Isolated Arthritis of the Temporomandibular Joint as the Initial Manifestation of Juvenile Idiopathic Arthritis.

OBJECTIVE: To describe characteristics of patients with juvenile idiopathic arthritis (JIA) presenting with isolated arthritis of the temporomandibular joints (TMJ). METHODS: Patients with JIA with isolated TMJ arthritis from 4 large tertiary pediatric rheumatology centers were included. Demographic and clinical data were analyzed using descriptive statistics. RESULTS: Fifty-five patients were identified (65% bilateral presentation). Six patients developed arthritis in other joints (median time 6 mos); 4 patients developed uveitis, all prior to arthritis. At last followup, 9% were still taki…

Sarcoidosis Presenting as Bilateral Vocal Cord Paralysis due to Bilateral Vagal Nerve Involvement.

We herein report a rare case of sarcoidosis presenting as bilateral vocal cord paralysis due to bilateral vagal nerve involvement. A 72-year-old woman with uveitis of the left eye complained of hoarseness and aspiration due to bilateral vocal cord paralysis. An endobronchial needle aspiration biopsy specimen of the mediastinal lymph nodes showed non-caseating epithelioid cell granuloma. Total protein and cell concentrations in the cerebrospinal fluid were increased. We diagnosed her to have sarcoidosis with bilateral vagal nerve involvement. Corticosteroid therapy improved her symptoms of hoar…

Identifying and Managing Complications and Comorbidities in Patients With Psoriasis.

The common comorbidities of cutaneous psoriasis include psoriatic arthritis (PsA), Crohn's disease, uveitis, and depression. In addition, cardiovascular disease risk factors (including metabolic syndrome) are seen more frequently among patients with psoriasis, and strong epidemiologic evidence has demonstrated that psoriasis is independently associated with myocardial infarction. Because these comorbid conditions and other medical complications adversely affect morbidity and mortality in patients with psoriasis, dermatologists can play an important role in promptly identifying and, when necess…

Bilateral Knee Pain Associated with Bone Infarction in a Patient with Behcet's Disease.

We describe a 31-years-old female patient with severe pain in both knees who had been diagnosed as Behcet's disease (BD) for 12 years. She had had a history of complications due to BD including superior vena cava thrombosis, pulmonary thromboembolism, uveitis, and erythema nodosum and has reported the administration of corticosteroid therapy irregularly. After radiologic evaluation, she has been diagnosed with bone infarction of both left and right knee with the existance of lupus anticoagulants (LA) positivity. Severe joint pain without the evidence of arthritis must alert the clinician to th…

Orbital inflammatory disease: unusual presentation of enthesitis in an HLA-B27 spondyloarthropathy.

BACKGROUND: Orbital inflammatory disease can complicate many systemic inflammatory disorders, including sarcoidosis, vasculitis, Crohn's disease, systemic lupus erythematosus, rheumatoid arthritis, myasthenia gravis and scleroderma, but has not been reported with spondyloarthropathies. OBSERVATIONS: The authors describe a 29-year-old woman who developed orbital myositis, in addition to anterior uveitis, sacroiliitis and peripheral arthritis, as a complication of an underlying HLA-B27 related spondyloarthropathy, which responded temporarily to corticosteroid therapy and more completely to adal…

Diagnosis and treatment of enthesitis-related arthritis.

Juvenile idiopathic arthritis (JIA) is a chronic, inflammatory disease of unknown etiology. The enthesitis-related arthritis (ERA) JIA category describes a clinically heterogeneous group of children including some who have predominately enthesitis, enthesitis and arthritis, juvenile ankylosing spondylitis, or inflammatory bowel disease-associated arthropathy. ERA accounts for 10%-20% of JIA. Common clinical manifestations of ERA include arthritis, enthesitis, and acute anterior uveitis. Axial disease is also common in children with established ERA. Treatment regimens for ERA, many of them base…

Cochlear implantation in a patient with atypical Cogan's syndrome complicated with hypertrophic cranial pachymeningitis.

A 55-year-old woman had bilateral sensorineural hearing loss (SNHL), vertigo, uveitis, and aortitis associated with Cogan's syndrome (CS). She had a history of listeria meningitis and hypertrophic cranial pachymeningitis (HCP), both of which were considered to be related to SNHL progression. She developed bilateral profound deafness within 1 year, despite medical treatment with corticosteroids and methotrexate (MTX). She underwent cochlear implantation (CI) of the left ear. Although the left and right basal turns of the cochleae were ossified, all electrodes were successfully inserted, and sub…

[A case of Heerfordt's syndrome with an elevated serum TNF alpha].

A 27-year old man who had developed uveitis, swelling of the right parotid gland, right facial nerve paralysis and fever, was admitted to our hospital. A chest X-ray film showed bilateral hilar lymphadenopathy. Serum levels of angiotensin-converting enzyme and lysozyme were elevated. Histological findings of transbronchial lung biopsy specimens showed non-caseous epithelioid cell granulomas. Therefore, sarcoidosis was diagnosed. This case also fulfilled the criteria for Heerfordt's syndrome. Adiministration of prednisolone (PSL) initially improved the patient's symptoms, but rapid tapering of …

High-grade lymphoma after azathioprine treatment for Vogt-Kaganayi-Harada syndrome.

