This paper explores population substructure through an analysis of 8,691 individual death certificates representing a native-born New York City (NYC) population and a European Immigrant population who died from tubercular infection in Manhattan, NYC, 1890-1930. From the death certificate data, four classes of tubercular infection were derived: pulmonary, chronic pulmonary, acute/miliary tuberculosis, and tubercular meningitis. The United Nations classification system was used to categorize the European regions of birth listed on the death certificates resulting in four international regions of

Specific T cell responses are central for protection against infection with M. tuberculosis. Here we show that mycobacteria-specific CD4 and CD8 T cells accumulated in the lung but not in the mediastinal lymph node (MLN) at different time points after M. tuberculosis infection or BCG immunization. Proliferating specific T cells were found in the lung after infection and immunization. Pulmonary, but not MLN-derived CD4 and CD8 T cells, from M. tuberculosis-infected mice secreted IFN-γ after stimulation with different mycobacterial peptides. Mycobacteria-specific resident memory CD4 and CD8 T ce

BACKGROUND: Safer, more effective treatment regimens for rifampicin-resistant tuberculosis are needed.
METHODS: We conducted a phase 3, open-label, pragmatic, randomized, controlled noninferiority trial in South Africa to assess a 6-month treatment strategy for pulmonary rifampicin-resistant tuberculosis. Participants with pulmonary rifampicin-resistant tuberculosis who were 6 years of age or older were randomly assigned to a regimen consisting of bedaquiline, linezolid, delamanid, and levofloxacin or clofazimine or both for 6 months (trial-strategy group) or the 9-month standard-of-care trea

Broncholithiasis is a rare respiratory condition usually caused by calcified lymph nodes eroding into the bronchial lumen, often following pulmonary tuberculosis or histoplasmosis infections. Patients may be asymptomatic or present with haemoptysis, recurrent respiratory infections or airway obstruction, for which bronchoscopic or surgical treatment may be required. No internationally standardized guidelines currently exist for broncholith removal. We report a 63-year-old woman with previous pulmonary tuberculosis, recurrent haemoptysis and thrombocytopenia that was subsequently diagnosed as i

Bronchial foreign body aspiration (BFBA) in adults is uncommon and frequently overlooked, particularly in tuberculosis-endemic settings where it may mimic asthma or pulmonary tuberculosis, leading to prolonged diagnostic delay. We report a 55-year-old woman with a two-year history of treatment-refractory wheezing and progressive dyspnea, repeatedly misdiagnosed as asthma. Chest computed tomography revealed a smooth intraluminal lesion within the right basal trunk bronchus with surrounding post-obstructive inflammatory changes but no distal atelectasis. Flexible bronchoscopy identified and succ

The coexistence of lophomoniasis and tuberculosis (TB), both airborne diseases, is relatively uncommon. Co-infections like these can complicate treatment strategies due to overlapping symptoms and potential drug interactions. We report a rare case of comorbidity involving two pulmonary diseases, lophomoniasis and TB, in an 82-year-old woman with Alzheimer's disease (AD) from northern Iran. Her primary symptoms included weakness, lethargy, dyspnea, sputum production, night sweats, and significant weight loss. Both TB and lophomoniasis can compromise the immune system, potentially worsening the

Amyloidosis is a rare disease. This paper reports a case of localized secondary hypopharyngeal amyloidosis presenting with pulmonary tuberculosis as the initial symptom. The patient lacked specific clinical manifestations and primarily exhibited symptoms such as cough, sputum production, acid reflux, belching, and abdominal pain. Chest CT indicated bronchiectasis with infection and pulmonary tuberculosis. Digestive endoscopy revealed a white mucosal elevation at the right pyriform sinus of the hypopharynx. Pathological diagnosis confirmed amyloid deposits in the hypopharyngeal mucosal tissue.

Role of transthoracic fine needle aspiration cytology (FNAC) in diagnosing 170 patients of non-resolving pneumonias was studied. There were 117 males and 35 females in the age range of 3 to 72 years. The neoplastic lesions diagnosed by FNAC included 51 primary lung cancers, 6 metastatic deposits, 1 malignant mesothelioma of pleura, 1 Hodgkin's disease and 4 thymomas. Benign lesions included 56 pneumonias and abscesses of bacterial and chemical origin, 35 pulmonary tuberculosis, 2 fungal granulomas and 1 sarcoid granuloma. Aspiration cytology was inconclusive in 13 cases. The sensitivity of FNA

Massive hemoptysis is a life-threatening condition which is defined by the American College of Radiology as more than 300-400 ml expectorated hemoptysis in 24 hours. The major causes of massive hemoptysis are benign diseases including tuberculosis, aspergillosis, and pulmonary abscess. Bronchoscopy is mandatory for the initial assessment of hemoptysis and several transbronchial interventions may be performed in some cases. In addition, arteriography is another important modality for hemoptysis especially for the patients with poor general condition that have contraindications for surgery. Beca

INTRODUCTION: The oral cavity is considered an easily accessible window to the body. The mouth is frequently involved in conditions that affect multiple organs. In many instances, oral involvement precedes the appearance of many other symptoms or lesions. A complete examination of the oral cavity provides a gateway for an accurate diagnosis and precise management of many systemic conditions. Gingival enlargement is one of the varied manifestations of many systemic diseases. Here, a case report of a gingival enlargement is presented that provided information for the diagnosis of post-primary pu

The emergence of multi-drug-resistant (MDR) strains has been a major obstacle in the tuberculosis (TB) control programme. In the present study we looked into the prevalence of MDR-TB in an urban tertiary care hospital in South India over four years (2007-2010). During this period, 641 clinical specimens (317 respiratory specimens and 324 non-respiratory specimens) were received for mycobacteriological culture and drug susceptibility testing for first-line drugs, using the BACTEC 460 TB system. Mycobacterium tuberculosis (MTB) was isolated in 34.8% (n = 223) specimens. Of the total 223 MTB isol

BACKGROUND AND OBJECTIVES: Rapid and accurate diagnosis of latent tuberculosis infection (LTBI) is crucial in military settings. This study aimed to detect the prevalence of LTBI using a whole-blood interferon-gamma assay based on Mycobacterium tuberculosis-specific antigens, and to identify risk factors for LTBI among military personnel in contact with patients with active pulmonary TB.
METHODS: This prospective study investigated contacts in military camps in South Korea of soldiers with active pulmonary TB. The whole-blood interferon-gamma assay was performed using QuantiFERON TB-Gold.

A 22-year-old woman was referred to our hospital suffering from repeated exacerbation of infiltrates confined to the medial portion of the right lower lung lobe, suggestive of pulmonary sequestration. Angiography revealed an anomalous vessel originating from the left side of the descending aorta, flowing behind it into the right lower lobe, the blood being returned to the pulmonary veins. A resection of the right lower lobe was conducted via thoracotomy. However, since there was no clear normal lung-pleura interface, this was classified as Pryce II type intralobar sequestration. BACKGROUND: Although household contacts of patients with tuberculosis are known to be particularly vulnerable to tuberculosis, the published evidence focused on this group at high risk within the low-income and middle-income country context remains sparse. Using nationwide data from Brazil, we aimed to estimate the incidence and investigate the socioeconomic and clinical determinants of tuberculosis in a cohort of contacts of tuberculosis patients.
METHODS: In this cohort study, we linked individual socioeconomic and demographic data from the 100 Million Brazilian Cohort to mortality data and
