[Craniofacial surgery].

Craniofacial surgery is the term given to surgical techniques which allow combined access to the neurocranium and the facial skeleton. These techniques are applied primarily for treatment of fractures of the frontal/nasal/orbital regions, tumours of the orbits and the anterior cranial base, and congenital malformations, including simple or complex craniofacial synostoses, e.g. Crouzon and Apert syndromes. Indications for surgical treatment and the timing of the surgery are discussed. In addition, the most common surgical techniques are described and some clinical cases are presented.

An unusual cause of benign paroxysmal positional vertigo.

Benign paroxysmal positional vertigo (BPPV) is a self-limiting condition characterized by vertigo and nystagmus induced by certain head positions. The most common causes of BPPV are post-traumatic following head injury and post-viral labyrinthitis. We present an interesting case of BPPV following an otherwise uneventful neurosurgical removal of a parietal osteoma using hammer and chisel. Caution should be exercised during such procedures as disabling vertigo can result for a considerable period of time.

View from beneath: pathology in focus. ENT presentation of Ollier's disease.

Cartilaginous tumours occurring in the head and neck are usually regarded and seen as solitary lesions. However, we present an unique case where the cartilaginous tumour in the ENT region was the presenting feature of Ollier's disease (multiple enchondromatosis). It is recommended that further investigations be carried out in patients with cartilaginous tumours presenting in the head and neck to exclude the possible presence of this unusual and interesting syndrome.

A well-preserved cranium from the Judith River Formation (Montana, USA) reveals the inner ear and neuroanatomy of a Campanian baenid turtle.

A cranium belonging to a baenid turtle was recently recovered from the lower half of the Judith River Formation, Montana. Badlands Dinosaur Museum (BDM) 004 is a well-preserved partial cranium that includes the posterior cranial vault, cranial base, and otic capsules. Based on diagnostic characters, the skull can be attributed to Plesiobaena antiqua, which has been previously reported from the Judith River Formation. It also shares with palatobaenines projecting posterior processes of the tubercula basioccipitale and a prominent condylus occipitalis with a deep central pit, demonstrating varia…

Combined endoscopic endonasal & transoral approach to transpatial lesion involving the infratemporal fossa and masticator space: A case study and literature review.

Transpatial skull base lesions involving the infratemporal fossa (ITF) are challenging due to the complex neurovascular structures of the region. Open approaches have traditionally been utilized to access these spaces. We present a 55-year-old woman presented with a mesenchymal mass involving the left ITF and masticator space. A combined endoscopic endonasal transpterygoid approach was performed followed by an endoscopic transoral-transmandibular corridor to access and resect the tumor. The post-operative course was unremarkable with no recurrence during her follow-up. Combined endoscopic appr…

Computed tomography quantitative analysis of cranial vault dysmorphology and severity of facial complex changes in posterior synostotic plagiocephaly patients.

BACKGROUND: Posterior synostotic plagiocephaly (PSP) impacts craniofacial skeleton. Study quantifies facial changes in children with PSP to … y detected by CVAI. Mandible reshapes early under the stress of altered biomechanical forces of the skull base while changes in the maxilla are secondary to the asymmetric growth of the mandible and occur only in severe cases.

Variability in the Arterial Supply of Intracranial Meningiomas: An Anatomic Study.

BACKGROUND AND OBJECTIVES: Intracranial meningiomas are a diverse group of tumors, which vary by grade, genetic composition, location, and vasculature. Expanding the understanding of the supply of skull base (SBMs) and non-skull base meningiomas (NSBMs) will serve to further inform resection strategies. We sought to delineate the vascular supply of a series of intracranial meningiomas by tumor location. METHODS: A retrospective study of intracranial meningiomas that were studied using preoperative digital subtraction angiograms before surgical resection at a tertiary referral center was perfo…

OBJECTIVE: Severe craniofacial fractures may present different needs in treating intracranial lesions over facial injuries. This paper exami … ntracranial clots or penetrating wounds were excluded. In all cases, all or almost all the anterior skull-base was injured with compound fractures of the frontal sinus, the orbital roofs, the lamina cribrosa, and the planum sphenoidale. Conventional posterior cranial vault expansion: indications and results-review of the literature.

In the late 1980s, craniofacial surgery units reported suboptimal cosmetic results, cranial volume restriction, and intracranial hypertension after anterior cranial vault remodeling in bilateral coronal synostosis associated with severe brachyturricephaly. A possible explanation was a severe associated growth restriction of the posterior calvaria with radiological synostosis at the lambda sutures. "Conventional" or "fixed" posterior cranial vault expansion techniques were developed to address these limitations, sometimes as the first surgical step in a two-staged protocol of total calvarial re

Effects of cranial integration on hominid endocranial shape.

Because brains do not fossilize, the internal surface of the braincase (endocast) serves as an important source of information about brain growth, development, and evolution. Recent studies of endocranial morphology and development in great apes, fossil hominins, and modern humans have revealed taxon-specific differences. However, it remains to be investigated to which extent differences in endocranial morphology reflect differences in actual brain morphology and development, and to which extent they reflect different interactions of the brain and its case with the cranial base and face. Here

Fracture line in post head injury optic nerve damage.

Diminution of vision following head injuries involving the optic canal is not uncommon. In order to find out how a blunt trauma to the frontotemporal region produced damage to the optic nerve in its canalicular portion, this study was conducted on cadaveric skulls and its results are described.

Adaptive Positioning and Targeting Optimization in Transcranial Focused Ultrasound (tFUS): A Numerical Study.

OBJECTIVE: Transcranial focused ultrasound (tFUS) has garnered significant attention owing to its capacity for precise depth penetration and high spatial resolution within tissues. However, the pronounced heterogeneity of the skull presents substantial challenges to precise targeting, limiting optimal therapeutic efficacy. Precise positioning is urgently needed. METHODS: A homogeneous spherical model was employed as the attenuation reference. In the skull model, the transducer operating matrix was determined using a radius positioning method, and an acceleration factor was introduced to exclu…

The role of traction in the treatment of severe idiopathic scoliosis: non-traction vs. traction.

BACKGROUND: Treatment planning for severe idiopathic scoliosis (SIS) requires consideration of both deformity correction and the appropriate grade of osteotomy. This study aimed to compare radiographic and clinical outcomes between patients who underwent surgical correction with or without preoperative skull-femoral traction. METHODS: We retrospectively reviewed patients with SIS who had a main thoracic curve greater than 90°and flexibility less than 30% and underwent surgery at our institution between 2014 and 2018. Patients were grouped according to whether they received preoperative skull-…

Sphenoid Temporal Meningioma With Granular Cell Features and Extracranial Infratemporal Extension.

Meningiomas usually arise from arachnoid cap cells and present as intracranial, dural-based lesions. Extracranial extension into the temporalis muscle, infratemporal fossa, orbit, and skull base foramina is uncommon and can mimic primary soft-tissue or peripheral nerve sheath tumors. We report a 54-year-old female who presented with a slowly progressive right-sided facial and temporal swelling over two years, associated with intermittent pain and mild proptosis but no focal neurological deficit. Computed tomography demonstrated a small extra-axial minimally enhancing right anterior temporal le…
