Psychosocial recovery, pain, and itch after burn injuries.

Burn patients face complex rehabilitation issues including physical, emotional, social, and vocational challenges. Problems with anxiety, depression, sleep, pruritis, and body image can affect the burn patient's ability to return to an acceptable quality of life. Burn patients require not only help in the hospital setting but also expert care as an outpatient from vocational counselors, social workers, physical and occupational therapists, psychologists, professionally monitored support groups, and peer counselors. This article reviews the nonpharmacologic treatments available and discusses th

[Clinical analysis of paroxysmal motor phenomena in the neonate].

The newborn infant is prone to motor phenomena of various physiological mechanisms and pathological significance whereas they can share close clinical patterns. A detailed clinical analysis, that should be supported by a video EEG recording, is necessary. That may help to distinguish myoclonus, jitteriness or seizures. Some rare phenomenom such as hyperekplexia or myotonia have also to be known. The pregnancy and birth history, the clinical examination and the search for association of various motor phenomena give essential clues for the diagnosis. Misdiagnosing non epileptic phenomona as seiz

Sleep disturbance is associated with decreased daily activity and impaired nocturnal reduction of blood pressure in dementia patients.

Our aim was to investigate the relationships between sleep disturbance and activities of daily living (ADL) and 24-h blood pressure patterns in institutionalized dementia patients. Using 107 institutionalized dementia patients (32 males and 75 females, mean age 76.3 years), patients with a mini mental state examination (MMSE) score<24 were classified into four groups based on ADL (normal or declined) and nocturnal reduction in blood pressure (dipper or non-dipper). The sleep/wake state was visually monitored hourly for 14 consecutive days, and daytime and nighttime sleep ratios were determined

Concurrent associations among sleep problems, indicators of inadequate sleep, psychopathology, and shared risk factors in a population-based sample of healthy Ontario children.

OBJECTIVES: Examine the contribution of sleep problems and indicators of inadequate sleep to psychopathology among children after accounting for shared risk and comorbid psychopathology. METHODS: Secondary analyses of cross-sectional data on 4- to 11-year-old (N = 1,550) children without chronic illness or developmental delay or disability. Parents provided information about sleep problems, indicators of inadequate sleep, symptoms of psychopathology, and risk factors for psychopathology. Teachers provided information about indicators of inadequate sleep and symptoms of psychopathology. RESUL

No refuge: an exploratory survey of nightmares, dreams, and sleep patterns in women dealing with relationship violence.

Sera of 624 blood donors were evaluated to determine seroprevalence of West Nile virus (WNV) antibodies following the 2003 WNV epidemic in Nebraska. Geographic factors contributing to differences in WNV seropositivity were evaluated. The overall prevalence of WNV in Nebraska was higher than reported previously in other U.S. locations (9.5% WNV immunoglobulin G seroprevalence rate), with the highest prevalence identified in the western part of the state (19.7%), followed by the central (13.8%) and the eastern (4.2%) parts. Regions of the state with the highest WNV-positive mosquito rates correl

Sleep and aggression in substance-abusing adolescents: results from an integrative behavioral sleep-treatment pilot program.

STUDY OBJECTIVE: To examine whether change in total sleep time during an integrative, behavioral sleep intervention is associated with aggression. Specifically, we tested whether adolescents who reported experiencing aggressive thoughts or actions after treatment had worse treatment trajectories (e.g., less total sleep time across treatment) than adolescents with no aggressive thoughts or actions after treatment. DESIGN: Nonpharmacologic open trial with 9 weeks of weekly assessment. SETTING: University of Arizona Sleep Research Laboratory PATIENTS OR PARTICIPANTS: Twenty-three adolescents r

Allergic rhinitis in children with attention-deficit/hyperactivity disorder.

BACKGROUND: Both allergic rhinitis and attention-deficit/hyperactivity disorder (ADHD) are common pediatric conditions associated with learning difficulties and sleep disturbances. There are conflicting research data regarding the association between ADHD and atopic disorders. OBJECTIVE: To determine the prevalence of allergic rhinitis in patients with physician-diagnosed ADHD. METHODS: Patients 5 to 18 years of age who presented with a Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition diagnosis of ADHD to an outpatient pediatric psychiatry clinic were screened for allerg

Critical incident exposure and sleep quality in police officers.

OBJECTIVE: Police officers face many stressors that may negatively impact sleep quality. This study compares subjective sleep quality in police officers with that in control subjects not involved in police or emergency services. We examined the effects of critical incident exposure (trauma exposure) and routine (nontraumatic) work environment stressors on sleep quality after controlling for the effects of work shift schedule. METHODS: Subjective sleep disturbances were measured by the Pittsburgh Sleep Quality Index in police officers (variable-shift workers, N = 551; stable day-shift workers,

Erectile dysfunction and symptoms of sleep disorders.

STUDY OBJECTIVES: Sleep apnea often is associated with impotence and/or erectile dysfunction (ED). The purpose of this study was to test whether a presentation for ED confers a pretest probability for obstructive sleep apnea hypopnea syndrome (OSAHS). DESIGN: A self-report survey for sleep complaints was conducted in consecutive male patients (>16 years of age) presenting to a urologic practice site. SETTING: The survey was provided to new and follow-up out-patients over a 2 month period of time. Other information collected included the presenting complaint, the clinical diagnosis for ED, an

[Neuropsychiatric disorders induced by MDMA ("Ecstasy")].

If neurotoxicity of MDMA (ecstasy) is now well documented in animals, it is not the same in humans. MDMA intoxication puts the problem of its possible link with the serotonin syndrome and the neuroleptic malignant syndrome. Neuropathological consequences following MDMA intake have been reported, including hemorrhaging and cerebral infarction, cerebral venous sinus thrombosis, and acute inflammatory CNS disease. However, the physiopathology of these complications remains unclear. In the same way, there have been various reports that have attributed MDMA to precipitating the onset of a wide rang

Neurosarcoidosis associated with hypersomnolence treated with corticosteroids and brain irradiation.

Narcoleptic features developed in a young man with CNS sarcoidosis. This was associated with a structural lesion in the hypothalamus as demonstrated on CT scans of the head. The diagnosis of narcolepsy was established by compatible clinical history and the Multiple Sleep Latency Test. Treatment with high-dose corticosteroids was ineffective, but when the low-dose, whole-brain irradiation was added, complete resolution of the narcoleptic features ensued.

A controlled study of Tourette syndrome. VI. Early development, sleep problems, allergies, and handedness.

Developmental milestones, problems with bladder and bowel control, sleep disturbances, allergies, and handedness were compared in 247 consecutive Tourette syndrome (TS) patients, 17 patients with attention-deficit disorder (ADD), 15 patients with ADD secondary to TS (ADD 2(0) TS), and 47 random controls. There were no significant differences in age of first talking or walking. By contrast, there were significant differences in problems with bladder and bowel control between TS patients and controls, as measured by age of first toilet training, age of last bed-wetting, frequency of enuresis, an

The "chubby puffer" syndrome. Upper airway obstruction and obesity, with intermittent somnolence and cardiorespiratory embarrassment.

Upper airway obstruction and obesity can produce cardiopulmonary distress in susceptible children. Three such children with chronic airway obstruction due to hypertrophied tonsillar and adenoidal tissue developed pronounced obesity, which in turn helped to provoke sleep alterations, cardiac signs, and abnormal arterial blood gas values. After tonsillectomy and adenoidectomy the symptoms were relieved.
