Patterns and predictors of patient and caregiver engagement in heart failure care: a multi-level dyadic study.

BACKGROUND: Heart failure is a burdensome clinical syndrome, and patients and their caregivers are responsible for the vast majority of heart failure care. OBJECTIVES: This study aimed to characterize naturally occurring archetypes of patient-caregiver dyads with respect to patient and caregiver contributions to heart failure self-care, and to identify patient-, caregiver- and dyadic-level determinants thereof. DESIGN: Dyadic analysis of cross-sectional data on patients and their caregivers. SETTING: Outpatient heart failure clinics in 28 Italian provinces. PARTICIPANTS: 509 Italian heart

Development and implementation of multidisciplinary care model to address the intersection of pain and opioid use disorder among patients on long term opioid treatment.

BACKGROUND: Chronic non-cancer pain and opioid use disorder (OUD) are common co-morbidities. Clinical guidelines and regulatory changes have created challenges for providers and programs in which chronic pain and OUD treatment may be sub-optimally integrated for patients receiving long-term opioid therapy (LTOT). METHODS: We identified a cohort of 61 patients in HIV primary care who met criteria for LTOT (i.e. prescribed opioids for chronic non-cancer pain  >90 days) and who experienced disruptions in pain management and present an analytic case study. A multidisciplinary team developed a two

Epiploic appendagitis: An overlooked cause of acute abdominal pain.

BACKGROUND: Epiploic appendagitis is a rare, often underrecognized cause of acute abdominal pain. Misdiagnosis can lead to unnecessary hospitalization, antibiotic use, or surgical intervention. Advances in imaging have improved the recognition of this self-limiting condition, but clinical awareness remains critical. AIM: To provide a comprehensive update on the epidemiology, anatomy, pathogenesis, clinical presentation, diagnostic strategies, differential diagnosis, and management of epiploic appendagitis, emphasizing its distinguishing features from other causes of acute abdomen. METHODS: A

Linking HIV-positive people in addiction care to HIV services in St. Petersburg, Russia - Mixed-methods implementation study of strengths-based case management.

Access to HIV services for HIV-positive patients in addiction care is challenging in Russia, because both care systems are organised independently from each other. Strengths-based case management is an effective strategy to connect people with HIV (PHIV) to HIV care. This mixed-methods study's objective was to investigate implementation of a case management intervention in St. Petersburg, Russia, designed to connect PHIV who inject drugs to HIV care. We analysed survey data from 118 HIV-positive patients in addiction care and conducted six focus groups (n=38). Quantitative analyses of fidelity

Frozen shoulder: Diagnosis and treatment of adhesive capsulitis.

Adhesive capsulitis, commonly referred to as frozen shoulder, is a frequent cause of shoulder pain and stiffness in middle-aged adults and is associated with substantial functional limitation. Although traditionally considered a self-limited condition, growing evidence suggests that many patients experience prolonged symptoms and incomplete recovery. Diagnosis is primarily clinical, based on characteristic history and examination findings demonstrating global restriction of passive glenohumeral motion, with imaging reserved for exclusion of alternative pathology. Management is predominantly no

A modified endoluminal vacuum therapy system for salvage management of recurrent anastomotic leak after revisional bariatric surgery.

Endoluminal vacuum therapy (EVT) has emerged as a minimally invasive and effective approach for the management of gastrointestinal anastomotic leaks, particularly in complex or refractory cases. Meta-analyses report pooled clinical success rates of ~80%-85%, with evidence suggesting higher success compared with self-expanding metal stents. We report the case of a 55-year-old female with a history of sleeve gastrectomy who underwent revisional bariatric surgery complicated by multiple anastomotic failures, open abdomen management, and recurrent gastrojejunal leakage. After failure of repeated s

Physiotherapy Approaches for Temporomandibular Disorders: A Multimodal Conservative Management Strategy.

Temporomandibular disorder (TMD) is a complex, multifactorial disorder affecting the masticatory muscles, the temporomandibular joint, and associated structures. These disorders are also associated with other symptoms affecting the head and neck region, such as headaches, ear-related symptoms, cervical spine dysfunction, and altered head and neck posture. TMDs require multidisciplinary care. Physiotherapy is one of the pillars of TMD management; it is part of the non-invasive treatment recommended by medical guidelines. It aims to eliminate or significantly reduce pain and functional discomfor

Recurrent Midface Deep Ulcer Caused by Excoriation Disorder.

Excoriation disorder, or skin-picking disorder, is a compulsive condition that leads to self-inflicted tissue damage. The authors present a case of a 43-year-old female with a large and persistent facial wound exposing the left malar bone that remained unresponsive to multiple medical interventions. Imaging revealed a 4×3x4 cm defect extending into the intraoral cavity. Despite surgical debridement and reconstruction, wound dehiscence persisted until psychiatric evaluation confirmed excoriation disorder. Treatment included cervicofacial and subperiosteal rotation flaps, acellular dermal matrix

Intralenticular metallic foreign body 'locked-in' the posterior capsule and the utility of multimodal imaging.

Intraocular foreign bodies (FBs) are common ocular injuries reporting to the emergency services all over the world. The authors highlight the findings and surgical management of a case of intralenticular metallic FB following an injury while using chisel and hammer. The ocular path of the FB (2 mm) could be traced from a self-sealed corneal perforation, extending through the anterior capsule rupture, terminating at the posterior capsule, forming a posterior capsule tent with a part embedded in clear lens. Preoperative ultrasound biomicroscopy gave clues on posterior capsule integrity and the e

Autoimmune polyglandular syndrome type 1: a case report and brief review.

Autoimmune polyglandular syndrome (APS) type 1 is a rare autoimmune disorder inherited in an autosomal recessive pattern due to loss of function of the AIRE gene and defective removal of self-reactive T-lymphocytes during the process of thymic T cell maturation. Its manifestation starts early in life with the cardinal clinical disorders being one of muco-cutaneous candidiasis, Addison's disease, and hypoparathyroidism. Recognizing the syndromic nature of one autoimmune disease will facilitate an active search for other conditions which would allow early detection, management, follow-up, and mo

Vulvar diseases: Approach to the patient.

Patients with vulvar dermatoses often delay seeking medical treatment because of anxiety and embarrassment. Moreover, women frequently self-treat with various home remedies and see multiple clinicians before presenting to a dermatologist. Despite serving as the primary providers for patients with vulvovaginal symptoms, gynecologists typically receive limited training in the causes and management of these conditions. Dermatologists are experts in the evaluation and management of cutaneous disease and should be the caretakers of all skin, including the genitalia. Vulvar disorders are underrecogn