Vogt-Kaganayi-Harada (VKH) syndrome is a rare autoimmune disease characterized by panuveitis, neuropathy and aseptic meningitis. Most patients require long-term treatment with steroids and immunosuppressants. Patients may develop concurrent autoimmune diseases, especially endocrinopathies. Secondary malignancies are rare associations. We report a Chinese man with VKH syndrome presenting with multiple cranial nerve palsy and bilateral pan-uveitis, who developed disseminated high-grade B cell lymphoma after 3-year treatment with azathioprine. This is the first report of systemic non-Hodgkin lymp…

Neuro-Behçet's disease--rapidly reversible with early diagnosis and treatment.

Behçet's disease is an idiopathic multi-systemic, inflammatory disorder that may affect major organs, including the central nervous system. It is a clinical diagnosis, and a history of recurrent oral and genital ulcers should be elicited, especially in cases of unexplained uveitis with atypical neurological symptoms. We describe the rapid neurological recovery and neuroimaging changes following high-dose systemic corticosteroids in a 44-year-old man with recurrent oral ulcers, panuveitis, and acute neuro-Behçet's, presenting with meningoencephalitis. Early diagnosis with the aid of appropriate…

[Bilateral serous retinal detachments in acute posterior multifocal placoid pigmentous epitheliopathy].

PURPOSE/METHOD: A particular subtype of acute posterior multifocal placoid pigmented epitheliopathy (APMPPE), different from the classical Gass description, is presented. A 22 year old woman who suffered a sudden decrease in visual acuity associated to bilateral serous retinal detachment concurrent with anterior uveitis is studied. We remark the fact that patient had been using contraceptives and antibiotics (norfloxacin) for the past two months prior to first visit. One month later, visual acuity does not show any recovery. A corticosteroid treatment is prescribed showing improvement. RESULT…

[Acquired ocular toxoplasmosis (panuveitis) after liver transplantation].

We present one case of acquired Toxoplasma gondii retinochoroiditis after livrer transplantation. Three weeks after receiving liver transplant from a seropositive donor for Toxoplasma gondii, the fundus examination showed a vitreal haze and an edematous retinal lesion with retinal hemorrhages. Routine screening tests for uveitis were performed. The serum toxoplasmosis titers revealed a seroconversion for toxoplasmosis (pre-transplantation tests were negative). The PCR assay on aqueous humor resulted in an amplified product identical to toxoplasmosis. This immunocompromised patient likely recei…

Ocular manifestations of Wegener's granulomatosis in north India.

Evidence of ocular involvement was found in seven of eleven patients with histologically confirmed Wegener's granulomatosis. Pain (7/7), redness (6/7), scleritis (5/7), reduced visual acuity (5/7), episcleritis (5/7), uveitis (3/7), proptosis, sclerokeratitis and conjunctivitis (2/7) each were the major ophthalmological features. One patient each had bilateral corneoscleral ulcers, eyelid infiltration, scleromalacia perforans and epiphora. Corneal, scleral and uveal tract involvement was observed more frequently than has been described in the literature. These complications responded favourabl…

Ankylosing spondylitis is part of a multifactorial syndrome: hereditary multifocal relapsing inflammation (HEMRI).

Complex associations between the different subgroups of seronegative arthritis, acute anterior uveitis, psoriasis and inflammatory bowel disease were found in a series of patients and relatives. Different HLA-B antigens were associated with different clinical signs. The results indicated the existence of a multigenic syndrome whose main clinical manifestations are relapsing innflammations at various sites. The genetic factors appeared to be 1) HLA-B27 associated disease susceptibility, 2) predisposition to psoriatic arthropathy, 3) predisposition to early onset familial psoriasis and, 4) a pro…

[Herpes therapy and prophylaxis. I. A critical review (author's transl)].

This critical review is based upon controlled experimental and clinical data. Dendritic keratitis initially should be treated by debridement of the diseased epithelium followed by antiviral medication. The advantages and disadvantages of different debridement techniques and different synthetic antivirals are discussed. Rational treatment of other forms of herpetic eye disease with antivirals, steroids, therapeutic soft lenses, collagenase inhibitors etc. necessitates first of all an exact diagnosis (disciform edema, interstitial herpetic keratitis, herpetic (kerato-)uveitis, metaherpetic erosi…

Exudative retinal detachment and scleritis in polyarteritis.

A 64-year-old white man, treated with systemic corticosteroids for five years, developed polyarteritis. He then developed a severe scleritis with an exudative retinal detachment in the right eye, which became blind and painful and was enucleated 23 months after onset of the scleritis. Histopathologic examination of the enucleated eye revealed granulomatous scleritis, chronic nongranulomatous uveitis, exudative retinal detachment, and perivasculitis of intrascleral, iris, ciliary body, and retinal blood vessels. Systemic findings were minimal and limited to elevated sedimentation rate, weight l…
