Brainstem dysfunction in alternating hemiplegia of childhood: a neurophysiological study.

Six patients with alternating hemiplegia of childhood (AHC) underwent motor evoked potentials (MEPs), somatosensory evoked potentials (SEPs) and blink reflex recording. No SEP abnormality was found. As for MEP recording, central conduction time did not differ between patients studied during either interictal or ictal phase, and normal subjects, suggesting that the pyramidal system function is spared. In blink reflex recording, the latencies of both the ipsilateral (iR2) and contralateral (cR2) R2 components were significantly longer (P<0.01) in patients during the interictal phase than in norm

Anodal HD-tDCS of vmPFC facilitates learning of monosemous and polysemous words: A computational modeling study.

Vocabulary learning requires learners to establish stable mappings between symbols and meanings. Reinforcement learning (RL) principles, through feedback-based value computation, provide a compelling framework supporting such mappings, with the ventromedial prefrontal cortex (vmPFC) playing a key role in value-based learning and decision-making. In this study, we combined artificial pseudowords with a probabilistic reversal learning paradigm and applied high-definition transcranial direct current stimulation (HD-tDCS) in the vmPFC to investigate its role in vocabulary acquisition. Brain stimulation prevents neural downregulation and optimizes learning.

Non-invasive brain stimulation, such as transcranial random noise stimulation (tRNS), has been shown to enhance cortical excitability and facilitate perceptual learning. However, the neural mechanisms underlying these effects remain poorly understood. Here, we demonstrate that tRNS over the bilateral intraparietal sulcus (IPS) optimizes learning by preventing the decline in neural activity that occurs during short, high-load attentional training, thereby sustaining excitability and enhancing behavioral performance. Using a multi-session tRNS-fMRI paradigm, we investigated how tRNS modulates le

Continuous theta burst stimulation over the dorsolateral prefrontal cortex improves force steadiness during submaximal contractions without visual feedback.

BACKGROUND: Without visual feedback, the central nervous system must rely on the integration of internal models and proprioceptive information. While the dorsolateral prefrontal cortex (DLPFC) is involved in motor control and sensorimotor monitoring, whether it causally influences force steadiness and whether any such influence is dependent on corticospinal excitability remains unclear. OBJECTIVE: This study aimed to investigate whether modulating DLPFC activity using theta burst transcranial stimulation (TBS) influences force steadiness during submaximal contractions performed without visual

Exploring interneuron conduction time in the motor cortex and the link to motor function in individuals with chronic stroke.

Interneurons in the motor cortex (M1) contribute to key motor functions, which are often altered after stroke. Changing the direction of transcranial magnetic stimulation (TMS) alters the onset time of motor evoked potentials (MEPs). Yet, how stroke changes motor evoked potential (MEP) onset latency in association with directional TMS is not understood. The goals of the current study were to 1) investigate directional MEP onset in the contralesional and ipsilesional hemispheres and 2) characterize the relationship between MEP onset latency and motor function poststroke. Twenty individuals with…

Dynamic cortical inhibition imbalance as a biomarker of clinical progression in early Parkinson's disease.

OBJECTIVE: The role of bilateral cortical inhibition in motor compensation during the progression of early Parkinson's disease (PD) remains unclear. This study aims to investigate the dynamic balance of between-hemispheric inhibition and the clinical relevance of its neurophysiological indicators. METHODS: Paired-pulse TMS was used to investigate the activity of different intracortical inhibitory circuits in 90 PD patients and 43 healthy controls during the "on medication" state. Spearman's rank correlations and general linear model (GLM) were used to analyze the association between TMS measu

Asymmetric Modulation of Brain Connectivity by Anodal Transcranial Direct Current Stimulation in Healthy Individuals: A Single-Blind, Randomized Sham-Controlled Trial.

Transcranial direct current stimulation (tDCS) applied to the dorsolateral prefrontal cortex (DLPFC) has shown asymmetric behavioral effects, though the underlying neurophysiological mechanisms remain unclear. In this preliminary study with 34 healthy individuals, tDCS was applied to either the left or right DLPFC or a sham group. Behavioral and neurophysiological changes were examined by the Stroop test and resting-state fMRI, respectively, which were measured before and after a 15-min tDCS session. Seed-to-voxel connectivity analysis with seeds placed under the tDCS target regions (F3 and F4

Study on the role and mechanism of intermittent theta burst stimulation (iTBS) induced N-acetylaspartylglutamate (NAAG) in suppressing ferroptosis in ischemic stroke.

This study investigates the mechanism through which intermittent theta burst stimulation (iTBS) inhibits ferroptosis in ischemic stroke by inducing N-acetylaspartylglutamate (NAAG). In SD rats subjected to middle cerebral artery occlusion (MCAO), iTBS treatment significantly decreased ischemia-reperfusion (I/R) injury, improving motor, coordination, spatial memory abilities and Cognitive Impairment by enhancing synaptic function and neuronal repair. Western blot analysis showed that in MCAO rats treated with iTBS, GPX4 protein expression increased, while ACSL4, TFRC, and DMT1 protein levels de

Effects of high-definition tDCS targeting individual motor hotspot with EMG-driven robotic hand training on upper extremity motor function: a pilot randomized controlled trial.

BACKGROUND: Delivering HD-tDCS on individual motor hotspot with optimal electric fields could overcome challenges of stroke heterogeneity, potentially facilitating neural activation and improving motor function for stroke survivors. However, the intervention effect of this personalized HD-tDCS has not been explored on post-stroke motor recovery. In this study, we aim to evaluate whether targeting individual motor hotspot with HD-tDCS followed by EMG-driven robotic hand training could further facilitate the upper extremity motor function for chronic stroke survivors. METHODS: In this pilot ran

Network-level mechanisms underlying effects of transcranial direct current stimulation (tDCS) on visuomotor learning.

