Chamber-specific regulation of myofilament Ca2+ sensitivity by nNOSβ via troponin I phosphorylation: Predominance in the right ventricle.

Neuronal nitric oxide (NO) synthase (nNOS) is an important regulator of excitation-contraction coupling in cardiomyocytes, yet its chamber-specific role in right ventricular cardiomyocytes (RVCMs) remains incompletely understood. In this study, we compared the effects of nNOS inhibition on contractile function, Ca2+ signaling, and myofilament-associated regulation in RVCMs and left ventricular cardiomyocytes (LVCMs). Pharmacological inhibition of nNOS with S-methyl-L-thiocitrulline (SMTC) enhanced sarcomere shortening and impaired relaxation in both cell types, with markedly greater effects ob

Successful Percutaneous Revascularization for Acute Myocardial Infarction From Left Main Occlusion and Concomitant Right Coronary Artery Chronic Total Occlusion: A Case Report.

Despite its rarity, acute occlusion of the left main coronary artery is associated with an exceedingly high mortality rate. This report describes a critical case of acute myocardial infarction (AMI) caused by acute total occlusion of the left main (LM) artery, complicated by a chronic total occlusion (CTO) of the proximal right coronary artery (RCA). A 55-year-old Chinese male with hypertension and a heavy smoking history presented with sudden chest pain. The initial electrocardiogram (ECG) showed ST-segment elevation in leads V1-V6 and aVR, indicative of anterior wall AMI, and the patient was

Detecting early structural and functional myocardial alterations in patients with repaired tetralogy of fallot: a prospective MRI study.

In patients with repaired tetralogy of Fallot (rTOF), right ventricular (RV) fibrosis has been associated with ventricular tachycardia, heart failure, and sudden cardiac death. Here, we aimed to identify links between diffuse myocardial fibrosis and regional mechanical dysfunction by cardiac magnetic resonance imaging (CMR) in rTOF. Clinical CMR was extended in 17 consecutive rTOF patients (median age 20 years, 4 females) to include biventricular native T1 (nT1) mapping as indicator of diffuse fibrosis, and tissue phase mapping (TPM) for quantification of biventricular segmental, directional s

Case report of antepartum and postpartum spontaneous coronary artery dissection in a high-risk pregnancy.

BACKGROUND: Spontaneous coronary artery dissection (SCAD) is a rare, non-atherosclerotic cause of acute coronary syndrome (ACS) that mainly affects women, particularly during or after pregnancy. CASE SUMMARY: A 38-year-old woman conceived via in vitro fertilization presented at 34 weeks' gestation with chest pain and rising troponin levels (191 ng/L → 5500 ng/L; reference <52 ng/L). Initially patient was elected for medical management. Day later telemetry detected frequent non-sustained ventricular tachycardia, considered a high-risk feature for underlying SCAD. Coronary angiography demonstra

Survival Beyond the Odds: Twelve-Week Follow-Up After Conservative Management of Post-Infarction Ventricular Free Wall Rupture.

BACKGROUND: Left ventricular free wall rupture (LVFWR) is a rare but often fatal complication of acute myocardial infarction. While urgent surgical repair remains the gold standard, isolated reports suggest that selected hemodynamically stable patients may survive with conservative management. CASE SUMMARY: We describe a 60-year-old man who presented with an inferior STEMI after three days of chest pain. Echocardiography and CT cardiac angiography confirmed a basal inferolateral LV free wall rupture with hemopericardium and clot formation. Despite elevated lactate and angiographic evidence of

Danazol causes significant changes in the cardiometabolic profile of patients with acquired aplastic anaemia.

BACKGROUND: Danazol is frequently used in treating patients with acquired aplastic anaemia (AA) in resource-constraint settings. We aimed to evaluate the cardiometabolic side effects of Danazol in patients with AA. METHODS: This prospective study included newly-diagnosed AA patients ≥13 years of age who were eligible for Danazol monotherapy (10 mg/kg/day, capped at 600 mg/day). Lipid profile and two-dimensional echocardiogram were obtained at the baseline and after 6 months of Danazol treatment. Transfusion of blood products and liver function test-based dose adjustments were done as indicate

Zero contrast technique in doubly committed subarterial ventricular septal defect closure in patient with body weight less than 10 kg: a case report.

INTRODUCTION: The complex anatomy of DCSA VSD, its proximity to valvular and conduction tissues, and concerns about radiation and contrast make transcatheter closure particularly challenging in pediatric patients. While zero-fluoroscopy closure was not achievable in this case, we successfully performed zero-contrast transcatheter closure in a baby weighing less than 10 kg. This report highlights the feasibility and early outcomes of this approach at our institution. CASE ILLUSTRATION: An 18-month-old, 9 kg boy with a history of feeding difficulties, failure to thrive, and breathlessness since

Post-traumatic Takotsubo Cardiomyopathy in a Pediatric Patient: A Rare Case and Diagnostic Challenge.

