Potential impact of obstetric history on postmenopausal fragility fracture risk: A reassessment of the Fracture Risk Assessment Tool.

OBJECTIVES: This study aimed to evaluate the impact of integrating obstetric parameters into the Fracture Risk Assessment Tool (FRAX) on the precision of risk assessment. PATIENTS AND METHODS: In this retrospective study, patients who experienced postmenopausal fragility fractures of the distal radius, proximal femur, or lumbar vertebrae between January 1, 2021, and December 31, 2023, were included. Obstetric histories, along with standard FRAX parameters, were obtained by phone interviews. Based on the FRAX major osteoporotic fracture risk score calculated without bone mineral density, patie

An update on methods for assessing bone quality and health in Cystic fibrosis.

With increasing life expectancy in people with Cystic fibrosis (CF), the focus of clinical care has shifted to management and prevention of non-pulmonary comorbidities. CF related bone disease, defined by low bone mineral density (BMD), is prevalent across all age groups and acknowledges the increased fractures rates that negatively impact lung function and quality of life. Dual energy X-ray absorptiometry (DXA) measurement of bone mineral content (BMC) and "areal" BMD (aBMD) is recommended for identifying and monitoring bone health in children and adults due to its low cost, low radiation exp

The clinical application of high-resolution peripheral computed tomography (HR-pQCT) in adults: state of the art and future directions.

High-resolution peripheral computed tomography (HR-pQCT) was developed to image bone microarchitecture in vivo at peripheral skeletal sites. Since the introduction of HR-pQCT in 2005, clinical research to gain insight into pathophysiology of skeletal fragility and to improve prediction of fractures has grown. Meanwhile, the second-generation HR-pQCT device has been introduced, allowing novel applications such as hand joint imaging, assessment of subchondral bone and cartilage thickness in the knee, and distal radius fracture healing. This article provides an overview of the current clinical ap

Kirschner wire infections in pediatric orthopaedic surgery.

BACKGROUND: Few studies have described the presentation, bacteriology, risk factors, and complications of Kirschner wire infections in pediatrics. The purpose of this study is to describe these factors to better understand, prevent, and treat infectious complications of smooth wires. METHODS: A retrospective review was performed to identify all patients (birth to 16 y) who were hospitalized for Kirschner wire infection from 1995 to 2012. Presentation, hospital course, bacteriology, outcomes, and complications were recorded. A management algorithm was developed from the experience. RESULTS: K

Systematic review of radiocarpal dislocations: epidemiology, treatment modalities, and outcomes.

PURPOSE: Radiocarpal dislocation (RCD) is a rare but severe injury, often resulting from high-energy trauma such as motor vehicle accidents and falls. This study aims to synthesize existing case reports and series to provide insights into the epidemiology, associated injuries, and management strategies for RCD. METHODS: A systematic search of PubMed, Scopus, and Web of Science was conducted, including English-language case reports and series up to August 5, 2023. Data extraction followed PRISMA guidelines, with bias assessment using Joanna Briggs Institute criteria. Descriptive statistics and

Feasibility and clinical usefulness of deep learning-accelerated MRI for acute painful fracture patients wearing a splint: A prospective comparative study.

OBJECTIVE: To evaluate the feasibility and clinical usefulness of deep learning (DL)-accelerated turbo spin echo (TSEDL) sequences relative to standard TSE sequences (TSES) for acute radius fracture patients wearing a splint. METHODS: This prospective consecutive study investigated 50 patients' preoperative wrist MRI scans acquired between July 2021 and January 2022. Examinations were performed at 3 Tesla MRI with body array coils due to the wrist splint. Besides TSES obtained according to the routine protocol, TSEDL sequences for axial T2-, coronal T1-, and coronal PD-weighted TSE sequences

To Angulate or Not to Angulate the Ulna during the Progressive Distraction Period Performed with a Monolateral External Fixator in Paediatric Patients with a Chronic Monteggia Fracture?

Background and Objectives: The purpose of this study was to compare the clinical and radiographic evolution of chronic Monteggia fractures (CMFs) treated by ulnar osteotomy and monolateral external fixators (MEFs) with or without angulation of the ulna during the distraction period. Materials and Methods: This retrospective study evaluated 20 children (14 boys and 6 girls) with CMFs. According to the strategy of ulnar lengthening, two groups of patients were identified: patients undergoing gradual lengthening with (Group A, n = 11) or without ulna angulation (Group B, n = 9). The mean age at t

Short and long-arm fiberglass cast immobilization for displaced distal forearm fractures in children: a randomized controlled trial.

