OBJECTIVE: Although olfaction is often compromised by such factors as head trauma, viruses, and toxic agents, the olfactory epithelium and sectors of the olfactory bulb have the potential for regeneration. This study assessed the degree to which olfactory function changes over time in patients presenting to a university-based smell and taste center with complaints of olfactory dysfunction and the influences of etiology (eg, head trauma, upper respiratory infection), sex, age, smoking behavior, degree of initial dysfunction, and other factors on such change.
METHODS: Well-validated odor identi

OBJECTIVE: Imaging of the head is expensive and can be stressful for children, some of whom need anesthesia for the procedure. The aim of this study was to determine which vertiginous children benefit most from head imaging.
METHODS: We conducted a retrospective chart review of all children aged under 18 years who were referred to the Helsinki University Children's Hospital Radiology Department (tertiary referral center) for head computerized tomography (CT) or magnetic resonance imaging (MRI) over a 1-year period. We analyzed and reviewed the medical records of 87 children who had undergone

Acute acalculous cholecystitis remains a diagnostic challenge in critically ill trauma patients. Laboratory studies are nonspecific and associated injuries or mental status changes may mask clinical signs and symptoms. We conducted a retrospective study to assess the utility of ultrasound in the diagnosis of acute acalculous cholecystitis. We hypothesized that ultrasound is inadequate as a screening tool for acute acalculous cholecystitis. The abdominal ultrasounds of all patients undergoing evaluation for acute acalculous cholecystitis in a 40-month period at our Level I trauma center were re

OBJECTIVE: To identify variables that increase the chance of neonatal scalp injury during vacuum extraction.
METHODS: We conducted a prospective observational study of 134 vacuum extraction-assisted deliveries at Olive View-UCLA Medical Center in 1995. Data collected included parity, gestational age, duration of first and second stages of labor, indication for operative delivery, station and position of fetal head, duration of vacuum application, number of "pop-offs," neonatal weight, and descriptions of scalp marks or injury. Cranial imaging studies were obtained if clinically indicated.

The relationship between alcohol use and injury severity was investigated in trauma patients admitted to a tertiary referral hospital during a 23-month period. Admission blood alcohol levels (BALs) were obtained on 427 trauma patients, who were stratified into three groups: those with no measurable blood alcohol, those within the legal limit of 100 mg/dL, and those over the legal limit or intoxicated. The no-alcohol group had significantly lower injury severity than the other two groups (p less than 0.001). Even when the BAL was well within the legal limit, injuries suffered by those in the al

Preoperative diagnosis of diaphragmatic rupture caused by blunt injury is often difficult because of serious concurrent injuries, a lack of specific clinical signs, and simultaneous lung disease that may mask or mimic the diagnosis radiologically. Previous reports have suggested that a preoperative diagnosis is established on the basis of chest radiographs in only one third of patients. In order to assess the value of chest radiographs and other imaging techniques in diagnosing traumatic rupture of the diaphragm, we retrospectively reviewed all preoperative diagnostic imaging performed in 50 p

Over a 26-month period, 25 patients admitted to the Trauma Unit at UCSD Medical Center following blunt trauma were investigated for suspected traumatic rupture of the thoracic aorta by computed tomography (CT) of the chest. A retrospective review of these patients was performed. Twenty-one (84%) also had CT of other body areas, most commonly the head or abdomen. Nine of the 25 patients subsequently had aortography; in 15 patients the CT findings were felt at the time to exclude rupture, and one patient was not investigated further because of severe head injuries. In general, if CT failed to sh

One hundred consecutive patients with injuries to the extraperitoneal rectum were treated over a ten-year period at an urban trauma center. The mechanisms of injury included firearms in 82 patients, stab wounds in 3 patients, a variety of other penetrating injuries in 10 patients, and in 5 patients the injuries resulted from blunt trauma. Treatment of the rectal injury was determined by the bias of the operating surgeon, the condition of the patient, and the magnitude of the rectal injury. Proximal loop colostomies were performed in 44 patients, diverting colostomies in 51 patients, Hartmann's

In the past two decades, 853 children and adolescents have been evaluated for elevated BP as inpatients and outpatients by the hypertension service at Children's Hospital Medical Center, Cincinnati. Most children with hypertension secondary to coarctation of the aorta and all children with glomerulonephritis are managed by other services and are not included in that total. In a retrospective study, 27 children (3%) were identified as having hypertension secondary to intrinsic renal arterial stenosis. The age at presentation ranged from 5 months to 20 years. The mean BP at that time was 171.6/1

