Early endoscopic realignment in posterior urethral injuries.

BACKGROUND: Posterior urethral injury requires meticulous tertiary care and optimum expertise to manage successfully. The aim of our study is to describe our experiences with pelvic injuries involving posterior urethra and their outcome after early endoscopic realignment. METHODS: A prospective study was carried out in 20 patients with complete posterior urethral rupture, from November 2007 till October 2010. They presented with blunt traumatic pelvic fracture and underwent primary realignment of posterior urethra in our institute. The definitive diagnosis of urethral rupture was made after r

Manufacturing high quality urinary catheters.

Forty-four percent of hospital patients with indwelling catheters can develop significant bacteriuria within 72 hours of catheterisation. It is, therefore, critical that catheters are manufactured with perfectly smooth surfaces and apertures to reduce the potential for infection. An ultrasonic cutting technology is described here that produces smooth apertures and eliminates problems associated with debris.

Use of quorum sensing antagonists to deter the formation of crystalline Proteus mirabilis biofilms.

Proteus mirabilis biofilms are a major cause of urinary catheter blockage. Antibiotic-impregnated catheters used to prevent catheter blockage have achieved limited success. Research has examined the efficacy of quorum sensing inhibitors against Pseudomonas aeruginosa biofilms, but there are few reports of the effects of these compounds against crystalline P. mirabilis biofilms. This study examined the effect of two quorum sensing antagonists, p-nitrophenyl glycerol (PNPG) and tannic acid, against crystalline P. mirabilis biofilms. Tannic acid and PNPG were observed to inhibit the quorum sensin

Robot-assisted laparoscopic bladder diverticulectomy combined with photoselective vaporization of prostate: a case report and review of literature.

PURPOSE: Open surgery, endoscopic technique, and standard laparoscopic technique are surgical options for the management of bladder diverticuli. In this article, we report robot-assisted bladder diverticulectomy (RABD) and photoselective vaporization of prostate (PVP) in the same patient sequentially. To the best of our knowledge, this is the first case report of RABD combined with PVP. MATERIALS AND METHODS: A 63-year-old patient with benign prostatic hyperplasia and a secondary large bladder diverticulum underwent sequential PVP and RABD. Cystoscopic examination revealed obstructing prostat

Adherence of Pseudomonas aeruginosa and Candida albicans to urinary catheters.

In this study, the in vitro adherence capabilities of Pseudomonas aeruginosa and Candida albicans clinical isolates to urinary catheters were investigated. Quantitative analysis was performed by colony-forming unit counts and scanning electron microscopy. Results demonstrated that the adherence of P. aeruginosa to urinary catheters was enhanced in the presence of C. albicans, while C. albicans adherence was not significantly affected. Further investigations are warranted to fully understand the pathogenic potential of their interaction in order to aid in the design of novel strategies for the

Influence of the great earthquake in the Chuetsu district on patients managing urination with clean intermittent self-urethral catheterization.

Natural disasters cause several medical problems for victims, especially for physically challenged people. The great earthquake in the Chuetsu district in Niigata Prefecture that occurred on October 23, 2004 caused serious damage in the disaster area. We investigated the effect of this earthquake on patients managing urination with clean intermittent self-urethral catheterization (CIC) using a questionnaire by mailing. The questionnaire was collected from 86 patients (60 men, 25 women, one sex unknown). The average age was 64.6 years old (males 64.1, females 65.8). By the first week after the

Introducing a novel technique to remove accidentally stitched or entrapped urethral catheters after radical prostatectomy.

INTRODUCTION: A crucial step during radical prostatectomy is anastomosing the bladder neck to the urethral stump after specimen removal. For this purpose, a Foley catheter is inserted transurethrally to achieve a patent and watertight anastomosis. The catheter should be removed 6-10 days after the procedure to avoid ascending infection and stricture formation. On occasion, catheter removal is not possible due to fixation by one of the anastomotic sutures. In this case, a longer catheterization period may increase the risk of infection and formation of an anastomotic stricture. MATERIALS AND M

Safety in urine sampling: maintaining an infection-free environment.