PURPOSE: The aim of this study was to investigate whether short-arm fiberglass cast (SAC) immobilization provides fracture stabilization comparable to that of long-arm cast (LAC) treatment of displaced distal forearm fractures after closed reduction in paediatric patients. METHODS: A prospective, randomized, controlled trial of children aged four to 16 years (mean 9.9 years) was designed with a sample of 120 children, whose size was set a priori, with 60 treated with SAC and 60 with LAC. The primary outcome was fracture stability and rate of loss of reduction. The secondary outcome analysis e

Paediatric post-traumatic osseous cystic lesion following a distal radial fracture.

Post-traumatic osseous cystic lesions are a rare complication in children. An aetiology of intramedullary fat seepage through the damaged bone cortex and its entrapment within the subperiosteum has been proposed. These lesions run a benign course and usually resolve spontaneously. The presence of fatty marrow gives it a distinct appearance which aids in its diagnosis and differentiation from other bone lesions. This case demonstrates a fat-fluid level within the subperiosteal cystic lesion in Magnetic Resonance Imaging (MRI) and this is a typical feature of post-traumatic cystic lesion in a ch

Giovanni Battista Monteggia (1762-1815).

Giovanni Battista Monteggia was born in Laverne on the 8th of August 1762. Monteggia started his education in the School of Surgery at the Hospital Maggiore in Milano in 1779.This hospital was called "Big House"and it is one of the oldest medical institutions in Italy. He passed exam in surgery in 1781. Monteggia was promoted to assistant at surgery in Maggiore hospital in 1790. He was among the first who gave a complete clinical description of polio. He described traumatic hip dislocation and special forearm fracture which was named after him. Strictly speaking, a Monteggia fracture is a frac

Valvular endocarditis and septic thrombosis associated with a radial fracture in a red-tailed hawk (Buteo jamaicensis).

A free-ranging adult female red-tailed hawk died suddenly after 3 weeks in rehabilitation for a radial fracture. Cause of death was septic thrombosis from a chronic bacterial valvular endocarditis, probably associated with injury at the fracture site. The challenge of clinical diagnosis of sepsis in wild birds is emphasized.

The distal radioulnar joint capsule: clinical anatomy and role in posttraumatic limitation of forearm rotation.

Posttraumatic limitation of forearm rotation can be the result of pathology at any location along the forearm axis. Scar contracture of the distal radioulnar joint (DRUJ) capsule, independent of the triangular fibrocartilage complex (TFCC), is one of the sources that may influence the pronosupination arc. We dissected the wrists of 8 fresh-frozen cadaver specimens to characterize the precise anatomy, relationships, and dynamic characteristics of the entire DRUJ capsule. Additionally, we performed surgical DRUJ capsulectomy in 9 patients with recalcitrant limited forearm pronosupination that wa

Elbow dislocation with ipsilateral radial shaft fracture. An unusual outcome.

The authors describe an uncommon injury of the upper limb, namely, dislocation of the elbow with fracture of the ipsilateral radial shaft, in a young man. After closed reduction of the joint, the radial head was found to be subluxated, and when a nonanatomic internal fixation of the radial shaft was undertaken, the radial head became dislocated. Repeat operation with removal of the implants and internal fixation with correction of rotation and angulation at the fracture site and reconstruction of the annular ligament restored normal anatomy.

[Dislocations of both forearm bones complicated by rupture of the capitate eminence of the humerus in children].

During the recent 21 years thirty one children have been observed with dislocations of the both forearm bones complicated by rupture of the capitate eminence. Closed reposition of the fragments has been performed in 23 children, in 8 children open reposition was followed by pin fixation with a thrust platform. The terms of fixation are dependent upon the patient's age and reposition quality. At an average they equaled 15-16 days in uncomplicated cases for patients aged 2-7 years, 17-18 days for 8-11 years-aged patients, 19 days for 12-14 years-aged patients. Good long-term results were achieve

Fractures of the radial head in adults with the complication "cubitus valgus".

Cubitus valgus is a well-known complication of fracture of the bones of the elbow in childhood, but it is an uncommon deformity in adults. Three adult patients are presented, in each of whom cubitus valgus developed as a complication of a fracture and resection of the radial head. The importance of reestablishing the integrity of the ligaments of the elbow following severe injuries to the elbow is stressed.
