Long-term secondary prevention with colchicine in patients with CAD: an updated systematic review and meta-analysis.

The CLEAR SYNERGY trial failed to demonstrate any benefit of colchicine in patients with acute myocardial infarction (MI). In this meta-analysis of the randomized clinical trials (RCTs) of colchicine in coronary artery disease (CAD), we updated the estimates of effect on cardiovascular events in patients with CAD. We searched for the largest RCTs of colchicine in CAD patients. The effect was assessed in a pooled analysis of individual events such as MI, urgent coronary revascularization (UR), stroke, cardiovascular mortality, and total mortality, for each of these endpoints using hazard ratios

Early Change in Proteinuria as a Surrogate Endpoint in Studies of IgA Nephropathy: An Updated Patient-Level Meta-analysis and Discussion of Appropriate Methodology.

INTRODUCTION: Immunoglobulin A (IgA) nephropathy is a rare renal condition associated with a high risk of kidney failure. However, conducting phase 3 clinical trials with kidney failure as a primary endpoint is generally not feasible because of sample size and protracted follow-up requirements. Hence, surrogate outcomes are necessary when assessing new treatments in randomized controlled trials. Previous meta-analyses have assessed the validity of early change in proteinuria as a surrogate endpoint, and the present research updates the analysis with additional patient-level data. METHODS: The

Hierarchical Grouped Horseshoe Priors for Subgroup Identification and Estimation.

A common issue in randomized clinical trials (RCTs) is the identification of subgroups and the estimation of their effects. Typically, RCTs are not powered to estimate the effects of subgroups. However, in some circumstances, treatment may work for some groups and not others, and it is of interest to identify these subgroups and estimate their treatment effects. In this paper, we introduce a novel hierarchical grouped horseshoe prior (HGHP) for subgroup identification and estimation. We show via simulation that our proposed approach yields superior positive predictive value and narrower credib

Characterization changes and research waste in randomized controlled trials of global gastroesophageal reflux disease and hiatus hernia over the past 20 years.

BACKGROUND: The results of many large randomized clinical trials (RCTs) have transformed clinical practice in gastroesophageal reflux disease (GERD) and esophageal hiatal hernia (HH). However, research waste (i.e., unpublished data, inadequate reporting, or avoidable design limitations) remains a major challenge to evidence-based medicine. METHOD: A cross-sectional analysis was conducted to comprehensively review and evaluate RCTs related to GERD and esophageal HH, registered in the ClinicalTrials.gov database between 2003 and 2023. A sample of eligible RCTs was identified by excluding early-

Beyond epilepsy management: A narrative review of the health effects of ketogenic diets.

The Ketogenic diet (KD), characterized by a low-carbohydrate/high-fat intake, induces a state of ketosis, where the body primarily relies on using fat for energy instead of carbohydrates. This diet is historically used in epilepsy management. Recently, KD has been the subject of increased research and public interest for its additional potential health effects. This narrative review offers an overview of the potential health effects of the KD and outlines its clinical implications. We searched PubMed and Google Scholar for randomized controlled trials (RCTs) published between 2014 and 2024, ex

Does the frailty index applied to randomised controlled trials really measure frailty?

BACKGROUND: Cumulative deficit frailty indices from randomised controlled trials (RCT) are increasingly used to assess whether trial findings are applicable to people living with frailty. The aim of this paper was to examine the range and type of deficits included in these frailty indices and compare these to those from cohort studies. METHODS: We identified RCTs assessing treatment effect modification using the cumulative deficit frailty index, as well as cohort studies assessing mortality risk associated with frailty, from recent systematic reviews. We extracted the deficits included in the
