We report a rare case of granulomatous balanoposthitis after intravesical Bacillus-Calmette-Guerin (BCG) instillation therapy in a 58-year-old man, which followed transurethral resection (TUR) for recurrent bladder cancer, when his anterior urethra was slightly narrow and his foreskin was with phimosis. Erosion of the mesh into the urethra is rare, usually presenting with hematuria, pain, voiding dysfunction or urge incontinence. OBJECTIVE: To evaluate a pudendal-thigh island flap for vaginal reconstruction.
METHODS: Forty-seven patients with congenital absence of vagina were undergoing the treatment. Based on the pedicle including the posterior labial neurovascular bundle, a pudendal-thigh island flap was designed and raised in the groin crease just lateral to the labia majora under the deep fascia. Urethral diverticulum is a rare disease, arising in the urethrovaginal septum and communicating with the urethra through an orifice. Characterised by clinical polymorphism, it is diagnosed by urethrocystography and transvaginal ultrasound. We report a series of 15 cases of urethral diverticulum over a 10-year period. MATERIALS AND METHODS: We studied 10 patients followed from 1 to 18 years. This article describes the technique and early results of antegrade collagen injection into the bladder neck and proximal urethra of men with stress incontinence postprostatectomy. Twenty men underwent collagen injection under general anesthesia. Follow-up ranges from 5 to 15 months, with a mean of 9.5 months. A mean of 14.5 cc of collagen was injected (range 5-25 cc). Using a submucosal needle tunneling technique for ureteral implantation, detubularized folded O-shaped ileal bladders were created for seven patients in need of bladder replacement. A segment of ileum of only 300 to 400 mm in length was required. Anastomosis of the reservoir to the urethra was simply performed by tailoring the ileum to reduce tension at the anastomosis site. The long-term results of Burch colposuspension for stress urinary incontinence were evaluated in 153 women operated on in 1984-1990. At follow-up 2 months postoperatively, 88% of the women reported continence, 5% were almost continent and the failure rate was 7%. At 24 months the corresponding figures were 86.7 and 7% and 39-102 months (mean 5 years) postoperatively they were 78, 11 and 11%. There were few complications.
