Symphysial osteomyelitis has been distinguished from osteitis pubis because of the more serious nature of the disease. We report a case in which there was a pelvic separation similar to that seen after trauma or pregnancy. The previously undescribed complications of bladder perforation and pelvic instability are also noted. There was no predisposing cause in this case, in contrast to the 40 previously reported. The causative organism was staphylococcus aureus, but pseudomonas aeruginosa and escherichia coli have also been found in other cases.

The case histories of 3 elderly patients who developed haemarthrosis of osteoarthritic joints with subsequent infection with staphylococcus aureus, are described. Trauma to the affected joints was a predisposing factor in 2 patients and, while only one patient developed clinical signs of sepsis, all had marked elevation of the erythrocyte sedimentation rate. Although suspicion of joint sepsis was obscured by the presence of a coexistent haemarthrosis, routine culture of joint aspirates showed infection with staphylococcus aureus and all patients recovered well with antibacterial therapy.

Staphylococcus epidermidis, an organism routinely found on the skin and in the hospital environment, has become a primary pathogen in infections associated with prosthetic devices. Because these infections are indolent and often clinically silent, diagnosis and therapy are often difficult. Pathogens are often misidentified as contaminants. The diagnosis of staphylococcal scalded skin syndrome is usually made upon the bases of characteristic skin lesions and the isolation of toxin-producing staphylococci. However, these criteria are not always reliable. An adult patient with characteristic skin lesions was colonized with phage group 2 staphylococci. The patient was subsequently found to have a drug-induced toxic epidermal necrolysis. The criteria necessary for the diagnosis of staphylococcal scalded skin syndrome are reviewed.

The process of neutrophilic polymorphonuclear leukocyte (PMN) migration in the bovine mammary gland during experimental Staphylococcus aureus challenge was studied using light and electron microscopies. The PMN were observed in various stages of margination in capillary lumens and migration into perivascular spaces. One hundred and five patients were given total parenteral nutrition (TPN) for an average of 15 days via centrally placed venous catheters. Thirty-seven polyvinyl chloride (PVC) and 80 silicone rubber catheters were used. All the silicone rubber and 8 of the PVC catheters were buried in a subcutaneous tunnel. When silicone rubber catheters were used, there was significantly less catheter sepsis (P much less than 0.01), catheter life was prolonged by 50 per cent and fewer catheter insertions per patient were necessary.

Two cases are reported of toxic shock syndrome in patients with pyogenic arthritis due to Staph. aureus with proven production of enterotoxin F and pyogenic exotoxin C. An ischemic complication of both lower extremities in one of the patients may be related to the use of a heparin-dihydroergotamine combination for postoperative prophylaxis of deep vein thrombosis.
