. Radiation of the entire brain is highly effective in the treatment and/or prevention of medulloblastoma leptomeningeal metastases. Infants treated on clinical trials with focal tumor radiation recur metastatically, whereas infants treated with only chemotherapy relapse locally. In murine medulloblastoma model systems, provision of a single dose of radiation to the tumor drives leptomeningeal dissemination. An inflammatory response after radiation-induced tumor cell death recruits a …

COG133 peptide-conjugated lipid nanoparticles sensitize medulloblastoma to radiation therapy in mice.

Medulloblastoma (MB) is a malignant brain tumor that often arises in the cerebellum and has a propensity for spreading to the spinal cord or other parts of the central nervous system (CNS). Craniospinal irradiation (CSI) has long been a cornerstone in the treatment of MB, especially for patients with high-risk or metastatic disease. However, CSI often leads to long-term neurocognitive deficits, including learning disabilities, and growth abnormalities, especially in children. In this study, we aimed to decrease the dose of irradiation and the proliferation of MB by using Volasertib (VSB), a Po…
