The authors report 19 cases of congenital ring-constriction syndrome of the limbs. Complications arose in eight of the pregnancies. The limb anomalies were always congenital ring-constrictions together with distal amputation. Moreover, 13 patients also had syndactyly and 15 associated anomalies were registered in 9 patients--8 of the limbs, 3 craniofacial and 4 visceral. One patient had a family history of hypospadias and cardiac malformation. The initial stages in the rehabilitation of a male child with severe microblepharon, corneal opacities, bilateral facial clefts, bilateral complete cleft lip and palate, and unilateral syndactyly are described. Review of the literature suggests that severe microblepharon is associated with other craniofacial anomalies, and often the child is stillborn or retarded. Bilateral facial reconstruction using an omental free-flap transfer for soft-tissue augmentation was performed on a 23-year-old man who had undergone previous surgical resection of an embryonal-cell rhabdomyosarcoma followed by radiation and chemotherapy. Eight weeks following surgery, the patient presented with pain and edema from a foreign-body reaction documented by electron microscopy. This was successfully managed with a course of steroid administration. The patient has remained symptom-free for 5 years.

The authors report the case of a 46-year old woman who, 7 years after bilateral implantation of a silicone breast prosthesis, presented with a seropositive rheumatoid arthritis involving the shoulders, wrists, metacarpophalangeal and interphalangeal joints of both hands, as well as the knees and ankles. Six months after the onset of the arthritis, she developed a Raynaud's syndrome and a lacrymal and salivary dry syndrome with unilateral axillary lymphadenopathy. No regression followed the removal of the breast prostheses, one of which was found broken.

The authors present their trends in conservative clitoroplasty on the basis of 27 cases from January 1980 to August 1985. The technique described preserves as much of clitoral tissue as possible, and gives safe and very satisfying morpho-functional results regardless of the virilization degree. Preserved skin of clitoral shaft and of the foreskin is used to reconstruct labia minora. Erectile function is preserved by freeing and burying corpora cavernosa in the omolateral labial region. Clitoral glans, suitably trimmed when necessary, is preserved on its ventral muco-cutaneous pedicle.

The anatomy and the technique of raising the galeal frontalis myofascial flap is described. In 38 patients with various pathologies, congenital anomalies, tumors, and trauma, this technique was employed. There were no extradural infections. The applications and complications of the flap are discussed. It is highly recommended wherever there is a possibility of direct intracranial nasopharyngeal communication.

An infant girl manifesting bilateral cryptophthalmos with multiple associated malformations was followed from birth. The globes were almost covered by a fold of skin and the findings were interesting in regard to the etiology of cryptophthalmos. The other congenital deformities included scaphocephaly, somewhat low-set ears, accessory tragi, a high arched palate, and funnel chest. The child was operated on at the age of 4 months with relatively satisfactory results, although she died suddenly of acute heart failure 4 months postoperatively.

It is often stated that any injury severe enough to avulse both lids will inevitably destroy the globe. Two cases are presented in which the globe was uninjured but exposed, requiring immediate coverage. Local skin flaps placed over mobilized conjunctiva were used for emergency cover. One was eventually made into functioning eyelids; the other remains dormant. First-line options for local flaps may not be available because of regional trauma. In the course of the past nine years, 68 patients (42 boys, 26 girls) were surgically treated for syndactylism in 84 hands. In doing so, 135 syndactylies occurring mostly in the form of severe malformations of the hand were separated. In most cases the web spaces were made according to the surgical techniques developed by Blauth. High septal hemitransfixion has demonstrated superior versatility and reliability in a series of 68 patients for correction of caudal septal deformity. The literature has contained many references to high septal transfixion for use in rhinoplasty to preserve tip support. An alternative application of this approach for repair of caudal septal deformity is herein discussed.

A 14-year-old boy presented with a Grade II left cheek fistula and inability to open the mouth following gangrenous stomatitis in early childhood. The treatment of the patient by multistaged reconstruction of the cheek defect is discussed. A large turnover flap was used for the lining, a technique which obviates the need for a second lining flap, and thus eliminates the disfigurement of a donor site. Also, it uses the healthy skin that would otherwise have been discarded in the creation of mucosal flaps.

Blepharochalasis is a distinct entity of redundant eyelids. It is characterized by a history of recurrent edema that leads to permanent changes in the eyelids. Even in the young, blepharoplasty may be indicated for psychological reasons or to relieve obstruction of vision, eyelid asthenia, and fatigue. Following surgery, recurrent edema may compromise the results or cause further eyelid changes. Effort should be made to define the etiology of the edema and correct the defect if possible.

A wide variety of deformities are seen by the craniofacial team. To facilitate the coding and retrieval of data collected, a system has been devised in which an attempt is made to transfer different clinical problems into units which can be processed more efficiently and encoded more exactly. The different surgical manipulations used form the basis of this system. Careful forethought and understanding are prerequisite for the utilization of this system by the craniofacial team.

The existing tissues are preserved and utilised as far as possible to reconstruct a vaginal opening. Secondary cleft lip-palate deformities frequently involve a combination of skeletal, cartilagenous, dental and muscular deformities. Correction of these deformities should be based on normalization of function by restoring normal anatomy as much as possible. Functional correction of the lip musculature is best done at the same time as surgical correction of the skeletal-dental deformities. In this article, we outline a fundamental technique for secondary lip-nose correction based on muscle reorientation and a concomitant Le Fort I maxillary osteotomy.