Takotsubo cardiomyopathy (TTC), also known as stress-induced cardiomyopathy, is a rare condition in children that causes acute, severe, but often reversible systolic dysfunction of the left ventricle. Physical trauma is a recognized trigger, although distinguishing TTC from myocardial contusion in pediatric trauma cases can be challenging due to overlapping clinical features. We present the case of a six-year-old boy involved in a high-impact motor vehicle collision. The patient initially presented with multiple traumatic injuries, including fractures of the skull, ribs, and right upper extrem

Sex differences in transthyretin cardiac amyloidosis.

Transthyretin cardiac amyloidosis (ATTR-CA) is a progressive disease characterized by the deposition of abnormal transthyretin protein fibrils in the heart, leading to cardiac dysfunction. Recent evidence suggests that sex differences may play a significant role in various steps of ATTR-CA, including clinical presentation, diagnostic challenges, disease progression, and treatment outcomes. ATTR-CA predominantly affects men, whereas women are older at presentation. Women generally present with a history of heart failure with preserved ejection fraction and/or carpal tunnel syndrome. A 45-year-old man with sudden cardiac death, cutaneous abnormalities and a rare desmoplakin mutation: a case report and literature review.

BACKGROUND: Arrhythmogenic cardiomyopathy (AC) is a rare, heritable myocardial disorder that is a leading cause of ventricular arrhythmia and sudden cardiac death (SCD) in young people. Desmoplakin (DSP) mutations account for 3-20% of AC cases. However, the number of patients with DSP mutations is extremely small in all published reports and genotype-phenotype correlations are scant and mostly non-gene-specific. CASE PRESENTATION: A 45-year-old man was admitted after an out-of-hospital cardiac arrest, with documented ventricular fibrillation. He had no previous history of heart disease or fam

Biventricular Pressure-Volume Loop Assessment Before and After Pulmonary Valve Replacement in Tetralogy of Fallot.

Patients with tetralogy of Fallot (TOF) may undergo pulmonary valve replacement (PVR) after initial full repair. We investigated indices of biventricular function, work and efficiency of TOF patients' using noninvasive pressure-volume (PV) loop analysis on cardiovascular magnetic resonance (CMR) images and compared pre-and post PVR groups. Biventricular segmentations of steady state free precession CMR images were performed using custom validated software (Segment version 2.0 R7067). Brachial cuff pressure estimated left ventricular (LV) systolic pressure. Right ventricular (RV) inputs were ob

Transcatheter ventricular septal rupture closure: A challenging case of basal inferoseptal aneurysm and rupture.

Ventricular septal rupture (VSR) is an ominous mechanical complication of acute myocardial infarction (MI) accompanied with a poor prognosis. Transcatheter closure (TCC) of VSR has been proposed as an alternative approach for surgery. This study presents a 79-year-old man with diabetes mellitus, hypertension, dyslipidemia, and chronic stable angina with 3-vessel coronary artery disease, who had refused a coronary artery bypass graft. He complained of orthopnea and dyspnea of New York Heart Association (NYHA) function class III after the recent neglected inferior MI. Transthoracic echocardiogra

SHROOM3 is downstream of the planar cell polarity pathway and loss-of-function results in congenital heart defects.

Congenital heart disease (CHD) is the most common birth defect, and the leading cause of death due to birth defects, yet causative molecular mechanisms remain mostly unknown. We previously implicated a novel CHD candidate gene, SHROOM3, in a patient with CHD. Using a Shroom3 gene trap knockout mouse (Shroom3gt/gt) we demonstrate that SHROOM3 is downstream of the noncanonical Wnt planar cell polarity signaling pathway (PCP) and loss-of-function causes cardiac defects. We demonstrate Shroom3 expression within cardiomyocytes of the ventricles and interventricular septum from E10.5 onward, as well

Hyperhomocysteinemia Alters Sinoatrial and Atrioventricular Nodal Function: Role of Magnesium in Attenuating These Effects.

Patients with hyperhomocysteinemia (HHcy), or elevated plasma homocysteine (Hcy), are at higher risk of developing arrhythmias and sudden cardiac death; however, the mechanisms are unknown. In this study, the effects of HHcy on sinus node function, atrioventricular conduction, and ventricular vulnerability were investigated by electrophysiological (EP) analysis, and the role of magnesium (Mg(2+)), an endogenous N-methyl-D-aspartate (NMDA) receptor antagonist, in attenuating EP changes due to HHcy was explored. Wild-type mice (WT) and mice receiving Hcy in the drinking water for 12 weeks (DW) w
