A man aged 65 and a woman aged 55 were referred to the orthopaedic clinic because of persistent pain in the right shoulder following a fall, 5 and 16 months earlier respectively. On physical examination they were found to have a painful shoulder and loss of strength on endorotation of the right arm. Extensor mechanism disruptions are relatively uncommon knee injuries. Within this group, delayed treatment is an infrequent but difficult clinical situation. Fibrous degeneration, muscle contraction and subsequent hiatus present a challenge to the orthopaedic surgeon. We present an unusual case of a successful delayed repair using the Mitek anchors system (Mitek Products Inc., a division of Ethicon Inc., Westwood, Massachusetts).

PURPOSE: Ruptures of the finger flexor pulleys are the most frequent injuries in rock climbers. Whereas multiple pulley injuries demand a surgical reconstruction, single ruptures are mainly treated conservatively. Nevertheless, the question of the clinical outcome or a persisting finger strength deficit after conservative therapy arises.

A left gastrocnemius strain was sustained by an elite cricket batsman while he was taking off to run. The exact moment of injury, captured by a camera in the middle stump, appears to correspond to the sudden appearance of a deficit in the gastrocnemius muscle, seen through the player's trousers. The strain occurred when the entire body weight was on the left foot with the centre of mass well in front of the leg. The injury probably occurred close to the time when the gastrocnemius complex was moving from an eccentric to an isometric phase.

We report a patient with rupture of the ring extensor tendon associated with calcium pyrophosphate dihydrate crystal deposition disease of the wrist. Crystal deposits were noted in synovium and the triangular fibrocartilage complex. Histological examination revealed chronic synovitis with foreign body giant cell reaction to crystals. The cause of the tendon rupture was synovitis due to crystal deposition.

A variety of disorders primarily involving the long head of the biceps tendon is discussed. By virtue of its particular intra-articular position, this tendon may be affected by primary articular and degenerative processes because of impingement by the coracoacromial arch. Disorders of the biceps, including traumatic tears and displacement out of the bicipital groove, are best evaluated by MR imaging.

Thirty percent to 50% of clubfeet will not respond to conservative treatment and will require surgery. The average failure rate in clubfoot surgery is 25% (range, 13% to 50%). Many complications can occur during surgery and postoperatively. Preoperative planning of the appropriate skin incision and special attention to wound closure may prevent some of these complications. Awareness of the multiple complications that can occur such as cavus, persistent adductus, overcorrection of the hindfoot, and recurrent deformity allows the surgeon to prepare and plan for these often unavoidable problems.

The authors report a series of 38 patients who had been examined by MRI and then operated for a rotator cuff syndrome. The correlation between the description of the cuff lesions after MRI and the surgical observations were excellent for 37 patients. To compare the functional results of early controlled mobilisation and static immobilisation following repair of extensor tendons, we conducted a comparative study between two centres. Reconstruction of the ulnar collateral ligament using a free tendon graft was performed on sixteen athletes. All participated in sports that involved throwing (mostly professional baseball), and all had valgus instability of the elbow. The Blackburne and Peel method of assessing the position of the patella was applied to 185 knees with Osgood-Schlatter disease in 125 patients. The normal index of 0.80 was confirmed in 73 controlled knees. The average index in the knees with Osgood-Schlatter disease measured 1.01 (patella alta) in boys and 0.91 (normal) in girls. Description of two cases of badly injuried hands, where the grasp-function has been restored. In one case, the traumatically amputated thumb was reconstructed in the conventional way, transposing the index finger on its neurovascular bundle. The other case demonstrates the use of modern microsurgical methods in substituting a freely transplanted toe with microvascular anastomoses for four lost fingers. Resulting function was satisfactory in both cases.

We treated twenty six patients with thirty four partial tendon lacerations by not suturing the tendons and by early mobilization of the digit. These partial tendon lacerations varied from 25 to 95 per cent of the cross sectional area. The mean and median cross-sectional area lacerated was 60 per cent. Twenty three of these patients obtained excellent function, one patient obtained good to excellent function and one patient obtained fair function. One patient was lost to follow-up.