The insertion of an indwelling urinary catheter is thought to be implicated in 80% of all urinary tract infections (UTI). In patients with an indwelling catheter, one of the most effective ways to determine whether or not there is evidence of a UTI is to aspirate a sample of urine for analysis, typically using a syringe and needle. Inevitably this puts the healthcare worker at risk of a percutaneous needle stick injury and accidental exposure to serious and potentially life-threatening viruses. Infection control is now taken increasingly seriously in UK hospitals, and the elimination of infect

Short-term catheterization after TIP repair in distal hypospadias: who are the best candidates?

Over the last decade, tubularized incised plate (TIP) urethroplasty has become the first choice of surgical technique in patients with distal hypospadias. Despite the excellent cosmetic and functional results, prolonged catheterization (7-14 days) remains the main disadvantage of the TIP repair. In this study, we investigated the outcomes of the short-term catheterization in children with distal hypospadias in order to elucidate any relationship between the length of catheterization and the patients' age, meatal localization and postoperative complication rates. The charts of 183 patients who

Continuous needle vented foley catheter suction for urinary leak after radical prostatectomy.

PURPOSE: Prolonged urinary leakage from the urethrovesical anastomotic site after radical retropubic prostatectomy is a relatively uncommon complication. After the failure of other conservative measures we successfully overcame this problem using a Foley catheter needle vented suction technique. MATERIALS AND METHODS: Anastomotic leakage was noted after retropubic prostatectomy. After the failure of conservative techniques, including catheter traction, passive drainage and active catheter suction, needle vented suction was used. An 18 gauge needle was inserted into the plastic suction tubing

Per urethra insertion of a through-and-through safety guide wire using a dual-lumen ureteral catheter during percutaneous nephrolithotripsy.

BACKGROUND: The second ("safety") of two guide wires is commonly inserted antegradely in percutaneous nephrolithotripsy (PCNL). We describe the retrograde insertion of a through-and-through safety guide wire during PCNL. METHODS: After inserting a percutaneous nephrostomy 1 day earlier, a guide wire was introduced through the nephrostomy tube to the bladder and pulled out through the urethral meatus via a cystoscope and grasper. The tube was removed, leaving the wire positioned from the flank to the meatus. A dual-lumen catheter was introduced retrogradely through the urethra over the wire, u

Osteomyelitis of the pubis: a complication of a chronic indwelling catheter.

Pubic osteomyelitis typically occurs after pelvic surgery or trauma. We present a case of pubic osteomyelitis ensuing from a chronic indwelling (for 8 years) urethral catheter in a 40-year-old woman. She presented initially with fever of unknown origin, and broad-spectrum antimicrobial therapy was initiated. Computed tomography of the abdomen and pelvis ultimately revealed cortical destruction of the pubic symphysis, and open biopsy confirmed osteomyelitis. Osteomyelitis of the pubis is a rare entity that typically results from bacteremia, trauma, or the spread of an adjacent focus of infectio

The use of a low-profile cystostomy tube to relieve urethral obstruction in a dog.

A 10-year-old, spayed female Dalmatian was diagnosed with granulomatous urethritis causing urethral obstruction. Due to the extensive involvement of the urethra, a urethrostomy was not possible. A commercially available, silicone, low-profile gastrostomy tube was placed as a prepubic cystostomy tube to achieve urinary diversion. This tube is easy to use, has a one-way valve, and lies flush with the skin margin, thereby decreasing the likelihood of inadvertent removal. This tube should be considered to achieve long-term urinary diversion when urethral involvement is extensive.

Transurethral retrieval of a proximally dislodged ureteral stent.

A proximally migrated ureteral stent into the renal pelvis is an uncommon problem that poses a difficult technical challenge. A 32-year-old woman had her ureteral stent inadvertently dislodged into the renal pelvis at the time of stent exchange. The stent was successfully retrieved and repositioned transurethrally with a snare fed through the left ureter and into the renal pelvis. This is a relatively noninvasive method of retrograde retrieval of a proximally dislodged ureteral stent.

The quality of hospital records of urethral catheterisation.

BACKGROUND: This prospective and retrospective study carried out over a period of one month (50 patients) looked at the quality of hospital records of urethral catheterisation with reference to the hospital protocol. The patients were catheterised on general surgical wards as well as in theatre. RESULTS: The quality of hospital records of this procedure was poor and a number of complications noted. CONCLUSIONS: Good documentation of urethral catheterisation would result in a better quality of audit and, therefore, identify potential ways of minimising complications. We recommend this by regu