Transcranial direct current stimulation (tDCS) is a non-invasive brain stimulation approach in which low level currents are administered over the scalp to influence underlying brain function. Prevailing theories of tDCS focus on modulation of excitation-inhibition balance at the local stimulation location. However, network level effects are reported as well, and appear to depend upon differential underlying mechanisms. Here, we evaluated potential network-level effects of tDCS during the Serial Reaction Time Task (SRTT) using convergent EEG- and fMRI-based connectivity approaches. Motor learni

Theta burst stimulation in neglect after stroke: functional outcome and response variability origins.

Spatial neglect is a strong and negative predictor of general functional outcome after stroke, and its therapy remains a challenge. Whereas inhibitory non-invasive brain stimulation over the contralesional, intact hemisphere has generally been shown to ameliorate neglect on a group level, a conspicuous variability of the effects at the individual level is typically observed. We aimed to assess the characteristics and determinants of the effects of inhibitory non-invasive brain stimulation in neglect, identifying which patients would respond to this therapeutic approach and which not. To this e

The effect of neurostimulation applied to the left dorsolateral prefrontal cortex on post-stress adaptation as a function of depressive brooding.

Depressive brooding following a stressful event predicts negative affect and neuroendocrine responses related to psychological stress. The dorsolateral prefrontal cortex (DLPFC) has been associated with the top-down regulation of thoughts and emotions, and abnormal neural activity within this region has been associated with increased psychological stress and ruminative thinking. The aim of this study was to investigate whether the modulation of the DLPFC could have beneficial effects on ruminative thoughts and the endocrine response following a self-relevant stressor. Significant improvement in treatment resistant auditory verbal hallucinations after 5 days of double-blind, randomized, sham controlled, fronto-temporal, transcranial direct current stimulation (tDCS): A replication/extension study.

BACKGROUND: Transcranial direct current stimulation (tDCS) is a potentially novel treatment for antipsychotic-resistant auditory verbal hallucinations (AVH) in schizophrenia. Nevertheless, results have been mixed across studies. METHODS: 89 schizophrenia/schizoaffective subjects (active: 47; Sham: 42) were randomized to five days of twice-daily 20-min active tDCS vs. sham treatments across two recruitment sites. AVH severity was assessed using the Auditory Hallucination Rating Scale (AHRS) total score. To assess target engagement, MRI was obtained in a sub sample. RESULTS: We observed a stat

rTMS of the dorsomedial prefrontal cortex for major depression: safety, tolerability, effectiveness, and outcome predictors for 10 Hz versus intermittent theta-burst stimulation.

BACKGROUND: Conventional rTMS protocols for major depression commonly employ stimulation sessions lasting >30 min. However, recent studies have sought to improve costs, capacities, and outcomes by employing briefer protocols such as theta burst stimulation (iTBS). OBJECTIVE: To compare safety, effectiveness, and outcome predictors for DMPFC-rTMS with 10 Hz (30 min) versus iTBS (6 min) protocols, in a large, naturalistic, retrospective case series. METHODS: A chart review identified 185 patients with a medication-resistant major depressive episode who underwent 20-30 sessions of DMPFC-rTMS (1

Cortical excitability in chronic migraine.

A proportion of episodic migraine patients experiences a progressive increase in attack frequency leading to chronic migraine (CM). The most frequent external factor that leads to headache chronification is medication overuse. The neurobiological bases of headache chronification and of the vicious circle of medication overconsumption are not completely elucidated. More recently, the same neurophysiological methods used to study episodic migraine were applied to CM and medication-overuse headache (MOH). Studies of cortical responsivity tend overall to indicate an increase in excitability, in pa

Magnetically evoked inter-enlargement response: an assessment of ascending propriospinal fibers following spinal cord injury.

The aim of the present study was to characterize a novel electrophysiological assessment, the magnetically evoked interenlargement response (MIER), by defining the anatomical location of the fast conducting (large myelinated) ascending axons that mediate the response and the relationship between the response and locomotor function following experimental spinal cord injury. Electromyographic (EMG) responses were recorded from the triceps muscles following magnetic stimulation of one hip. Short-latency (approximately 6 ms) EMGs were recorded from triceps muscles in normal controls and following

Transcranial direct current stimulation improves reduced global BOLD-CSF coupling in patients with insomnia disorder and comorbid anxiety: a resting-state functional MRI study.

BACKGROUND: Insomnia disorder with comorbid anxiety is a prevalent clinical challenge. Although transcranial direct current stimulation (tDCS) is a promising noninvasive neuromodulatory approach, its effects on glymphatic-related neurovascular-cerebrospinal fluid (CSF) coupling in insomnia remain unclear. This study investigated the effects of tDCS on global blood oxygen level-dependent (gBOLD)-CSF coupling using resting-state functional magnetic resonance imaging (rs-fMRI) in patients with insomnia disorder and comorbid anxiety. METHODS: We conducted a 2-week, double-blind, randomized, sham-

Accelerated Intermittent Theta-Burst Stimulation as a Treatment for Cocaine Use Disorder: A Proof-of-Concept Study.

There are no effective treatments for cocaine use disorder (CUD), a chronic, relapsing brain disease characterized by dysregulated circuits related to cue reactivity, reward processing, response inhibition, and executive control. Transcranial magnetic stimulation (TMS) has the potential to modulate circuits and networks implicated in neuropsychiatric disorders, including addiction. Although acute applications of TMS have reduced craving in urine-negative cocaine users, the tolerability and safety of administering accelerated TMS to cocaine-positive individuals is unknown. As such, we performed…