No detailed reports on pneumocephalus caused by any factors other than head trauma, and their courses accompanied with this disease have been so far available. We recently experienced a case of pneumocephalus complicated with severe clouding of consciousness, ocular deviation, and unilateral spatial neglect with the results being reported hereinafter. A 54-year-old man had often received ear douche therapy due to chronic sinusitis and tubal obstruction about 3 years before without any history of head trauma. On Jan. 6, 1984, sudden clouding of consciousness accompanying stiffness of the left a

Nontraumatic spontaneous intracerebral hemorrhage in infants is rare and usually occurs due to infection, trauma, or known coagulopathy, most often originating from the thalamus and extending into the ventricle. In this case report, we describe a two-month-old late preterm female infant (36 weeks of gestational age) who was brought in due to irritability, projectile vomiting, and feeding difficulties. The clinical history suggested cow's milk protein allergy, as the baby had recently been switched to formula. The neurological signs observed during physical examination, including upward eye dev

Traumatic dental injuries (TDIs) can significantly compromise oral health-related quality of life. In younger patients, luxation injuries are associated with a higher risk of pulp necrosis. This case report describes the multidisciplinary management of a 9-year-old patient who presented with a rotated and fractured permanent maxillary right central incisor exhibiting pulp necrosis after dental trauma. An initial attempt at regenerative endodontic treatment was unsuccessful, requiring apexification. Orthodontic treatment was performed to achieve maxillary expansion and rotational alignment of t

Laryngotracheal trauma is a rare but life-threatening condition requiring prompt diagnosis and management to prevent airway compromise and long-term functional deficits. Therefore, it is of interest to evaluate the etiology, clinical presentation, management and outcomes of laryngotracheal injuries in 15 patients aged 15-60 years treated between 2021 and 2023 at a tertiary care center. Injury severity was graded using the Schaefer system and all patients underwent fibreoptic endoscopy and contrast-enhanced CT, with functional outcomes assessed using the Dysphonia Severity Index and New Zealand

Acute pediatric radiation injury is fortunately rarely encountered in pediatric emergency medicine and trauma. Most reported experience with the condition has arisen from unanticipated, untoward events associated with radiation therapy for pediatric oncologic illness, as scientific study of pediatric outcomes following major radiation and nuclear events, such as Hiroshima, Nagasaki, Chernobyl, and Fukushima, has focused on long term morbidity, mostly with respect to thyroiditis and thyroid neoplasia, instead of acute care-for which data cannot be found in the English language scientific litera

CASE: A 48-year-old man with multiple injuries sustained a Gustilo type 3A right open distal humerus fracture with a 4-cm wound, contralateral radius shaft, and ilium fractures. Following wound irrigation on the day of injury, second look debridement showed crushed triceps and severe contamination extending into the distal humerus medullary canal. After 6 days of initial plate fixation, he developed signs of deep infection. Radical debridement twice resulted in a 10-cm bone defect, necessitating reconstruction with a Lambda-shaped free vascularized fibula for both columns.
CONCLUSION: Lambda-

Different techniques for artificial iris implantation with or without an intraocular lens, depending on lens status, are described in the literature. We describe a surgical technique for a custom-made artificial iris and toric-intraocular lens intrascleral flange fixation. We modified the "Backpack" artificial iris implantation surgical technique to facilitate an accurate alignment of the toric-intraocular lens in a patient with aphakia, aniridia, and high asymmetric astigmatism secondary to blunt trauma. Two months after the surgery, uncorrected visual acuity was 20/30, corrected to 20/25 wit

A median nerve schwannoma is an uncommon type of tumor that develops from Schwann cells in the peripheral nerves. We present a case report of a young prisoner with a rare median nerve schwannoma presenting as a swelling on the anterior aspect of the right wrist along the radial edge, with associated paresthesia and a positive Tinel's sign. The case was diagnosed using MRI which showed the target sign of biphasic contrast enhancement in both the mass's center and periphery as well as distinct encapsulation. The mass was managed with surgical excision which confirmed the mass to be a neurilemmom

A 66-year-old man was reported to have persistent chest pain for 4 hours after accidentally swallowing a fishbone. An isolated esophageal foreign body (EFB) was suspected in the community hospital. In our center, an emergency chest CT scan revealed an EFB in the upper part of the esophagus of the patient which penetrated the left esophageal wall as well as the distal aortic arch. However, the experience of the treatment strategy for this lesion is still not enough available. Considering the surgical trauma and the risk associated with advanced age of the patient, the option for open surgery wa
